Which generally pays more?
Is there any speciality that generally has a higher cpl rate?

It could depend if you are working for a clinic or hospital versus a national or smaller owned company. Do they pay by the line, by the hour? They would probably not pay based on the work type you are doing.
I transcribed Radiology for many years, and they are more likely going to be shorter reports than Acute Care or clinic notes and more consistent wording so you could make more shortcuts but I did not make anymore money doing rad versus anything else.
Radiology became very boring and repetitious after awhile and my boss figured I would be better suited to do more extensive and more detailed work, so went to op reports. The OPs are my favorite as I can save most of them as shortcuts as they are pretty much the same wording most of the time. I did not get more cpl for those either, but as I could do more reports and more lines with OPs than with acute care/clinic notes I increased my wages.
So there are a lot of variables to go by, and only from experience will you be able to figure that out in some cases. I did not care for clinic notes, so would rather do Acute care with OPs and radiology was too boring, but that was just me.
And get hired? Opportunites are limited with clinic work, which is all I have experience with, but nobody will give me a chance. Should I apply for acute care positions anyway? Anyone MTs who only did clinic work have any stories about breaking into it? ...

I lost my primary accounts recently and was switched to an account where most of the jobs are really short clinic-type notes (less than a minute). Has anyone started to get more work in the last year (in a clinic setting)? It was steady for me a few years ago, and now I am overwhelmed with the amount of dictation I have!! Each of the doctors I work for are seeing wayyy too many patients in one day, thus more dictating, etc. Im putting in like 10-12 hour days now, whereas I used to only work 5 or 6.

I have always hated it when the doctor crams in every disease the patient has in the first line whether or not the list is relevant to the chief complaint.
I can't think of anything more pointless than having a PA or resident dictate discharges when all he or she does is lift the consultants' impressions and plans and then plug in his or her standard "vital signs remained stable and the patient was discharged in stable condition."
