At home MTs - - QA Question for you

What do your QA people actually do? 

I have two jobs, as I've said before.  Both outsource pieces of their work and at both I wonder just what it is that the QA people do?  At both I really wonder how the MT even got their job at all but I really wonder what the QA people do!

At the hospital, we send the op reports out, probably 60 a day.  They get done by a company owned by the former Q but they claim they're done in America.  Every day we come in to 50 of them in our own QA for us to fill in the blank.  Our American doctors, the ones we used to fly through and love because they were so good, usually one have a few blanks but the ESL guys have anywhere from 5 to 8 to 12 blanks each.  That's right, you read that right, 12 blanks in one document.  So, what is the QA department at the MTSO actually for, if not to check the work of the MT who isn't actually doing the report in the first place? 

Now, the clinic is where I really swear I'm going to go bald one day.  This girl, I want to hunt her down and....serenity now....serentiy now.  I know for a fact she herself, personally, as an MT, has been told at least 10 times because when we hired this company we got to talk to her more than 10 times lol, about formatting (and we're not picky, I swear.  We're not one of those clients who nitpick every little thing at all...you'll see!!).  Anyway, she does this thing....heading, HISTORY OF PRESENT ILLNESS, text under it, four spaces under that text, next heading REVIEW OF SYSTEMS, text under it, two spaces under that text, next heading PAST MEDICAL HISTORY, text under it, three spaces under that text, next heading PAST SURGICAL HISTORY, text under it, four spaces...you see where I'm going with that?  It drives me crazy. 

At the top of the report, we have to do this whole header thing with the patient's name, vitals, DOB, vaccinations, etc.  There's a format for it...top line, date of visit, name, date of birth.  She constantly, constantly does 2/8/12 John SmithDOB then returns and puts the actual DOB on the next line and goes on about her merry way.  She's been told....a LOT.  Does it really take that long to fix it?  No, it does not.  It's not a formatting thing in the system itself because we use the same exact system here.

Where is the QA?  Why don't THEY fix it?  Why don't they TELL her to fix it?  Why don't they gig her for it? 

And she insists on putting her cc under the document information.  Every single thing she types we have to go into and almost redo the entire thing.  It would almost be faster to type it myself.  It drives me crazy.

I have only worked for a couple of services, both national, but when working for them their QA department was there to fill in blanks & to spot-check or QA our work (work that had already been done which they reviewed for errors & gave us "grades" on). When initially hired, an MT was on "full QA" meaning everything they did was reviewed before being submitted to the client hospital. However, once you were given the green light to go off full QA, your reports went directly to the client hospital without going through QA unless you had X number of blanks. There was an established "acceptable" number of blanks that the client hospital would accept, otherwise the report had to be submitted to QA first. (If memory serves, this acceptable number was around 2-3 -- depended on the client hospital and what they contracted with the service.)

That would be my point to you -- the answers to your questions depend on what the contract is between the hospital and/or clinic and the MTSO(s) they are using.

You did not say what your position is at either the hospital or the clinic. The hospital's HIM director & transcription manager or office manager of the clinic should be made aware of the poor quality of work being submitted by the MTSO and they should be contacting the service(s) if there is a concern.

When I worked in-house at the hospital & did QA, we also outsourced a little. We did not have the ability to even review or fill in blanks of the MTSO work before it was submitted. What they typed was submitted straight into the system. (and if they had blanks, the report was submitted with blanks. Fortunately, it was never a problem, but we would have preferred being able to fill in their blanks.) However, we QA'd their work & sent them back "grade cards" if you will (sent to the MTSO management and to the MT herself), just as we did our hospital MTs. And, when we had one particularly unsatisfactory MT from the service, we requested the service take her off our account - which they did.

Bottom line - The service is an employee of the hospital/clinic & if their work is unsatisfactory, they need to be told & the situation remedied, just as it would be done with an MT working directly for the hospital/clinic.

Was laid off in April.  Have a couple offers from transcription companies, and one from a clinic which is part of a huge school/hospital system in our area, (which actually is through a temp agency).  There are only 2 full-time NP's and a handlful of part-time NP's.  The hospital job is a 45-minute drive, 1-way, in light traffic.  Pay to start is $13, but if this temp goes to permanent, potential is much greater.  The clinic is open M-F, 8-4, closed holidays.&n ...

Love my job as an at-home MT and love working from home, but that set aside, I think we can all agree that if you're looking to make good money, this is likely not the profession to pursue anymore.  So I saw this ad in one of my magazines, August issue:Train at home to Work at Home . . .
