How to type book reference in letter form? - traci

Letter to insurance saying why procedure is needed. In body, he references this book.  How to type?

Perez and Brady's Principle and Practice of Radiation Oncology, fifth edition, radiation oncology textbook, page 1622, also states

I do a lot of these types of letters for an ortho guy. I wouldn't change much except "Fifth Edition" is capped. Unless he referencing 2 separate books (the textbook and if that is the case you wouldn't cap it because he is just referring to as a general textbook but sort of sounded like he was just saying what the other book was), the rest of it looks fine.

Has anyone actually read all 500+ pages? Our group was told we were doing headings the wrong way for an account which is new to our group, but that was not listed in the CP. When questioned, we were directed to the BOS2. Fortunately, we were given the path to find that section. Although some of what was demanded was in the BOS2, other demands were not. Very frustrating. A few weeks ago, for no reason that I can find, I cannot get the first letter of a sentence to be in all caps automatically, (unless I do it manually.)  The same for the letter " I " by itself when typed.   It was always automatic and now all of a sudden it is doing this and I cannot figure out how to change it back. Can anyone tell me what the more preferred way to type the above sentence would be?   Also, should I not use commas before "dated" and after the date or should I just straight type it "The report dated 08/12/09 states...."  We have a few different opinions in the office (which only dictated and have somewhat poor use of the English language) and I would like to see what you, the MT community, thinks.   Thanks! ...

I worked as an IC transcriptionist in 2013 for approximately 4 months.   The company sent a W-9 form and is requesting it be signed and sent back.  I did not make a total of $600.00 when I was working for them, which I've understood from others that if you make less than $600.00 you don't need to send in a W-9 form.  Can someone clarify on this? Does anyone else have problems supplying work references?  It seems the jobs I'm interested in all very emphatically ask for work references that can be verified and I've worked at home for just two companies in the past 10 years and had no contact with any other employees besides my supervisors.  I definitely don't want to give the current employer as a reference so that leaves me one that I worked for six years ago.  Not sure what to do in this case. ...
