I mean, I'm an IC, or whatever I'm called now since I'm doing VR.....there has been talk around the company that perhaps the providers don't even know their documents have been moved to VR.  In reality, I think that this might ring true!  Some of us have quit for ethical reasons, and I can say for certain my VR isn't getting any better with the ahhs and ands and everything else that seems to be in the dication that shouldn't be.  After all, the VR doesn't know to keep those words out.  And, I've read on this board where a lot of us are still having the same problems with making the corrections but the VR still putting words like those in there.  One would think that after nearly 5 months of this that the dictators would stop that crap....not if they don't know the system has changed.  Hmmmm?????  It would be a really easy way to get around having to change our rate of pay but not how much they are charging the client.  Hmmmmm?

They do not care. All they care is that they get their dictation finished and their charts signed off. Have you ever seen some of the electronic medical record stuff being generated by scribes, nurses, or whoever is inputting the data? It is full of spelling errors and abbreviations (which we ourselves are not allowed to use). If it does not affect their paycheck, why do they care by what method their work gets done?

why would you be something else if you are doing VR? IC = independent contractor, i.e. not an employee, who provides a service. I am confused.

The reason that most VR software does not get better (learn) is because there is no consistent correction of the text. Until MTs are doing corrections exactly the same way for a specific dictator, the software won't learn from it.

And the clients do not care one bit if the dictation is transcribed or if it is VR text corrected. They have a standard they want met (all different of course) and they don't care how it is done.

It needs to learn the physicians and a lot do not know they are - using voice recognition

The reason that most VR software does not get better (learn) is because the physicians still dictates in the same manner they always have, muffled, unclear, chewing gum, talking too fast, dictating near an air conditioning, placing their mouth on the microphone, and 100 other stupid reasons. The correction of the transcriptionts has nothing to do with it. You and I could be the only 2 people doing VR and correcting the same mistakes over and over again, and we would get the same result because the VR is not learning the physician, not because you or I are not consistent in the corrections. From what I know of speech recognition, it has a speech dictionary and words unknown to it have to be added (usually through spoken word, not from the transcriptionist). No reason to bash the transcriptionist for the VR not getting it. I've been in documents for VR and not one word was right (the physician was ESL and the computer never understood a word). Until that physicians trains the VR to his/her voice, it will never get it, even if I correct it 100 times. It has to learn the voice. VR=voice recognition.

if a doctor pronounces toilet like terlet and the MT corrects it to toilet, the system learns that as the word - for that speaker only. It is a computer system, not intelligent, and it is based on 2 types of input - what the person speaking says, and what the word is corrected to if it is not correct in the first place. You can teach a VR system to produce the word chicken when the word turkey is spoken if you correct it consistently enough times.

knows. There are various items to be agreed on and, in some places, negotiated.

on staff. These physicians could care less as long as the work is being done. Some of them are probably standing around the water cooler finding out for the first time today that their dictation has been on VR from someone who has the skinny on all the stuff they never bother to investigate because it's not as important as the job of actually taking care of the patient. That's why hospitals have administrative people to take care of this stuff, so the physicians can apply themselves to patient care!!!

Speech Wreck is ANTI patient care. Anyone who uses it or enables it to be used is compromising patient care. Don't kid yourself. Would you want the crap that Speech Wreck turns out in YOUR medical records? Your family's? Anyone's?

Our hospital went VR and there was no physician training on it, those in the pool that were felt to be better understood by the VR were thrown into the VR system. There were no scheduled days to train the physicians on VR. Some VR systems are made just for that, to be able to hook them up without the need to have the physician training the system. Yes, after time the VR is supposed to get better, but really, it is only as good as the person speaking into the VR and editing is always going to be required, unless they go to templates that you can just point and click and then the transcriptionist would not be needed and the physician can just go in and fill out the template (my doctor's office has this technology now and my nurse keys in the information right in front of me, making changes that only takes seconds for her to do).

Providers editing their own reports, yea. Go back through and look at your old reports and see if the blanks are filled in. About 85% are not filled in. Many providers are salaried employees of the institution. When they stop seeing 1-2 patients per day to edit and/or type their reports, it will take awhile 2-3 years I suspect at the least, for the bean counters to figure out why their salaried employees are seeing fewer patients, generating less income.
