Wondering of anyone has thought of/heard of this position? TIA! - SmileyChica

Posted: Feb 12, 2014

Im wondering if an MT with 23 yrs exp would ever be hired to dictate in-house FOR the physicians or input on an EMR FOR physicians?  Im not even sure that is a position or if it would be legal?  Anyone do this?  If so, what would you charge?

I seriously doubt it. Unless you're working closely with - the doctor in office, Sm

[ In Reply To ..]
you wouldn't know what they want to say for history and treatment plan, etc. That's what the PAs and NPs do.

Sorry, I doubt your 23 years of experience qualifies you to make any medical decisions for dictation or to even dictate anything appropriate for the doc to read and sign off on.

Not even EMR, because you didn't see the patient, weren't with the doc when they saw patient, so you dont know what to input.

I agree with all of that. I guess I was thinking more....Entering chart info from handwritten charting or office typical normals info via dictation. The VR could recognize a person as much as they could recognize a doctor. Busy MD who does not want to input, but will review and sign off? Just a thought.

Have thought about it - see msg

I have thought about it in terms of an experienced MT would know what to do and what not to do to make it easier on the MT, but even LPNs don't dictate, as far as I know. That's saved for ARNPs, PAs, DOs, and MDs.

Legality - Just me

I don't think a facility would ever hire an MT with experience to dictate. For one thing, it is not anyone's job to "make things easier on the MT." As mentioned, only licensed practitioners can dictate. When we've had residents or medical students, the attending physician has been required to co-sign their notes. I can't see that working, mainly because if those records ever appear in court, you are on the line as the responsible party. In other cases, you or the facility you work for could be sued if there are critical errors that affect patient care. You would have to be present in the room during the exam, and unless you were the one doing hands-on with the patient, your dictation would be purely subjective. For legal reasons, this is just a bad idea. That being said, why would they need an MT to dictate when they can use VR, dictate it themselves or use point and click? Would you then transcribe your own dictation?

Guessing easy out for insturance to not pay and what would the HIPAA Gods have - to say? Quick, new rule. haha NM

The law office where I work would be afraid of this because they need the signature of a licensed medical professional. (Our firm does a lot of med/mal and I do a lot of records organization, etc. But no, I do not sign off on any records.)

nothing new and makes no money

I have a telephone interview for a scribe - position tomorrow

[ In Reply To ..]
afternoon. I told them in my cover letter I wanted $15-$16/hour. That is the going rate for scribes in my area. The position sounds like more than just transcription -- lots of following up on patient issues, paperwork, making sure the EMR is complete. I will post more when I know more. It will probably be a lot more responsibility than it's worth, but I desperately need the money.

Just had my interview (cancelled yesterday - because of the snow)

[ In Reply To ..]
Basically, the founder of the practice is a little older and can't get the EMR filled out for the 35 or so patients he sees each day. My role would be to complete the medical record as he dictates it so that the record is complete before the patient walks out the door. Sounds challenging but also lots of fun. They will hire a FT and a PT so the scribes aren't typing their fingers off all day. He is interviewing MAs and LPNs, who know the medical side, but is also interviewing transcriptionists because they can keyboard more quickly and also are familiar with what comes next in a SOAP note. I think he is leaning toward a transcriptionist. The program they use is eClinical Works. I've viewed some videos of it and it looks fun to use! Just thought I'd pass that on.

Not talking about a scribe - Not the OP, but...

[ In Reply To ..]
A scribe follows the doctor around into the patient, and just inputs directly upon the doctor's instructions, live. The scribe does not go out on their own and input, and definitely does not sit down and dictate.

some scribes do - gollymolly

[ In Reply To ..]
I have a few friends who are working in what is termed a scribe position. The doctor gives them a form after he sees the patient and they dictate it at one hospital, in the other they simply do the inputting.

Thx for the replies... - SmileyChica

Im not considering it. When my MT job is done, so am I. I do it b/c I like it. I just read this and see a lot of MTs with a lot of medical experience and thought this might be a new avenue someone could look into. Great answers. Lots of intelligent thoughts.

Do you have a professional license as an M.D., nurse, or PA-C?

No....sm - SmileyChica

And, like I said, Im not interested in doing this. It was just an idea that I thought might be an option for others with experience who were having trouble finding employment in the current changing MT field. When my current MT job ends, Ill be done. But, I do still love it!

Something I Just Thought About And Am Wondering
Oct 05, 2013

I was just on my FB page and get a lot of "political type" posts.  I saw one regarding Catholic priests supposedly not being able to say Mass tomorrow at military installations because they are considered "government subcontractors" and could either be fined or jailed if they violate this order.  I am wondering about the MTs who are contracted for VA accounts, because to my way of thinking, the MTSO and possibly the MT, could face the same consequences. As an aside, I also saw a post ...

if our supervisors have our best interest in mind.   I know they are all scared for their jobs too, but couldn't they go to bat for us and let the "higher ups" know that having us in one giant pool typing so many accounts is not going to help in keeping jobs in TAT.   Yesterday, I got a different hospital for the first 2 hours of my shift - no back-to-back jobs from the same facility until well into my shift.   I was playing catch up all day and all night to ke ...

I work in-house, got the news a week ago that the hospital has hired an outside company to take over the transcription, which will save the hospital millions of dollars.  We have the option of going with the other company or simply going home; in other words, take it or leave it.  Needless to say, the outsourced company's benefits are not as good in comparison to the hospital's, much more expensive and also this will be strictly voice recognition.&nbs ...

Wmx - Another Thought On This
Sep 02, 2010

I think this was a very large account and they could have divided the MTs who had this as their primary and put them on various other accounts.  Yes, this would have overstaffed those accounts and more MTs would be getting NJA as we all scrambled for what work there was.  Also, this way WMX would not be putting out unemployement for anyone and soon most of us would not have benefits because we couldn't keep our line counts up with NJA all the time.  All in all, looks like the ...

I'm just wondering what is with companies not calling you after you have passed a test and said they will? I don't understand it. Yes,  I realize that they may have several people, but is it too much to ask for a phone call to do an interview or something? I Thought I Was The Only One
May 13, 2011

I just realized that no one cares about the quality of medical records anymore.  I have had to go to VR and I am now making anywhere between 4 and 12 dollars/hour.  I guess people care more about the quality of their hamburgers than they do peoples' healthcare. I have started to just send them through.  I don't even really listen to the work.  I look at it and change any major spelling errors, but other than that, what the VR says is what they get.  The fu ...

Could it be that the MModal and Nuance are buying all of these smaller, better MTSOs because they want to limit our employment options?  If MModal (the Q) and Nuance are the monopolizing companies, and an MT who works for them just had all she/he could take, where could she/he go?   Just a thought....   ...

Wondering About Pay
Dec 14, 2009

I work as an MT in a very rural area, so the pay is VERY good for me.  We are a one income household since my husband got laid off and I easily pay the mortgage, car payments, all of the bills, etc.  I was wondering, if a person lives in a much more expensive area, like California where a person's mortgage would easily be four times mine, do they get more money per line to compensate for that, or is it the same? ...

As an in-house MT we were required to wear business casual clothing.  Another job required scrubs.  Now that I work from home, I wear pJs, sweats, jeans, shorts, whatever.   I noticed that when I am "dressed" (not in pj's or sweats) that I seem to be more productive and I was wondering if others have noticed the same thing.  Sometimes I wonder if the way we dress to transcribe affects our productivity as a professional employee.  You know when you dress up to go ou ...
