I've only recently discovered this.  If doctor dictates the age of TP and it's wrong, but you can look at the demo and fix it, don't.  Just blank it.  I just had a major error come to my E-mail about this.  I had changed the age, probably because the physician stated it wrong.  However, when you look up a report in escrption, it doesn't tell the patient's date of birth so you have no way to prove anything.  I'm blanking from now on.  I don't trust this place.

Something along the same line. It came through with a physician's name spelled wrong by 1 letter. I googled it and found the correct spelling so I corrected it and was marked off because they said that only the last name was dictated (not first) so I was assuming - now mind you I had an address, etc but no more - just leave a blank. On the one hand they tell you to research and then when you research you're handed an error and told not to do it!!

And I guarantee you that I check every age that comes out of a doctor's mouth and I remember this one, thinking in the back of my mind, "Bet I see this come back to bite me in the ***." Of course, at the time I didn't realize they don't show the birth date when you are trying to look it up after the fact. So, QA (whether they can see it or not, I don't know; I know QC can) heard the doctor say the wrong age and saw that I had changed it to the correct age and gave me a MAJOR ERROR. That's their favorite, don't you know.

Our TL had sent a memo about this. Blank and author discrepancy - note. Sorry and thanks for reminding me. nm

QC cannot see the demographics so must go on what is spoken. If it is correct and you can prove it, send it in. As far as everything being a "critical error", we go by the AAMT guidelines page, we have no choice. There are 12 different types of errors and we must mark what type it is in the appropriate column. It's not a personal thing, we do not see names of the MT, we get no benefit from it at all. We don't know if you are new or have been here 10 years. We have the same strict guidelines as everybody else.

"as best I can tell" vs. "as best as I can tell" would need to receive an Omission/Addition Minor 0.25 deduction versus simply a Comments NonError. After all, the definition of a Comments NonError is "Educational. Used to grade anything that is really inconsequential; not impacting the integrity of the document as well as grammar. Anything that reads just fine."

How, pray tell, does the loss of the second "as" in the above impact "the integrity of the document"? Not only does the first one read just fine, it's actually the preferred way to say it, with about 3 times the number of hits on Google.

This is my main beef. These kinds of little errors that have absolutely no effect on patient care are given these seemingly very minor deductions, but those add up, especially when the report is a short one. I think no short reports should be allowed for Post Audits at all anyway, and I have worked other places where they weren't since it was recognized how disproportionate the effect of errors in those would be on the final percentage. Also, everywhere else I've worked, such as the above example would definitely have been marked as a non-error/FYI. And I think still could be here by the definitions supplied, so it IS often quite subjective what QA are doing to the rest of us when they choose to mark something very minor with a point deduction that could simply have been given an "educational" NonError. I believe you all do have a lot more "choice" about the decisions you make than you seem to think you do.

But describing that one here might "out" me too much. I will say that I went out of my way to look it up in past reports, and though dictator said it dyslexically, I'd be willing to bet a whole month's salary that what I put was correct (sure could use that money). And I would have been *expected* to do exactly what I did with it at other MTSOs in the past, would have been considered derelict in my duty if I hadn't used my own experienced judgment to make the decision I did. Now we're just extensions of the VR and expected to have no more actual "sense" than it does, binary on and off, no room for the human factor and common sense.

report? I always have dictators dictating wrong age and I use my brain to correct it based on DOB. What I am hearing is that we should just blank it and cause more work for someone(not sure if inhouse or QA people). This is so ridiculous. BTW, does this kind of blank go against the MT?

explain that the demographics showed DOB xxxxxx and this is why you blanked it? What DN would you use? I would want to use discrepancy. Waste of time, I know, but MT should not be dinged for leaving a blank in this situation.

Leaving a blank for age being dictated wrong would end up being half of my reports.

Obviously they have no faith in the MT having a brain.

So sick of QA. I have just started leaving blanks for almost everything. If I don't leave a blank, I will most certainly get a CRITICAL ERROR. When did small words become a critical error?

Just know that Nuance is making sure you and I do not get a bonus because our managers have to have their ridiculous amount of time off, CEO's have to have their million dollar bonuses, HR has to have their paid days off, and don't forget about India, and the list goes on.

There are no other scores anymore, but critical and major errors. I am always getting an erroneous error for something. The word "the" seems to be a favorite lately. I have not checked FIESA in a long time and I do not plan to. I am sick of their little games of trying to make me feel like I can't do my job.

It's fine to not look at FIESA but now they email it to us to make sure we get the beat down.

And "a" and "an" as well, but especially "the," since there's a very clear example of that in the Account Wise Error Categories list, unless they've changed it.  (I can't find the list on the new Fiesa, just the old one, but if you don't have it or can't find it, I would request it from your TSM.)


Definition of a Comment NonError 0 is:  Educational.  Used to grade anything that is reallly [sic] inconsequential; not impacting the integrity of the document as well as grammar.  Anything that reads just fine.

I'm not sure if they think we have no brains or if in reality they have no brains. There is SO much that is wrong with this whole QA process, it's tiring to even speculate.

If it was me, I would fight it, especially if this was on a post audit, as it can be checked. If this was QC, I would ignore it and keep going by the demographics.

the whole huge 2-year difference, but if I did notice a discrepancy now, I would just blank it. As I said in my Fiesa Feedback/Action box: "There is no way for me to confirm the accuracy of this, but reviewed./xx"

When you have a new report come in, just type in the name of the one you got an error on and check the DOB? You can always go back to the original patient. Well, I think this works anyway.

Slightly off topic but related: I used this website frequently to verify ages to make sure I didn't get my math wrong, which I sometimes did, especially when I was tired. (I have worked on MT platforms before that automatically calculated the age of the patient for us, but that seems to be too much to ask from EditScript and Nuance. Wouldn't want to help the MTs increase production or anything.) The nice thing about this website is that you can put in dates other than today and calculate an age based on that date, which is helpful when the docs are catching up on backlogs and dictating reports from several months ago.

If you have one of those accounts where they want all medication allergies capitalized, note that DQS 7.1 DOES NOT pick up all-cap typos/misspells.  This would include typos in the physical exam as well if the subheadings are all caps, but the medication one is more critical.
