As a coding student (2 months in), I did not know this, but it's very good to know because as a transcriptionist all I see is the office visit or op report and sometimes wonder how it would be coded. Now that I know a coder has access to the rest of the patient's file and is able to communicate with the doctor, I understand the coding process better.

I think the post "as per usual..." was in response to the the comments by "Coder" and "Jofalltrades" being off topic a bit. My question wasn't what to do if the doc dictates an obvious error, it was what to do if as a coder I see a report where an obvious transcription error wasn't caught and has come through on the reports I'm trying to code from.
