If you have a doctor that dictates a procedure, very clearly, and you can actually spell it, but it comes up misspelled in spellcheck. So I go on google, look in all my books, and benchmark (can't help it I am anal) and cannot find that exact procedure. In this case it was dictated as an "otomy" but all the references had is as an "ostomy" do you leave it as "dictated and spelled," do you change to the correct form which is the "ostomy," or do you leave a blank? Thanks in advance trying to keep from bothering QA any more than I have to.

I agree. Dictator said it, spelled it. Your butt is covered. Changing something like that unless it's really obviously a gross error is more risky to you than just leaving it.

But it was dictately very precisely so it could be spelled "as dictated" so that is what threw me.

By definition they are 2 different things and not having it correct could affect the way the procedure is coded and billed. Because it was dictated precisely does not mean that the doctor has his terminology straight. Your butt might be covered but you have to live with the fact that you might have made a mistake. I don't know why people think it is okay to make errors because it was what the doctor said. Can you flag it for QA and ask them to research it or ask the doctor? My opinion is that it is better to question something than make assumptions.

Congratulations for being so careful. Anal is good. Meticulous transcriptions are the best. In my opinion, I would put ...otomy......we have one ORL doc who says tonsillotomy, no one else ever does. Yesterday a surgeon dictated that they used a certain instrument and "bullfrogged" the body part he was operating on. He knew what he wanted and how it looked and that is what he got. Using the wrong word is "incompetence," not "meticulous transcription."
You are supposed to know the difference between otomy and ostomy. Use the correct form. If you don't know what to use, refer it to someone who does and then educate yourself.
And you folks wonder why doctors and institutions no longer see any reason to use MTs instead of speech recognition? Why should they bother when the MTs' work product is no better?

For example, a tracheotomy is a surgical procedure. The tracheostomy is the resulting hole.
