Unacceptable brief forms - jd

I have some lists I printed out back when I was in school with unacceptable and acceptable brief forms and confusing abbreviations. There are things on the unacceptable list such as cath and a fib. I feel certain that I printed the lists from either JCAHO or ISMP, but I have been unable to locate them. Does anyone know where I can find these? Thanks!

JCAHO is one list, but not the be-all, end-all. I would check with the company you work for, if you are working for an MTSO. They often have JCAHO-type information for abbreviations.

From there, do keep in mind that there are client preferences as well, so that needs to be checked against whatever list you have.

Both of those are acceptable short forms. But as the poster above mentioned, the Client Profile is always the first authority on acceptable abbreviations/short forms.

might try the AHDI site. ps - I believe that's afib. - also, did you look your BOS? nm

JC has a short list of do-not-use abbreviations - on their website

It is very short and does not include things like cath and afib.

Other than that, if the abbrev or short form is in a recognized reference like Davis, Stedman's, or Dorland's, it is fine. The facility has to make it clear what that reference is.

One thing you need to be careful about is not to make your school list "the law" because your school said so. Those kinds of things have been passed around for decades, causing aggravation and trouble every time they rear their heads.





OP-So, do you work for an MTSO - Or on your own?

If you are working for an MTSO, they should have a list for you. From there, you check the client preferences (client profile, account specifics, whatever the MTSO you work for calls them).

If you are on your own with your own accounts, you may have to go to your client and ask their preference.

Suddenly we have new QA and it's gotten crazy! We have some new PA's that dictate things so out of order as compared to the "norm" and QA wants us to give it to them instead of making reports alike as we were trained to do.  I've had some dictate drug allergies in review of systems and medications in the plan and QA wants this. Wondering if anyone out there has ever thought of filling the gap by moving into other forms of transcription... As EMR increases, pay dwindles and opportunities for traditional transcription diminish, I'm thinking that it might be wise to broaden my scope a bit. A doctor wants me to do PFTs from a premade template he created.  It is about a page long.  It would involve typing the clinical history (2 lines or so) and filling in a lot of blanks.  He does not want to pay my line rate that I get for typing his office notes, but wants me to give him a flat charge per report that is much less than what I would get for typing dictation.  Any advice on what I should price this at? ...
