Yale-NH MTs HEADS UP - MT is dead

Epic is coming to Yale-NH very soon.  They've posted lots of Epic jobs on their website. MTs working this account prepare yourselves to be slaughtered. Epic came to our account and we are now producing less than 100 lines per day.

EMR is mandated to be in place in ALL HOSPITALS, DOCTOR'S OFFICES AND CLINICS by 2015. MTs have rounded to corner to the end.

EMR has been used for years, especially in Veteran hospitals who have - lots of MTs

The easier, more casual jobs are gone. That's for sure. It's also true that things are changing, but there's nothing new about that either. I was getting discouraged a couple of years ago, but things seem to be changing for the better now. The skills MTs have are still going to be needed. There are even new jobs opening up that use the same skills. You have to go on-site for some of them and they don't always call it medical transcription, but the same skills are used.

Scribe jobs are an attempt to do something that doesn't need to be done. Starting from the fact that most people don't want strangers in the exam room with them and going to the fact that the scribe is being forced to almost think for the doctor, which is just scary.

Finally, most MT jobs aren't transcribing work in doctors' offices. Most of our work comes from hospital dictation.

There is nothing positive about scribes.

My MTSO has accounts with hospitals that use EMR. But the doctors still dictate. Yes, the intent was for the doctors to do their own notes, not by point and click, but to actually type their own. Their system is sort of a mix between the two. Now some do do their own notes, some do half/half, some dictate everything. We type up the notes directly in the patient's chart from the doctor's dication. So just because a hospital has EMR does not mean the doctor won't still dictate. Obviously this depends on the hospital and the platform they use for EMR too. But I think most doctor's would prefer to dictate than do their own work as it is just too timeconsuming and my docs keep themselves pretty busy.

Yes, they are still dictating.

In the typical EHR, the doctors dictate a narrative that is 1 or 2 brief paragraphs. The rest is templated or point and click.

You are comparing apples and oranges.

There is no one-size-fits-all system and no consistency either - Variations

There are some specialities that still dictate pages per report. Also, there are hundreds of different EHR systems. Each one is different. Some use the same dictation as they have been doing in addition to the pull-down, point-and-click, etc.

HEADS UP REGARDING ASR

We all know this, but it seems things are getting worse with regard to ASR. I've been noticing it is substituting right for left and vice versa even though the dictation is crystal clear. Also, it is substiting Mr. for Mrs. or Ms. and vice versa even though the dictation is crystal clear. As someone noted previously, it is still making blatant errors in numbers (ages, lab values, vital signs, etc.). These are only a few out of a zillion examples lately. Just wanted to give a heads up to everyone out there.  I have been working for ANP Transcription out of New York for three months now and will NEVER work for them again.  They push, push, push for work to get done but can't seem to send out paychecks on time.  I have had to call and email each month.  Mysteriously, my very first paycheck didn't make the check run and my second check I was told was mailed out on July 9, but I have yet to receive that one, too. ...

If you have one of those accounts where they want all medication allergies capitalized, note that DQS 7.1 DOES NOT pick up all-cap typos/misspells.  This would include typos in the physical exam as well if the subheadings are all caps, but the medication one is more critical.
