Steri-Strips and s/l "light guard" sutures. (sm for another ??) - New to surgery

Posted: Feb 24, 2011

This is going to be a long day, I can feel it. I am covering for someone and she gave me samples, but this first doctor talks fast and so I am having an even more difficult time.  Thanks for your patience.

The catheter exit site was opened slightly with a small skin incision and dissection was performed to expose the anterior fascia. The catheter of the system was cut "by the neutral pleasure" and attached to the reservoir. Sorry, I have slowed him down, changed his tone, but he is just talking too fast.

The wound was irrigated, checked for venous stasis, and then the skin was closed with uninterrupted 4-0 Vicryl sutures, presented Steri-Strips and "white/light guard" dressings.

Title of operation and operative site would be helpful. - sm

Did he name the catheter or reservoir in the body of the report? What kind of procedure is this and where is it being performed?

It is a Lap-Band procedure sm - New to surgery

The catheter was replaced into the abdominal cavity and the reservoir was positioned into the subcutaneous pocket so that the mesh was in direct contact with the anterior fascia. The reservoir was inspected and appeared in excellent position. The wound was irrigated, checked for venous stasis, and then the skin was closed with uninterrupted 4-0 Vicryl sutures, presented Steri-Strips and _____________ dressings. The patient tolerated the procedure well and left the operating room in stable condition

Maybe light gauze dressing? - cr

S/l "terminating" With Skin Steri-Strips. Sm
Jun 27, 2013

Here is the whole sentence.  The site was irrigated thoroughly and then closed with nonabsorbable suture for the deltoid fascia, followed by absorbable suture in successive layers, s/l "terminating" with skin Steri-Strips.   I really don't even know if it sounds like terminating.  I hear what I think sounds like a "t" sound in the beginning and "ing" sound at the end.  He says is so fast, and I have tried to slow him down with no success.  THanks!  ...

Please Do Not Call Out The Guard
Dec 01, 2014

I mentioned some of the really bad things I noticed in my experiences as a transcriptionist. Please rest assured each and every item was taken care of in a proper manner.  There is no need, in these instances, to bring about any further action. Sometimes the gentlest word in the right place brings about tremendous, forward-moving and positive change. That is what happened here. Everyone, seek peace and stand up for yourselves at the same time. Not easy to do. :) ...

Hello! :) The doctor dictated something that sounds like "astake" strips, and I'm completely stumped. Some Light For MTs (I Think)
Feb 13, 2013

I have a friend who works at a local hospital and she told me that they are about to start outsourcing their central scheduling department.  She said it is a U.S. based company that will be answering calls, scheduling tests, etc.  She also said that they have already begun outsourcing the discharge followups and surveys.  It is bad for the hospital, as they are losing staff, but I guess it could be good for MTs who just don't want to leave their home. Maybe they could work fo ...
