Amen to that! I remember way back when I was working in the hospital and we had to come up with a "book of style" so to speak. I think every monthly transcription meeting we came up with more changes. In other words, one rule could have 5-10 exceptions. Even the official AHDI Book of Style was questioned. I agree it is an unholy mess.
