1) is this the correct way to type the weighted score of 1+3 in this paragraph??

The weighted values for speech symptoms associated with velopharyngeal incompetence, (McWilliams and Phillips) were completed today. Overall, he received a weighted score of 1 + 3, indicative of a competent-to-borderline competent velopharyngeal mechanism.

Continued enrollment in speech/language therapy is recommended to target depression of developmental phonological processes and improved articulatory precision for "s" and "z" phoneme. He will continue to benefit from two 30-minute sessions weekly and also address speaking without a s/l "clau doral" opening.

Note, he has had cleft lip and palate repair. thanks.

I think the competence scores are derived from the sum of weighted values for speech symptoms, so you have it correct at 1+3; see changes below.

Continued enrollment in speech/language therapy is recommended to target depression of developmental phonological processes and improved articulatory precision for "s" and "z" phoneme. He will continue to benefit from two 30-minute sessions weekly and also address speaking without a glottal opening. (sometimes they say "glotteral" but glottal is correct)

I have a couple dictators who dictate impression/plan at the beginning of their reports. This comes through in ASR and so I have been leaving these at the beginning of the report even though it is not the way I was taught to do traditional H&P, consultation reports. This is a first for me. The doc is NOT ESL. But, it took me quite a while to figure out what she was saying when I heard, "The patient lickscrapitus in his knee." Later on, I realized that she was has difficulty with pronouncing some A's and E's correctly. Does anyone know of any company that does NOT use speech wreckognition. I dont care what they say about once you are used to it you can do twice as much in the same amount of time. I have NEVER found that to be true. What I do find to be true is it takes me just as long, sometimes longer to clean up the mess it makes and I do it for half the pay.
