Does anybody out there work at home for a hospital, not a transcription company.  How do they handle the running out of work situation - do they expect you to use your PTO or take it without pay.  Are they more flexible with scheduling - I was hoping for some place that might say you have to work 8 hours a day anytime between say 8 a.m. and 8 p.m.  Do they still pay by the line, and less for speech recognition?

I'm locked into a certain schedule which is fine by me. We NEVER run out of work. Overtime is pretty much always on. It's about 90% VR editing and 10% straight typing. We are paid straight hourly with a generous incentive program on top of that. It's an incredible job. After only 9 months with the hospital, I already have 100 hours of paid leave accrued. I will never work for an MTSO again.

WOW, how lucky are you. I would love to have that type of set up. There is only one hospital in my area that does that and those MTs will be there until the end of time. No chance of ever getting on with them. I hate all of the companies. Not one good one in the bunch, I don't care what you read about on this board. Again, WTG, I bet you really appreciate your job and what a nice feeling that must be. Where are u? I may move!

There isn't a single day that goes by that I don't sit down at my computer and thank my lucky stars for getting this job. Like the hospital you mentioned, the turnover at this place is practically nil. I just happened to look on their jobs board at the right time. They were expanding to other hospitals and needed more staff in transcription. It has completely altered my outlook on transcription and how long I intend to continue doing it. I'm a content MT in Texas.
I sure wish I could find an inhouse job like yours - in Northern California! Still looking!

We have a minimum QA score that we have to meet each pay period (98%) and minimum line count and then there is another line count level to reach if you want to get incentive pay that is pretty easy to reach.

Yes, we are expected to maintain 98% or higher on QA scores, we have TAT, we have a minimum LPH to maintain, etc.

As poster noted below -- yes, we are always in danger of losing our jobs to a service -- just like a service is always in danger of losing an account. We maintain super TAT and have an average department QA score above 99% -- if we continue to provide great, quality service we can compete with the services and will keep our jobs.

After working for MTSO companies only (2 at the present) I applied for a hospital job. It seemed to go okay and they have a few more to interview. The benefits are way better and it seems great EXCEPT the hours. It is evenings and both weekend days every weekend. Not sure I should take it if offered. Even though I work pretty much 24/7 from home now, I do not know if I could get used to this, plus there is a bit of a commute. Has anyone had experience with this?

I work from home for a hospital. When we (infrequently) run out of work, we can use PTO if we have it or time off without pay. If things pick up later in the week, we can make up the missed time. Running out of work does not happen nearly as often as I experienced at The Q! Choosing my schedule was somewhat flexibile when I was hired, but once chosen then it is basically a set schedule. That is not to say they are not flexible, because they very much are -- if I oversleep - no big deal, work a little longer at end of shift. If I have a school function/meeting/dr appmt - no big deal to get PTO or make up later. Hours are not as open-ended as you suggest, however. That would be sweet, but they do have TAT to consider, so I understand their position. We are paid by the line. The down side - at least IMO and at this particular hospital - no raise since I've been there (coming up on 4 years now) and no incentive/bonus plans, no shift differentials. We all rotate weekends, so work 1 weekend day every 5 weeks or so (with a day off during the week in trade). We do get paid OT and the bennies are terrific as far as health/dental, retirement plan, accrual of PTO/sick time, etc. Computer/equipment is provided and maintained by hospital.

We have not gotten on board with speech recognition yet, but that is the next step. I do not know what the pay rate will be for that, but hear through the grapevine that it will be lower than straight typing, of course. How much lower remains to be see.

All in all -- Keep your eyes open, and if you see a hospital job, snatch it up and get away from the services! I'm sure there are stinkers for hospitals out there too, but overall my experience has been that the difference between working for a service vs. a hospital directly is the difference between night and day! I also will NEVER work for a service again. I'll find another job outside transcription before I'll go back to a service, especially a national!!!

I also work from home for a hospital and would never, ever work for a MTSO again. I have awesome health/dental, I get CTO/PTO plus sick time. We have never run out of work in the 4 years that I have been here. We do have set schedules that we have to work, but I have a great boss who is very flexible and if I need time off to take one of the kiddos for a doctor's appointment or go to a school function or whatever, I can either use CTO or make up the time sometime the same week. Like another poster said, our turnover is nil.

I worked for a hospital for 32 years, the last 15 at home. I had a set schedule and loved it and thought I would still be there. We had very little turnover but it did happen occasionally. The hospital decided to outsource last April. One day I had a job for as long as I wanted to work(I was transcription supervisor) and the next day I was told I would have to take a major cut in pay and take a clericial position or retire. I chose retirement. I also had great benefits and fortunately had a state retirement and was able to take full retirement, but alas no insurance so I am back working part-time for way less than I made before but since it is a secondary income and to pay for my insurance I am living with it. Glad all of you still have hospital jobs but unfortunately that can change in the blink of an eye. The basically just got rid of 7 positions in order to save money.

That is too bad. :-( Our hospital used to outsource part of their work but then a few years ago, the management realizd that they would save a LOT of money by hiring in a few more transcriptionists and ditching the service.

Thanks. I think they are realizing that they made a mistake but now they don't have anybody to fall back on so they are stuck. I suppose they could rehire and/or hire new people. The funny thing is that I felt like I had job burnout and while I was extremely upset, it was almost a relief too. After I had not been working at all for about 3 months, it hit me that I still loved MT and the burnout was with the place I worked and not my job. I am doing clinic notes now instead of acute care and I love it. The only pressure I have is keeping my accounts within 24 hour TAT. Some days I work 3 hours, some days I put in a full 8 but it works for me. I now think it was a hidden blessing but I am still pretty ticked off about it too.

I'm at a point in my life where my family NEEDS my benefits. Husband is self-employed and we still have kids in school, so for us to go the self-insured route again is somewhere I don't want to go right now. It sounds like the job you have now works perfectly for you. :-)

Doesn't matter what sector of the economy you work in, nowadays there's no such thing as a guaranteed job no matter how stellar an employee you are. Having said that, the hospitals I work for outsourced all the transcription a couple of years ago. They were so unhappy with the TAT, the quality and the cost that they brought it all back in house. Hopefully, they have a long memory regarding how unhappy they were with the services.

I used to work at home for a hospital/clinic, now I work on site for them (my choice). First of all, we haven't run out of work in years, if we did you would be expected to take PTO or make up the time later. Down time for technical issues is paid at your regular wage. The scheduling is flexible, but you are expected to get your hours in and submit a schedule every week. We have no VR, it is all straight transcription, production based pay.
