I wouldn't use the q alone. It's one of the abbreviations that can be misinterpreted. I would type every 3-months followup. :-)

If you have a verbatim account you cannot expand unless it is one of those abbreviations that are dangerous or unacceptable. It is q.3.months. I looked up q3 months and that is an unnacceptable version as q3 can be interpreted wrong.

q.4 hours is incorrect. Just because you see it used doesn't make it correct.

Which is correct:  "The patient is here for a 3-month followup visit."  or  "The patient is here for a 3 month followup visit."   I was always told in this instance that it is without the hyphen becuase putting the hyphen in it will mean that the visit is 3 months long versus a visit at 3 months' time.   Which is the correct way to do this?  I got a correction back on a file and I was pretty sure I had it right.   ...

It's always the little things that snag me up.  I know the rules on follow up as a verb  and followup as a noun and adjective, but when a doc says, "CHIEF COMPLAINT:  Follow up acne", does he mean "recheck acne" which would make follow up a verb and thus spelled follow up, or does he mean "here for a followup of acne"? and thus the followup spelling.  What is the general rule on this "shortened" statement? I think it is a ridiculous thing to lose points over, but I am INSISTING that the way I typed this sentence is correct:   The patient returned to the cancer center for followup of her metastatic cancer.   Isn't "returned" the verb in this sentence?  I am adamant that followup should be one word here, but I am being told it should be "The patient returned to the cancer center for follow up of her metastatic cancer".  This is making me crazy.  The explanation I r ...
