Need help understanding a Fiesa grading, if possible - HangingInThere

Does anyone understand why they take off an entire point for a lab value that is 1 point off?  Such as a glucose level of 230 and is transcribed 231?  This doesn't effect patient safety or treatment.  I can understand a discrepency that is a significant difference or would change treatment.  This is a minimal error and should be graded accordingly.

Yes, that is their very reason for FIESA and the overinflated point values and classification of "critical" errors when they aren't, to STEAL money from you so they can have it.

As a QC, we are given standards for grading that are to be applied across the board. A lab value that is off by 1 point or less is scored as Downgrade Medical Value. An error of over 1 point is a critical error. We have no latitude to score otherwise. Those are the guidelines we are given.

There are strict guidelines we were to use when grading, and the example you gave is one of them. One point off is not damaging to the patient and should have been scored a minor error or even an information only note. When it was started, we were told it would be used to see where people's strengths were and put them on appropriate accounts and that doesn't seem to be the case. It depends on the QA person too. A bunch of people were put on QA who hadn't even been transcriptionists very long and they tend to grade very hard, like it's a power trip. People need to remember where they started, and that the job is to help people, not to degrade and punish.

Are there no B's given on this?  I have checked it each day since I started (which was just a few days ago).  I either have A's or C's (with a percentage of 97 or 98%).  Today I typed 27 reports and got 100% accuracy on 25 of those and 2 that had C's but my overall grade was a C!!!  What am I missing.  Since I am technically still on 90 day "probation" it worries me.  I am typing well over the line count required and have made no "major" mistakes ...

Hey guys..have a question. This quarter is over this week and with the new grading system they have implemented for post-audits my QA score is quite bad at about 96% to 97%. I have never had a low score like this ever in my career and have always been between 98.5% to 99%.  Have any of you experienced a low final score for a quarter before and if you have what happened to you? Wow!  I kinda wish I came across these posts about Nuance before I started working for them a few months ago!  I have had good luck so far with QA scores between 99-99.6%, but this week, unless I get at least 2 more 100% postaudit jobs (which I doubt), I will not be so fortunate :( But, I am still extremely lost on QC% and how that is calculated.  I cannot get a straight answer other than it is system generated weekly from my TSM.  I wish I could find out what factors are in ...

I am trying to get a grasp on understanding HIPAA.  Does anyone have any suggestions where I can find "easy-to-understand" HIPAA rules?  I have a doct that I type his documents in Word and print them to letterhead for him and then hand-deliver them myself to his office.  Is this breaking HIPAA rules?  I just need to know what I have to be doing to be HIPAA-compliant and not aware of the facts.  Thanks for any input! ...

I know we complain and complain about this on here and we fight and argue with the TLs about this...I was marked off half a point for EACH blank that I left on a dictation that the tape cut out.  The only explanation from QA was that "it was clearly understood".  I am just wondering how it was clearly understood.  I went back and listened to the tape and sure enough where I had blanks, the tape was cutting out.  So why am I being marked off for blanks.  Then on a few ot ...

Okay, first quarter FIESA reviewed about 80 reports of mine and I actually got my incentive.  Now for this quarter, I am wavering on not getting my incentive with a 98.88 and for the past few weeks they have not reviewed a single one of my reports.  I smell something fishy.  This whole FIESA thing is to a) review enough reports a quarter so you fall below 99 and then b) if you are below 99, stop reviewing reports so you are stuck there with no chance to bring it up!  How fric ...

our post-audit reports are randomly pulled.  If you score high enough to get the bonus, your reports are gone through by the hatchet queens in India until they can find a report they can "audit" to bring you below 99%.  If you are kicked back to 98.99%, they still meet their contract requirements to the client, and Nuance does not have to pay you a bonus.  ...

I almost always get my critical errors on one particular doctor who I think I understand and then it turns out I miss something.  I think there is a way to do a report on critical errors and see if I can trend the dictator but I can't figure out how.  Does anybody know?  And before you tell me to ask my TSM I did but as usual never got an answer. ...

Much as I dislike FIESA and all this QA, what I really hate is finding out I've been typing something WRONG for 30 years.  The only thing I can say for myself is some QA decades ago filled it in for me and I didn't look it up.  Been typing Campo's fascia instead of Camper's fascia. Took a 1-point post-audit hit on it, too.  Shakes me up, what else have I been doing wrong for 30 years?  And will it knock me out of bonus territory next quarter... ...

I must admit, I never looked at FIESA, not only because it was depressing and humiliating, but its not the most user friendly site. My manager was constantly badgering me for not looking at it though and actually had me try to get on the other day when she called. How do other people get away with not looking at it? Just for "fun," I counted how many times I prevented a critical patient safety error today by catching the speaker saying the wrong age, gender, right vs. left, hyper versus hypo, you get the picture.  I quit counting at 8.  Never in my many years at Nuance have I made 8 CPSEs in 1 day.  So are we getting credit for the good work we do?  Not very likely. ...
