Need help/input. QA problem....followup/follow up - GR

I know how to use followup and follow up in most circumstances, and I thought I knew how to use it in all circumstances.

However, what about in these instances:

CHIEF COMPLAINT: Followup/follow up.

CHIEF COMPLAINT: Followup/follow up of left knee pain.

I was 100% confident in myself, but QA has given mixed feedback...

I would say:
CHIEF COMPLAINT: Followup of left knee pain.

You could use "follow up" if dictator said something to the effect of "The patient is here to follow up on left knee pain.

Think of it this way. - sm

When you follow up on something, you are actually doing something. "The patient is here to follow up on/for..."

Followup is a thing. Followup CT scan or a followup visit.

Chief Complaint: Followup.

Chief Complaint: Follow up left knee pain, or followup for left knee pain.

The patient will follow up.

The patient has a followup.

Follow up is a verb, like "the patient is to follow up with her primary care". Followup is a noun or adjective, like "Followup of left knee pain (if you can put in another noun that is telling, like putting in Exam of left knee). "A followup chest xray", as an adjective. Anymore BOS says not to use a hypen for an adjective.

Boss and I were having a discussion on this because I had given a tip for other MTs to use...that if they could say "appointment" instead of followup/follow up, that they would use "followup" instead of follow up. And she said...

Well, not always. Sometimes the chief complaint would be Follow up of urinary retention - that is a verb so would be two words.

That makes no sense to me. Shouldn't that be "Followup of urinary retention." ?????

If you take out the word up and it still makes sense, it is 2 words. If you take out the word up and it does not make sense, it is 1 word.

Follow of urinary retention does not make sense, so it is one word.

It's always the little things that snag me up. I know the rules on follow up as a verb and followup as a noun and adjective, but when a doc says, "CHIEF COMPLAINT: Follow up acne", does he mean "recheck acne" which would make follow up a verb and thus spelled follow up, or does he mean "here for a followup of acne"? and thus the followup spelling. What is the general rule on this "shortened" statement? Thanks. ...

Well, in trying a couple of your suggestions, I find it did not make a substantial difference. I found I already try listening ahead and I had been using ctrl M to jump around the report when I need to, but never works for going faster through the actual report because I can't tell what is background gibberish while the dictator is thinking and what might be the dictator. On the 3 horrible reports that I had to submit to QA yesterday because of the 3-blank rule, thus losing 3 cpl probably, QA had no more luck filling in the blanks than I and sent them through with 5-10 blanks. That makes me so mad -- not at QA, just in general that I get penalized financially when it was the DOCTOR'S FAULT AND THE PICKY FACILITY'S "RULE." ...

I think it is a ridiculous thing to lose points over, but I am INSISTING that the way I typed this sentence is correct: The patient returned to the cancer center for followup of her metastatic cancer. I ask this because in my pre-employment test with M*Modal, I got dictations where they dictated the wrong punctuation and I edited it according to the AHDI Book of Style, which is one of my chief references when transcribing.
