What happened to standard forms? - Dallas

Suddenly we have new QA and it's gotten crazy! We have some new PA's that dictate things so out of order as compared to the "norm" and QA wants us to give it to them instead of making reports alike as we were trained to do.  I've had some dictate drug allergies in review of systems and medications in the plan and QA wants this. I don't get it. I just don't understand how a doctor can know to check the plan for what meds a patient is on. 

I also have a PA that will say "like" every other word such as "the patient like came in and like he was like not feeling well and like I asked like what he thought had made him like feel bad."  Where do they find these people?

I just cringed when I read that - Like, groovy man.

The use of the word "like" after every third word out of someone's mouth is one of the biggest things that will make the hairs on the back of my neck stand up - like fingernails on a chalk board for me.

I think - and this is only my assumption - that QA might be thinking that it's the facility's issue to deal with the idiot PAs and their training or lack thereof. It certainly will be easier for you to just do it as they say it rather than try to figure out where it should go.

Is the document going to be a mess? Yes. Is critical information in the document? Yes. Even if it's in the wrong place, it's still there.

Especially if this is a verbatim account, I'd do it verbatim. Someone else somewhere will read this and realize the PAs are idiots and hopefully do something about it.

Tell me that I've made mistakes. Nine years ago it was beat into my brain by the former owner, who has since passed away, that all reports need to be the same so that it's easy to locate what you're looking for, especially allergy location and medications. These new QA folks obviously have never had to look at a report quickly in an emergency situation.

They have even started changing how vital signs are typed. You'd think BP would be important along with temp but we have some who want height first, weight is somewhere in the middle, half the time there's no BP or temp, you might get a pulse every now and then. It's UNREAL and making me HATE this job.

If You Work As An IC, Do You Still Get W2 Forms

taken out of your paycheck each pay period, or do you have to figure all of that out yourself?  If so, do you think it's worth it to be an IC, or not worth it? I have some lists I printed out back when I was in school with unacceptable and acceptable brief forms and confusing abbreviations. There are things on the unacceptable list such as cath and a fib. Wondering if anyone out there has ever thought of filling the gap by moving into other forms of transcription... As EMR increases, pay dwindles and opportunities for traditional transcription diminish, I'm thinking that it might be wise to broaden my scope a bit. I've thought about court reporting (as a broad spectrum...not just actually in the court room) and other forms of transcription other than within the medical community. A doctor wants me to do PFTs from a premade template he created.  It is about a page long.  It would involve typing the clinical history (2 lines or so) and filling in a lot of blanks.  He does not want to pay my line rate that I get for typing his office notes, but wants me to give him a flat charge per report that is much less than what I would get for typing dictation.  Any advice on what I should price this at? ...

Time and time again it is reiterated the importance for us to remain professional, as we are a reflection of MQ.  For example, never say "poor dictator" because this may be insulting to the holy than thou doctor, PA or RN.  What about the annoying PA that constantly insults me by saying in middle of her dictation, "note to my typist."  And this is not insulting?  All the training, years of experience and BS I have to put up with to be called a "typist" repeatedly.  ...

I have been here a little while, about 3 months.  I have been switched around between CCMs already to go where my primary account goes.  I needed a few hours to go to a school function and asked for that leave.  I have never been responded to by my CCM.  Is this usual?  I put up my shift on the swapboard, said I'd make the time up.  Etc. i am on 100% QA and am constantly confused.  I am corrected for things, ask my coach.  She tells me QA is correct.  Then when I change it how she says to I get it changed back to the way I was doing it before~!  I love Fluency, i am amazingly fast at it but i think QA will ultimately do me in...for doing what i'm told to do...GRRRRR ...
