Where is MM ASR getting this stuff??! - dnr

I just got a report where words were added that were essentially a diagnosis that could match the symptoms and treatment the patient had, but THE DOCTOR NEVER SAID THAT! Talk about adding words! Now MM ASR is diagnosing patients along with all the other extra words it adds. At least this string of words was obvious, but there are a lot of little care-changing words added that could be missed. It's amazing what it makes out of thin air when the doctor isn't dictating as it's getting to be output that could possibly be seen in a patient with those signs and symptoms or what might be said in an operative report with a particular diagnosis. So scary.

I've had reports where the doctor isn't saying a word and SR is just typing away, adding words during the silent time. Very strange.

I saw a demonstration of this a few years ago, you can Google it and read up on it, but basically they are using this with the speech engine and it recognizes certain words and links them, just like our brains, and they are hoping to use this with helping automate coding as well. I could be wrong, but I don't know why else it would pull something in like this that DOES go with what was said, unless it was programmed like this. Pretty scary to me! I know of several hospitals that have gone ENTIRELY to this type of speech wreck and the physicians have to correct it themselves. I know someone in HIM at one of these hospitals, and of course the physicians aren't doing much editing, but no one seems to care. Personally I am asking for copies of all of my medical records and I am going to check them carefully for errors!

is a giant scam that these companies came up with to make money off our backs. I do know you are right about why it is putting stuff in that it shouldn't, but it is also supposed to learn and it doesn't. I warn everybody I know to read their medical records. The accounts I am on, it can't even get the name of the facility of the accounts right. That's pretty bad. It always adds psychiatric to the physical and the doctors never dictate psychiatric. So much wrong with using this stuff, yet the facilities keeping getting suckered in. I guess I would like to have the company's salespeople to sell that ocean front property in Arizona.

It is comparing a word or words to repetitive patterns and then matching and adding to the output. E.g., "fevers and chills" is dictated a lot so if the ASR "hears" the word "fevers" it matches it to "fevers and chills" and then automatically adds "fevers and chills" rather than just "fevers." It also does the same thing with medications where oftentimes several medications repetitively follow each other so it will add medications. This started with the 7.1 version. Unfortunately, apparently, the software department at MModal could care less about fixing it.

a full report full of jibberish and nothing was even dictated. I should have left it there and collected the lines for it, but I deleted it.
