What these companies are doing is just plain awful. From time to time, even the most experienced MTs cannot hear the simplest things. It happens to everyone! When not working alone, you simply ask for help from the MT sitting at the desk next to you, and voila she hears it instantly. No one should be punished for this. I am so grateful to MModal for laying me off. This was the best thing to have happened in a long time. It turns out things are not as bad as you think. I was able to find other MT work in the real world, and am being paid decently and I can take the time I need to do the reports correctly. Not to mention I can ask someone to take a listen if I need to, and not be penalized.

I know it is difficult to take the step to quit. Perhaps what you should do is job hunt like you would if M-Modal did let you go.

the being in school part makes it hard to leave. M2 is at least working with me on my crazy schedule. I will take day classes until finals in december. Then next semester all online except one that meets nights 2 times a week. After that in the summer I will transfer to a university where i have no idea what the schedule will be.

I live i a relatively small town where the only place that really pays is the military instillation and we all know how stable DOD jobs are right now. Plus they aren't hiring for anything I could do. I did apply for a cardiac monitor technician but i'm not sure i'm really qualified for that or even what the pay is.

I will be glad when I get done with school!

I actually just stumbled across the accounts, I wasn't really looking for MT work but I knew someone who knew a group of doctors who were looking for an MT as they did not like voice wreck and wanted to go back to dictating. I am charging a high enough line rate that I can take the time to make sure the reports are absolutely correct, and not feel cheated and angry. As for the temp work, it pays reasonably well, too. Not great ($15/hour) but okay for my area. As much as I don't approve of obamacare, at least it is generating some temp-type jobs that we, with our background, can do well. I am holding off taking anything permanent of a medical records nature though, as I want to take advantage of the MModal TAA plan next semester and get away from MT entirely. I think that is the prudent course of action; particularly since I don't want to get bought by MModal for the third time!

This ASR is awful. I wish you well on your new job. Both my internist and cardiologist use voice recognition in their offices, do long reports, and they state they never have troubles. This is just horrible here. I am at an advanced age, but looking as well. Perhaps someone will want a seasoned MT, I am hoping soon to find something. Good luck to you and to all who are looking because by the time you divide your net, your DSL bill, your electrical bill etc, you are, we are, making mere pennies on the hour. Not right.

Docking us - for assistance

I have always wondered why MM docks us for asking for assistance from QA when going above our 5% limit.

While I am not comparing our jobs as that of surgeons or police officers, when they ask for assistance (police officers will at times ask for backup and some surgeons have assistant surgeons in the operating room), they are not docked for this assistance. Asking for assistance is only to help you do your job better.

We all know how MM operates, though they will find any reason to pay us less and less for our efforts.

Lately my attitude has been if I can't understand what they are saying is "they should be speaking a bit clearer if they want me to try to transcribe/edit that". If they aren't willing to do so, why should I be willing to try to decipher what they are saying? I've been in the field for over 30 years. I know the business so to speak, I look up terms I'm not familiar with, but if they mumble, if they stumble, it they slur or speak a mile a minute, I'm just not as tolerable as I was in years gone by. And if MM wants to fire me for that, they are losing a valuable employee, even if they don't want to admit that, but I know I am good at what I do.

send anything to QA. I don't care if it has 100 blanks. If the account is not set up to automatically go to QA with so many blanks, without me sending it, then that is not my problem. If it is set up to go automatically, that does not count against me so I don't care if it goes. In my opinion, if a doctor receives a report with a lot of blanks, that is up to the facility to either get the doctor to dictate better or fix it themselves. Not my problem.

Mostly foreign, half of them don't finish a word let alone a sentence..And we are supposed to fill it in how? I can look at what he normally says (the one really really bad one) but he is not coming close to even saying a word let alone an intelligent sentence and I don't feel comfortable just throwing anything in there, of course, he is 100% QA or I would send him reports so full of holes he would think he is reading swiss cheese. This is getting to be a bit much. I need a new job, but don't have any idea what I want to do with the last few years of my working life.

You can send up to 5% of your total line count for the pay period to QA without your paycheck being affected. If you do 60 rpts a day, you can send approx. 3 rpts per day to QA without any deduction and then the deduction would only be on the lines above that 5% for the pay period, not all of your work. That's my understanding. I send rpts to QA and my pay has never been docked.

I just looked at the First Time Right policy on the Intranet (Departments, Operations, Customer Support Services, Medical Transcriptionists) and it says rpts which are systematically routed to QA are not counted against the MT incl. Client Rule, Dictator Rule, and Work Type Rule so those would not be counted against you. It also says it's calculated weekly. The best way to keep track is to look at your statistics for the week and count the lines for your rpts that have Yes next to them. If that number is less than 5% of your line count for the week you'll be fine. Hope this helps!

If this was legal and a totally acceptable way of doing business then every McDonald's employee on earth would be working in the negative for getting burgers wrong. Could you imagine telling an whole industry of fast food workers that their pay was going to be docked for anything over 5% complete accuracy? Something is wrong here.

If other companies thought for one second this was legal and OK, every company in business (in this industry and other ones) would be docking employees and taking money back from them.

My gut tells me this isn't even legal.

I recently tested for a company.  The test consisted of two typing reports, one 5+ minutes, the  other 7+ minutes. Neither dictator was difficult,  but rather sloppy.  One yawned continuously throughout the physical exam and the other mumbled in the laboratory section.  Naturally,  I placed blanks in places where I was in doubt or just flat out clueless. I agree with the poster below who says they do better without QA hanging over them, leaving the same blanks and just nit picking the rest of the report.... Very infuriating.  However, here I am, stuck on a report with a resident who is speaking so low I have the volume all the way up, then he gets loud for a second or 2, just long enough to blast out my ears, and then I can't hear him again.  I'm only 3 paragraphs into it (its 668 seconds long) and already have 3 blanks where ...

Because we are hourly employees, the can legally punish us during PM as long as they pay us minimum wage. Drop me to a penny a line, and I'm out of here no matter whether or not I have any new job prospects! Laptop With Docking Station, Can You Use Footpedal?

I am shopping for a laptop to work on.  I just found that I can buy a docking station to hook into my bigger monitor and use wireless keyboard and mouse, but has anyone used one before with a footpedal?  I'm hoping that using a docking station doesnt somehow interfer with using a footpedal.  Let me know what your experience was or if you know anything about this please. ...

the accuracy level but instead of taking responsibility for that, turns it around on the MTSO as being wrong for penalizing them for not meeting the company's accuracy expectations.  You know what they expect and you know what the penalty is if you cannot meet that expectation.  How is that wrong on the MTSO's part?  You cannot control the actions of the MTSO, you can only control your own actions and if you cannot meet accuracy, how is that a problem of the MTSO? ...

Wow!  I kinda wish I came across these posts about Nuance before I started working for them a few months ago!  I have had good luck so far with QA scores between 99-99.6%, but this week, unless I get at least 2 more 100% postaudit jobs (which I doubt), I will not be so fortunate :( But, I am still extremely lost on QC% and how that is calculated.  I cannot get a straight answer other than it is system generated weekly from my TSM.  I wish I could find out what factors are in ...

I am trying to get a grasp on understanding HIPAA.  Does anyone have any suggestions where I can find "easy-to-understand" HIPAA rules?  I have a doct that I type his documents in Word and print them to letterhead for him and then hand-deliver them myself to his office.  Is this breaking HIPAA rules?  I just need to know what I have to be doing to be HIPAA-compliant and not aware of the facts.  Thanks for any input! ...

Does anyone understand why they take off an entire point for a lab value that is 1 point off?  Such as a glucose level of 230 and is transcribed 231?  This doesn't effect patient safety or treatment.  I can understand a discrepency that is a significant difference or would change treatment.  This is a minimal error and should be graded accordingly. ...


QC Leaving?? Leaving More Work For MTs?

Sorry QC was put on the grid and suffering losses (like we have), but from my point of view the more of them leave, the more work left for us.  Isn't that horrible we are reduced to feeling that way about our co-workers??? Some QC say they are not allowed to do MT unless the account is behind, some say they are allowed when they run out of work.   From the length of time we have been suffering from OOW I have to believe they can and do jump in on our work when out of QC ...

Utterly confused. I took a big, BIG hit with the QA deduction in paycheck this week. So, I decided to investigate. Like everyone else, I have about 10 "regular" accounts. No blank rule is mentioned anywhere on any of the Account Specifics. I wrote the QA person for my CMC questioning how many blanks were acceptable without sending to QA, and got the "first time right" explanation. This is BOGUS...if I have one blank on a lengthy report and hafta send to QA and they take the total line ...

I got a complaint from corporate that my lph are not where they should be via my TSM. I have been at MModal 6 weeks and just got off training. I was hired in at tier 3. My lph are gradually going up every day but not to suit corporate. I do straight transcription, no VR, on Futurenet. A lot of ESL bad dictators. I am still getting new dictators every day I work. I'm just sayin....as a radiology MT in this company for 14 years...we have always sent radiology reports straight through to the docs to review and sign (because of turnaround time)....never knew they did QA until I got new accounts 2 years ago....and have had some pretty bad foreigners.  I believe it is docs job to go over his reports anyway...he is legally responsible.. ...

It just occurred to me that MQ could have another card up its sleeve in order to knock down our pay, yet another scourge to get us to quit.  All it takes is more than 1 report sent per day to put my percentage of reports to over 5% to QA, which would result in 50% salary decrease.  I just had a report where ALL of the blanks were from horrible sound quality.  Words were missing, the surgeon was speaking from his speaker phone which picked up shuffling papers, you know those s ...

They are more and more following M*Modal.  Now if a report is QA'd with a blank, you will be penalized in your score for that blank, and this has been verified by a QA person, who received the new directions recently.  As I understand it, you are penalized if QA was able to find the blank either in old reports or by research, or just knowing the term.   So you have a choice, spend a lot of time on research and see your LPH plummet to minimum wage, or blank it and then be pen ...

I'm on accts that do not allow any blanks at all through to the cliient.  ALL blanks must go to QA.  I haven't ever worked on an acct that would not even allow one blank to the client.  Anyhow, might not would be so bad but the dictators are TERRIBLE!  Many ESLs also.  Combine no blanks allowed with terrible dictators = sending more to QA.  That means docked pay if 5% or more.  Oh, and don't let me forget, no samples yet.  So you have to ho ...

I just looked at my review for March and I see all these 0.5 deductions for "blank filled in by reviewer."  There are also Valid Blanks that take off no points.   Sorry if this has been asked but did they say they were going to do this?  And if so, when?  Also, I listened to these blanks.  There is no way they can tell me 100% what was said in any of them, which is why I blanked them.  So they are guessing.  Two reports I have gone through so far are for real ...

I have been taught better safe than sorry and I have been a transcriptionist for 15 years.  I am docked .05 for each blank, but the standard 3.0 (CPSE) if I got it wrong it happens to be pullled for QA.  I know this is a rhetorical question as we are all professionals, but would you leave a blank and take the 0.5 right off the bat or risk the patient on a lab value, drug name, dosage or diagnosis and take the 3.0 IF that report was pulled for QA? ...

Yep, way back when they started this 99.6% to 99.7% junk that the clients would be complaining about blanks. ROLF As predicted, their little policy bit them in the butt. Since they implemented the 99.6% to 99.7% requirement, I have left a lot more blanks out of self defense that I normally would not have left, but if it is the least bit iffy that QA might not agree, there goes a blank.
