I am an MTSO and see the WMX/Nuance merger as an opportunity to improve employee relations.  Until the last few months, it seemed the Webmedx could do no wrong.  Employees were happy and seemed proud to work there.  Then it changed, obviously as they were gearing up for the sale. 

So, as a growing MTSO, I want to ask you this:  What did they do that made people so happy?  What can other MTSOs do to become the "perfect" service to work for?

I am not looking for crazy answers saying that we need to pay 15 cpl and have total flexible hours and days and no timeclock and free insurance with 8 weeks of PTO. 

I am looking for real advice.  What is a fair line rate, in your opinion, for an employee on a 65 character line?  What is an attainable production requirement?  What is a fair rate for VR editing?  What would you like to see in a service? 

We are a good-sized company and have good and bad people and good and bad policies, but honestly, it has all been by trial and error.  Yes, I have a degree in business and was a transcriptionist prior to opening my business, but that is not all it takes.  There is no school or training program for MTSOs.  We learn through the mistakes of others.  I think it is time to turn that around and be proactive rather than reactive. 

I would love to hear from transcriptionists, QA, editors, managers.  What would it take to have a great company, to go from good to great?

First is pay well, especially the truly experienced MTs, not ones who have worked 20 or 30 years, but ones who have worked that long and can produce quality work no matter what the specialty! Do not overhire, making sure work is there. Since you said not to be crazy about advice, then I won't say MT pay should not fallen and still making same cpl as 10 years ago (okay, I just did, sorry:)), but as far as pay, 10 cpl at the least, period, for a quality MT who you don't have to babysit, and I mean the least is 10 cpl. In past years, I've worked for MTSO who paid up to 13 cpl and local contractors who paid me that too, but realistically that is at the top of the scale nowadays but said just to give you a picture of what is good pay for subcontractor MTs. Thanks for asking. Those are my top 2 areas.

I would suggest that in addition to everything MetroC said, to enlist the AID of your MTS, especially the experienced MTS. They can actually HELP YOU. They are smart and educated, and they have ideas and if you engage to aid, you might find a treasure trove of ideas that can aid in running your company more smoothly. There is way too much of an attitude of 'just shut up and type' in this industry, very insulting to the very individuals upon which you depend to make it happen. There are many industries where anyone can come in off the street and learn a skill in a week or 2 to take over a job. As we all know, not here. These are skilled specialists, and it is insulting to be treated as though we have nothing to contribute other than to 'shut up and type and be glad you have a job'. MTSO's should be glad they have us...without us...there is no MTSO. That's all....respect...mutual respect (and 15 cpl - just kidding) :)

I agree with both "My 2 cents" posts. Respect and decent wages are what makes a company great. Treat your employees how you would like to be treated. Would you sit around with no work and get no pay? MTs are expected to work for low wages and be available whenever the MTSO needs them. Respect goes a long way. If an employee feels valued and respected, they will go the extra mile for the company. Pay should be at least 12 cpl for experienced MTs.

I would love to pay every one of our employees 12 cpl. However, if you do the math, 12 cpl plus taxes (employee portion inc UE) = 14.5cpl. Most hospitals and clinics are now looking at companies that charge 13 and 14 cpl. Before you say it, it has nothing to do with standing our ground. That is the going rate now, and it is probably not going back up.

Maybe it's just time to give up and sell to MQ or Transcend; it seems that they can somehow make it work but taxes and benefits and overhead are killing me. Overhead is part of life, and I accept that, and there is more to it than most people realize (accountants, tax preparation, clerical help, account managers, sales commission, taxes, insurance, rent, phones, electric, head and A/C, water, benefits, bank fees, etc) and before you know it, that 2 to 3 cpl that you thought was so great of a margin is actually -2 to -3 cpl.

Average incomes have now dropped below what is required to keep people in the industry and attract new workers. Normally speaking, anyone who can do this work can transition pretty quickly to work that will pay better than this. I'm sure that if it weren't for the dreadful job markets many more would have left already; of course, in that case likely rates could not have fallen so low.

The kind of skilled people you want NEED to be able to make, what, a minimum of $35K per year?, to support themselves. Other middle-range clerical positions pay that to established employees.

Of course, in a production pay environment, faster people have to be able to make more than that, or why bother to be faster? Or why not go where their industry will pay off? Not rewarding high production also causes problems: when someone can produce well working very intensively 5 or 6 hours a day without sufficient incentive to continue, BUT the employer insists on a full 8 hours--record fudging and threatening memos time. At WMX, the more and more you produced the higher and higher your line rate went thanks to a good incentive schedule that built on decent but industry-standard base rates.

Some who are supplementing a partner's income are able and willing to work for less in return for staying at home and not retraining. Would it be an option to choose for those?

BTW, beyond paying as well as you can, I'd add in the simple factor of trust. WMX was a stable, sensible, and reliable company, no kissy-kissy or anything like that, but the strange, counterproductive things that happen at some companies didn't happen there.

Not cents. DOLLARS. And they, or an MTSO, or anyone else has the audacity to think 14.5 cpl is too much money?

You MTSOs, and the McHospitals you have as clients, deserve to be relegated to doing OUR underpaid MT work, and living OUR cr@ppy lives for a change. I don't think you'd last 2 weeks.

I would say that one of the best things you can do is actually have some type of relationship with your employees. And that does not mean just yelling at them when they make a mistake. I never hear from anyone when I do a good job, only when I make mistakes. It can get very frustrating and disheartening to slog through day after day without a nice word (or any word at all..) from the people you work for.

Secondly, actually have work for your employees. I start at 5 am in my time zone. There is often no work for me...and I work on 3 hospital accounts across the country. Weekends and holidays are especially bad..but I know the company I work for overstaffs..by a lot..and my primary account has been taken over by India and now we only handle the reports that are stat.

Oh, and offer a bonus or lower hourly rate on accounts that are mainly ESL. There is nothing more frustrating than to have unintelligible reports and know that no matter how hard you try, you will not will not make your quotas.

In the end, I think it all comes down to respect.

I have worked at WMX for about 4 years now. I never really thought that the company could do no wrong. I really haven't been that impressed with them overall but stayed because the pay was better than what I could find elsewhere and they were more flexible than many other companies. The bonus plan was also a very good incentive and was better than any others I have seen. This was based on production and you could get up to an additional 2 cpl if your production was over (I believe) 10,000 lines per pay period. They also had good differentials for evening and weekend shifts and paid double time for holidays.

The things that I think make an MTSO excellent are a good platform that allows an MT to be productive (functional, fast, not a bunch of extra steps with demographics, etc.), good compensation, a good bonus plan (reward for quality, production, or both), steady accounts with enough work (not forcing you to jump back and forth between 6 accounts with different account specifics all day long), managers who are competent and able to provide you helpful feedback and answers to questions, a bit of flexibility (12-hour window is nice), technical support, good benefits (reasonably priced health insurance, at least 10 days PTO a year, etc.), good communication, and POSITIVE feedback (we want to know that we are valued as MTs and not just feel like a number or only hear from you when we have made a mistake). I'm sure there are probably others, but that is all I can think of right now.

The things that happened at Webmedx that upset so many were mostly related to a lack of communication and dishonesty. It's not really surprising when you think about it, they are the types of things that would make anyone think twice about working for an employer who handled things that way. For example, I went through 3 managers in about a year's time before the merger went through. It seemed like they kept making drastic changes to try to get things right but would only just mess things up worse in the end. There was a period of low work and they decided that instead of having us work on multiple accounts we would just work on accounts handled by our specific manager. I went from having 4 accounts to only having 1 account and sat for 2 weeks with almost no work at all. I asked for more accounts or why my accounts had been taken away and got no reponse. I was finally switched to a new account but had all of the accounts that I had been working on for 3 years taken away. Talk about a blow to your productivity! I also felt that I had zero job stability as many MTs were laid off during that time. We also suffered through a few pay cuts. With one pay cut we were switched to a new speech recognition editor and actually told that we would, in fact, be so much more productive on this new editor that it wasn't really a pay cut at all. We all knew it was a lie from the beginning, and it certainly didn't turn out that way. If you are going to cut pay, call a spade a spade and be done with it. Don't patronize your staff by feeding them lies about how they will be more productive and pretend like you are doing them a favor. There were also many problems with their platform and they kept blaming all of the technical problems on either your ISP or the "internet backbone." It is more than frustrating, to say the least, to hear that it is your problem when you can access all other websites and your computer is functioning normally other than being able to access their website. If you have a technical problem, just own up to it. Don't lie about it and think that your MTs are too stupid to know what is really going on.

Anyway, sorry for the rant. It really has been a pretty bad year at WMX. Hoping to get out soon. I hope this helps you somewhat. I think the biggest take-away is that honesty really is the best policy. So many things in life would be so much easier if we were just honest with each other and stopped playing games.

Firstly, I commend you for actually taking your time about this...this is refreshing :)

Yes, I was very happy with Webemedx. Reasons were, (with my experience with them that is, because we all know everyone's experiences are different), they took a chance on me coming from straight clinic work to jump right into acute care and they helped me out by just giving me H and Ps, discharges, and consultations. My first manager was wonderful!!! They also put me on the same exact accounts my husband was on, (my husband had worked at Webmedx 1 year prior to me joining), and they had him help me whenever I needed help and hold my hand basically through the whole process with no issues knowing that it was going to slow him down a little bit with his production and they were fine with that, just as long as I understood what I was doing exactly. They took a chance on me when other companies wouldn't. After all my testing they were utterly happy how well I did and emphasized to me that they only hire the best of the best and that I should feel great about my accomplishment of testing well for acute care with coming straight from clinic.

The first pay cut came with the new system. Yes, it was a pay cut per se, however, I read very, very fast and am accurate with what I read that for me it was not a paycut but a pay raise. The QA has been fantastic. I don't hear anything back and then all of a sudden I wake up and I have an email telling me I was post audited and my scores are excellent. Their QA do not breathe down our necks or nitpick.

My first manager left. My husband and I got moved to another manager and she was great too. Then months later they downsized management again and I received a new manager who is fantastic as well. 3 managers so far, all the same experience of them being fantastic, nice, and outgoing. We are not numbers to them. We are people and they appreciate us. The rule of you get respect by giving respect I live by. Whenever our AMs have ever needed help we were always there for them to help with the OT no matter what time of day. We work 10pm to 6am and our managers know that if for some unknown reason the accounts go crazy at noon they are more than welcome to wake us up and call us and we will be right on working helping out.

The Enterprise system is fantastic for me. I love the VR and the work with my accounts.

The PTO and the paid holidays are what make me happy too. The other companies I worked at I never had paid holidays off. The 401k was a big plus as well. I never had that at my other companies either. The benefits are so-so, but they are like that almost everywhere. My mother still works in clerical work for a truck driving business and her benefits there are basically the same exact as mine.

What has honestly kept me waiting this out are my accounts, the system, my AM, and I'm still making comfortable money to live on. I am looking at my other options however. Only because we know from past experience that working for Nuance is a pure nightmare. It gives me high hopes though that maybe, just maybe, Nuance is trying to change their ways and that is why they bought a company like Webmedx so maybe they can learn from a far more superior company, in my opinion, on how to treat their employees and MTs better.

It sounds like somebody who is a new to the business and happy with whatever they get, living on 2 incomes. Wake me up at noon after I have worked a graveyard to work some more for a company that just keeps cutting my pay and lying to me? I do not buy this.

Yes I am absolutely 110% for real. I am in no means "new to the business." I've been an MT for a total of 7 years and my husband for a total of 8 years. I worked for a small company for 5 years doing straight typing on clinic work. We live by our means comfortably here in the south where the cost of living is cheaper. Everyone's situation is different based on bills and etc. and everyone's situation is different based on each company we all work for. I don't mind working a graveyard and then waking up at noon for a little bit to work extra to hit OT hours if I need to because I personally like to have the extra money if it is there to be made. If that is the case for me to wake up early, like I said before my shift isn't until 10pm, I will take a nap in the interim during the evening hours if need be to make sure I can still work my scheduled shift. It's not impossible for myself or my husband to do. It's also not like my AM is calling me everyday to do that either. That has only happened about 3 times since I've been employed here. I understand you're upset about them cutting our pay and "lying" to us, but it happens just about everywhere in the industry and I've learned to accept that. My last company I was with, the small company where I was an IC previously, lost the account I was on and had to let me go...supposedly....I found out later from other MTs I was friends with that half of us got offered another account and the other half of us just got lied to and got told that there was no other work for us, meanwhile a new account had come on. At that point, I can't be upset or bitter about it. I just held my head up high and applied and tested at other places and Webmedx basically fell into my lap because my husband referred me. I apologize if my always looking at the glass half full instead of half empty bothers some people, but that's just my personality and who I am and I really shouldn't be apologizing for it.

Sorry if I do not agree with you that 7 years is not a newbie to me...sorry if I may or may not have children, I feel that is no business of yours and yes I DO have a life that I need to work around...I help take care of my brother who has schizophrenia and help support him because he can't work and take care of himself properly because of his illness. Sorry that I don't have children yet because I don't want to subject them to my brother's illness as he has to live with us and it's bad enough my husband and I have to live with his illness and his delusions and have to worry about each day if he has to go back into the hospital that day to get his medications adjusted and how long he has to be admitted for or if/when they will suggest he needs to be put in a state hospital. I also help pay for each and every prescription of his that he needs and help make sure he pays any of his bills on time. He basically is like having a child that is 35 years old so don't go and judge me AT ALL when you don't KNOW ME!

And also to add, sorry that I have to HAVE a job that requires me to be at home so I can make sure I can monitor him and also sorry that I like to make sure that I'm at least trying to keep a happy outlook on this career as I don't have many choices with the situation I am in as not many jobs offer you to stay at home to take care of your schizophrenic brother!

And "wonderful time" my ass...I'm dealing with the cards God dealt me and I'm trying to live my life the best I can with the pair of deuces he dealt me :( so...once again...I apologize for trying to make the best out of every situation I possibly can.

We have ALL had excellent QA scores, flexed our butts off for the company, helped the AM when they needed it, volunteered for OT when needed...but then we get laid off via e-mail. Is that what we deserve?

I am glad that your experiences have all been positive but believe me, stick around this industry long enough and you too will be taken advantage of, disrespected, ignored, etc. it is just a matter of time. There is NO respect in this industry anymore.

Top 10 list
1. Pay is always a priority. Anything below 0.085 cpl with spaces is a slap in the face to hardworking MTs and 0.05 for VR with high editing required.
2. Don't overwork your workforce. Constantly requiring MTs to produce more and more only makes for tired and burned out MTs who look for greener pastures.
3. Don't punish your MTs for not meeting your contracted obligations with the clients. Getting clients to sign on with great perks that your workforce cannot fulfill only makes for lost accounts and cranky management.
4. Don't expect something for nothing. MTs are not on salaries and unless they type they starve. Pay them on time for everything they do. Want them to help mentor new employees...pay them. Want them to work the help desk...pay them.
5. Keep your software platforms up to date. MTs cannot get new equipment in the marketplace to support antiquated software and/or operating systems.
6. Have a reliable, easily accessible, and fully staffed help desk for MT questions or dictation blanks 24/7/365.
7. Give your MTs morning, afternoon, and/or evening status reports. This should be all that is required to keep your MTs up to date on what needs to happen during their shifts or new info from the facilities. Nothing is worse than coming to work after a day off to have an e-mail box overflowing with bits and pieces of information that can easily be sent in a daily briefing.
8. Test new releases of software before putting it into production. This is very important with VR. Having to open two versions of playback systems because the new version doesn't play the voice file back completely takes time off the speed of turnaround the VR is supposed to gain.
9. Limit or eliminate the communication of personal issues and situations. You are running a business not a support group.
10. Remember that MTs need SLEEP!!

In addition to My2Cents and other comments here, I think another thing that needs to be mentioned is the dictators. Whatever happened to patient quality care in this? Every company states 98% plus QA. How can this be, how can a patient actually get that with all the blanks that have to be left because of a bad dictator. Why is it that the MTs are basically the ones that this falls on? The problem should be solved by going directly to the source, not telling an MT that they have to try harder. I always felt this was a double standard...tell us "verbatim" but yet we are supposed to fix the ELS dictation to "sound better" If the hospitals want to hire those doctors, then they should be made to answer for the problems. Charge them by weighted doctors, and then pay the MT that does them the same way. IMHO, if the hospitals are not going to be part of the solution, they are part of the problem, stop making it look like the MT is not good enough. The reason things are the way they are is because people let things go and then after a while they tend to become acceptable/a practice, and not long after that things become unmanageable. These hospitals bounce from company to company saying they are looking for a better price, IMHO, that is not all completely true, they are looking for someone to take on those challenging dictators that they are hiring, and the cycle continues. I know there is much more than this to what makes a good company, but a lot of that has been covered above, and I do feel this is an issue and has been for a long time. I know if it were me needing care and I had one of these bad dictators speak to me the way they do in dictation, I would fire them there on the spot, and I have done that too. If an educated person cannot talk to me any better than that, who is charging by the minute, how can any of my other health care providers have my best interest, especially when time and life are key factors in these situtations for many cases?

I agree with the previous posters plus have one other thing that all MTSOs, large and small, should think about. Consider charging clients by the dictated minute/second and paying the MTs by the minute/second instead of a line rate. We should be making this an industry standard. The industry has become backwards with more responsibility being put on the MT with less pay, when in reality the dictators need to be taking more responsibility for what/how they dictate. When clients know they are charged by the dictated minute/second, clients still feel like that is in their best interest, as they only pay for use, plus it protects the MTs/MTSOs from dictator abuse at the same time, i.e., a 25 or 30 minute report equaling only 30 transcribed lines because of an unorganized dictator or one who redundant, or decides to have a conversation with the nurse at the front desk while he has the dictation line open, or one who takes a cell phone call while the dictation line is open. These types of dictators are becoming more and more commonplace and this is the solution to that. This forces the client to address their problem dictators, but most do not and will just pay the price to avoid a hassle. It is a win/win deal for clients, MTs and MTSOs.




If they're billing the hospital by the minute, I sure don't want - to be paid by the minute

I would much rather file my nails while the MD looks for the lab work than get paid $1.50 for a 2 page report that a fast talking MD was able to compress into 90 seconds. Especially if the hospital starts encouraging their staff to make the dictations short and sweet.

If the MTSO wants to declare that their staff is required to transcribe 100 minutes a day, I'm down with that, but I would sure as heck not want to be on a regular acute care account getting paid by the minute.

A box of Cheerios costs close to $5.
A gallon of gas costs around $4.27.
A pound of hamburger is close to $5.00.
In CA, average rent on a 1-bedroom apartment is around $1200/month.
The very cheapest auto insurance is over $100/month.
A gallon of milk costs almost as much as a gallon of gasoline.
Phone/DSL service (with NO cable TV) is $100/month.
A 60-day supply of one of my medications (which insurance doesn't cover, even when generic) is $77.

My rent is $950/month (I get a discount for managing this apartment building). And my take-home pay, working 6 days a week for one of the megaMTSOs, is $1000/month.

Do you have to wonder in the slightest how that's working for me?

The "perfect" MTSO doesn't have to send cheery little emails with cartoons. They don't have to have "contests" promising gift cards to the winners, when we all know the winners are hand-picked. The "perfect" MTSO doesn't have to acknowledge MT Week (something I never even knew existed until just recently, anyway).

No, all the "perfect" MTSO has to do is pay a fair wage for the skills, mindset, experience and knowledge we have to have in order to do this work.

If you don't have the chutzpah to stand up to your chintzy clients, and tell them that medical documentation is NOT perpetually "on sale" for less and less and less money, with ever-shorter and more-ridiculous TATs, then you're in the WRONG BUSINESS. You can't go on forever cheating your employees, and thinking you'll get by on VR (hah!) and offshored transcription. Sooner or later, that bubble's gonna burst, Big Guy, and then you just might lose YOUR job. If that were to happen, you might just get a taste of the kind of lives your employees have been living for far too long.

Couple off he top of my head - sm

1) Weighted dictators with commensurate pay. If you used to be an MT, it should be easy enough to figure out who and how much of a bump certain dictators deserve.

2) If you feel the need to penalize for sending to QA or using the number of reports sent to QA as a part of performance evaluation, do not count the blanks that could not be filled in. If I couldn't fill it and your QA staff couldn't fill it, then it obviously is not my fault and I should not be punished for it.

3) Rotating weekends when coverage is necessary. I understand hospitals are 24/7/365. However, rather than making 50 people sit there picking reports of as quickly as they come in, why not actually look at past statistics, determine the actual coverage needed, and let people get a full weekend off. It's just wrong to me to be forced to give up spending time with one's family so you can continue picking reports off as quickly as they come in. Nursing staff rotate weekends, lab techs rotate weekends, even the physicians rotate weekend coverage. There is no reason the MTs should be any different.

4)Actually hire part-time people. While I wouldn't be one of them, there are some people that actually WANT to work every weekend, either to supplement a second job or because of child care considerations. Take advantage of these people. Somewhere along the way the MTSOs got hung up on requiring 5+ days of work a week (while in the rest of the world everyone was reduced to part-time so the companies wouldn't have to pay benefits- MTSOs could have told them how to get around that, huh?).

5. Return back to the days of differentials. Want 2nd shift coverage? Pay an extra 0.005 for it. Want third shift? Extra 0.01. Do it for weekends, too. Companies didn't have trouble keeping weekends and holidays and shifts covered until they took away the 'carrot'. If you're going to ask people to give up being 'normal', give them something in return.

That's a few. I'm sure I could think of more, but much of the rest has been covered by the other posters. $$$$$ especially.

Honesty and integrity - Vet MT

In a word, the perfect MTSO deals with its employees and clients with integrity and honesty. This became a rare commodity at Webmedx in the last couple years. I left because it was the identical people and experiences I had at Edix when it began its descent into Spheris, and ultimately into Medquiest. These same self-serving prevaricators transformed my home office from "it doesn't get any better than this" most days to a miserable sweatship. Don't hire the Flying Monkeys for any position in your management. Treat your help with respect and your clients and your employees with honesty. You'll be fine.

I worked for WMX for close to a year and I thought they were just awful. I hated their platform and there was never any work (and this was 3-4 years ago). All MTSO are basically the same, there is no perfect one. It all hinges on what accounts you are assigned and that is luck of the draw more than anything.

I liked Transcend quite a lot and then had a personal crisis where I ended up quitting under less than ideal circumstances. Transcend is one of the most reviled companies on this site but I would go back to them in a heartbeat based on my previous experience with them.

Point being, perfect for you and perfect for me are two very, very different things.

...in my opinion. I've been with them for years and saw a change when they hired a guy from the steel industry to try to run transcription. Since then, I've just seen one dumb decision after the next. In my opinion, the atmosphere in the company changed dramatically in summer of 2010 when Webmedx bought another company with a completely different culture and then gave them management positions. IMO, they and their unprofessionalism and lousy attitudes permeated the company, and it's hard to reverse a change like that. A year later, it doesn't surprise me that it appears to me the company has been run into the ground and the people I hear from are all miserable and looking to make a change.

So don't use Webmedx as your model of what a good company is. Webmedx has been messed up for a long, long time, imo. You'll probably have to go back 6+ years in the archives to find the days when it was hard to find negative posts about them.

You are absolutely right - that is when it started the - Steep Decline!

Ever since they bought the other transcription company and gave them free reign....it has been a hell hole. That is exactly when the attitudes changed and we were treated like dirtbags instead of valued employees. Left there last year and have never looked back!

P.S. that "steel mill" guy also ruined other companies.

I worked for LabCorp for 7 years - started at $14/hr - left 7 yrs later at $19.86/hour -- company has own credit card for employees -  called a Clear Card - available to ALL employees - Regular working hours. I have over 15 years as an MT. I think I'm pretty darn good after all these years. Perfect? No. But darn good.
