I was looking over already transcribed reports by others to check on a medication and came across a mistake that really makes us look foolish and is something, IMHO, that any transcriptionist would know and this doctor speaks good English. Would you notify QA so they can correct the report or notify the hospital to correct the report? Thanks for your opinion.

your last time coming across a mistake in reports that you look up. I have seen a great many in the repository that had mistakes. Trust me, thats not the only report out there with a mistake in it.

I seriously doubt Mmodal would be pleased with you if you took it upon yourself to personally call one of their clients. I would think they rather you report it to them and thats with any company, kind of like an ethics thing.

I notified my supervisor about something similar and she was actually offended.

It happens. I worked for a hospital for 10 years before being outsourced to mmodal. Even then when we viewed previous reports, every so often you would come across a mistake or something you knew was wrong.

I remember one time at a dept mtg our transcription supervisor showed us a report that had been typed a year prior, it was an OP note and it had the incorrect terminology for a body part or something. The mistake was in the patient's chart and never got caught until the insurance company decided to question it.

And where I worked, we were all good transcriptionist and many had been there since the place was founded, but hey the point is it happens. People make mistakes and thats anywhere you go. We just do the best we can to produce the best quality work possible and move on.

I agree. No one is going to be able to sit and edit/type all day (not much time for proofing) and never make a mistake. The level of accuracy we need is 99%, not 100%.

I reported a couple errors I found in a report, and I started getting audited like crazy!

I just saw one in the repository that said:

INDICATION: Thank you.

Now, we all know that means "end dictation, thank you" and should have been edited out!

Not taking out that kind of stuff is really crazy. I saw an old report where they left the CCs in at the end of the report. I guess that's one way to get paid for them. LOL.

The only way I say something is when I find continuous - wrong use of a term

The more you go into back reports the more your going to get disgusted..They are generally ok but there are some real winners there...Besides the report has gone through and the doctor SHOULD check it for accurary, it may have been fixed on their end already..who knows, who cares, they dont pay enough to...

I am a former Spheris MT, now begrudgingly a MQ MT (not that Spheris was all that great either). I have posted on here many times about putting in requests to have doctors removed from ASR and have been told it doesn't do any good. Apparently some people are listening. I was told that they are flooded with requests now and it is taking a little longer to get to my requests reviewed.
