My clinic just went to EHR with eClinicalworks - unknown

Am I now out of a job or is there a place for transcription with this program?  Anybody have an idea of what to expect?

My small account went to emr and they use Iknowmed, old program, but we still do their stuff, we type it in word, and then paste it into the iknowmed system, works pretty good, it is just choppy, put in the hpi separate, sh separate and so on.

They have not said I am fired, but everything is a drop-down menu with a place to put "notes." There is no place to type a report, no blank page, no word processing feature. It is definitely NOT a transcription-friendly platform. I am so bummed. I really liked this job, but I like transcribing. I don't want to be a data-entry clerk trying to find the correct drop-down for "sharp, throbbing back pain an 8 on a scale of 1-10 radiating down the right leg in an anterior pattern almost to the toes." I told the trainer, my office manager, and the doctors I work for that I can type an ROS in probably 15 seconds, but it will take more like 15 minutes to take it off the patient form and try and find all the corresponding drop-downs and there is always the chance of missing something. Anyways.

You might want to reconsider that one and depending on how bad you need the job, be careful what you say to the trainer, your office manager and especially the doctors. You must be amazing to be able to transcribe a full ROS in 15 seconds; in my experience some of them are pretty lengthy. After my facility changed to an EHR, we had transcription although less than before. Our downfall was that they chose to outsource everything. I needed a job and I needed the benefits (years of seniority) so I kept my mouth shut and was offered another job within our organization. I am doing some things that I love, and some things that I don't, but that is how it goes with any job. I also love transcription but I knew my chances of finding another traditional transcription job were slim to none. Unless the tell you that you are fired, I would say to hang in there while you continue to look for something else. Depending on the size of the organization they may help you find an internal job doing something else, but not if you get on the wrong side of the docs and office manager.

The frustration factor is just so high. I left MModal and loved this job. I was making decent money--not as much as in the olden days, but not too bad--and now this. I do need to keep my mouth shut, do whatever the powers that be tell me to do, and try to keep making some kind of paycheck for as long as I can, even if it is for "point-and-click." At least I am paid by the hour and not production.

She puts all that into the EHR system when she interviews the patient before the doc sees.

There is no more transcription to it. At all. However, consultants still need to send out letters and responses to the primary care physicians and that will probably not end. Its a lot of copy/paste and then some transcription about history/progress. So far the labs still have to be dictated.

Look for that niche. Those positions are going to be sweet in the next few years. Half IT half MT.

FYI, the docs have a choice whether to do it like you are doing, each thing separate or as one note. I had one doctor who did it all separate, sometimes 20 parts for one note, would looked like I had 2 weeks of work in there sometimes and would freak out my boss, and two other docs who just did it in one dictation as an office note. I always liked IKM though, did not have to deal with anything but maybe move a note in the chart if the doctor put it under the wrong section.

also is to do each section separately, which seems so much slower cutting and pasting each section from Word to the EMR. I asked if we could not just have a blank template and do the document in Word and copy and past the whole thing into EMR. The trainer (not American, but don't know that that is pertinent) looked at me like I was insane to suggest such. I also asked if I could type the document and scan it into the patient's file instead of printing it. I think there were systems out there that were more transcription-friendly, but our small clinic is part of a larger group and were told to use this one and it is drop-down menus and point-and-click diagnoses. I guess I will keep trying to find a way to make it work. Just depressing. I knew the news wasn't good, but wondered if anybody else in a smallish facility had come across this problem and won. Our clinic went ECW. Told would still have job and needed and we were scolded for thinking differently. Two months later, after the medical assistants and doctors (it uses Dragon Medical) used combined drop-down, click/point, and DM dictation, we got laid off....all of us. Sigh. Money talks. Budget cuts and drs/bosses not wanting their pay cut, so we were.

will not even acknowledge transcription. He is pushing hard for the doctors to enter everything and when they were not enthusiastic, he came onboard with the Dragon---Dragon this, Dragon that, it is a miracle product, no mistakes, can do it all. This is right in front of me. I am so depressed---and not old enough to retire--but too old for a new career. I really thought this job was a good one. Stupid me.

oh my. this sounds exactly how my training session was and yes point-blank said 'there is no transcription'.

I lost my primary accounts recently and was switched to an account where most of the jobs are really short clinic-type notes (less than a minute).  This pay period, my paycheck will be over $200 less than it was when I had my primary accounts, and I'm barely making my minimum line counts.  I've been doing this job for a long time and have always had really good line counts before.  I've been feely really down in the dumps like maybe I've just kind of lost ...

with 1 other girl, and then the QA girl types on this clinic also.  I still have not figured this out.  We keep the work caught up and this QA girl gets on there and types with us when there is nothing to type.  I mean there can be 10 jobs there and she will get on to help us type 10 jobs.  My company makes me type any other clinic that is behind and this being the holiday, we have been given an extra 45-50 jobs to type from other clinics and the QA girl is nowhere to be seen ...

Has anyone started to get more work in the last year (in a clinic setting)? It was steady for me a few years ago, and now I am overwhelmed with the amount of dictation I have!! Each of the doctors I work for are seeing wayyy too many patients in one day, thus more dictating, etc.  Im putting in like 10-12 hour days now, whereas I used to only work 5 or 6.
