I have a theory regarding doctors - Enlightened

Is it possible these doctors had mothers who were transcriptionists and they now have unresolved issues with their mothers, so they take it out on us, as transcriptionists? Sometimes I feel as if I am being punished by them.

Doubt it. I just think that they really believe they are - higher than God

are just arrogant slobs who managed to make it through medical school. I think if their moms were MTs they probably would be more respectful.

After explaining to him that cell phones do not record, he stated he had spoken with her to use a landline and is certainly going to insist it. He did. She emailed me apologizing, stating she will now use a landline.

There is hope. You just have to point out that bad dictation is UNACCEPTABLE.

I am still having issues with one more doctor, but the other 95 dictate clearly on landlines for me. :)

allow us to attach notes to our jobs when a dictation is really bad. How will they know if we cannot tell them?

It is called risk management, and they need to address it immediately. Nobody wants to suffer through horrible dictation, and it severely affects TAT and patient care. My contracts specify a landline for sure, and when my clients put in a VOIP, I had them reinstall a landline for dictation.

My theory is they're busy and aren't thinking about anything but getting it done so maybe they can go home at a reasonable hour. Why is that such a hard concept to understand?

The purpose of language is to communicate. If no one can understand what you're saying, then communication is lost. Any grade schooler can easily grasp this concept. I don't care how fast they talk, as long as they make sure it's understandable. What a novel idea, huh?

Busy is part of the job, and they have a responsibility to accurately document the healthcare that was rendered to the patient on their watch so that the healthcare rendered to the patient in the future can be done safely and accurately. They have no excuse for the irresponsible manner in which many of them approach their dictation. It's inconsiderate, it's selfish, and most of all it's dangerous.

My elderly parents have a bunch of doctors, and they say with some of them they can't understand at all what they are saying because these doctors mumble and talk so fast, are barely audible, etc. They aren't even considerate for elderly patients who are half deaf (my Dad has a hearing aid but still has obvious trouble hearing). My parents keep saying "what did you say"?, but they don't change. I told them to make the these doctors say it over and over until it's understandable.

I write in my program's Comment Box to the hospital that "dictator is not understandable", "dictator is using cell phone - not audible", etc, etc. Figure once in a blue moon the message might get through. :p

My brother-in-law is a doctor, about 60 years old now, and he told me most students these days don't want to go into medicine because they can make so much more money in business. I feel like, well if you care so much more about money than helping people, go into business then. No sense going into medicine and wind up being resentful and crabby because you want to be making more money. I think the great majority of doctors make well over $100,000 a year, so seems to me that should be enough, but if you want to get really rich, either be a surgeon or go into business. Don't shi- on us and the world because you feel you should be making more money beings that you feel entitled to it by virtue of being a doctor. You make a lot more money than most doctors in other countries. Wish I could say that to a few doctors that dictate. hah :p

they just don't want to "work" for it!

I heard a doctor "complain" once that the clinic he was working for was reducing their pay rate. He was complaining that he could not afford to live on $47,000 a MONTH! HA!

Yes, doctors do make a lot of money. They also have enormous college loans to pay for. And don't forget about the outrageous malpractice insurance they have to carry. Ones in private practice also have business expenses, salaries for staff, etc. And not to get political, but with Obamacare, some are getting out of medicine altogether because they can't make money anymore.

Unfortunately, yes, some do get a God complex and forget that there is a person trying to transcribe what they are saying (or a patient who can't understand them). When my mom had to go to a specialist who had a heavy accent, I had to go with her to translate.

My problem is with the MTSOs who won't confront the doctors about their lousy dictation skills. I think many of them would be receptive to constructive criticism if they realize that 1) a patient's care is at risk, and 2) there is a human being trying to accurately transcribe their words. They've forgotten the days when they had to write all their notes by hand. They need to appreciate how important our job is to them.

Well, again, I just keep putting polite notes in the Comment Box to the hospital, "poor dictation quality, cell phone dictation", etc. I don't know if any of the doctors even see those comments or if it just gets tossed out by hospital staff. ?? :p One Comment Box note about the cell phone to one doctor seemed to get through, though. She still uses a cell phone but it's much more audible, doesn't totally fade out anymore.

About the money, I used to work for an 8-doctor cardiology group, and they all lived in huge mansions, belonged to country clubs, etc. I don't think the specialists like that are having any money problems, even these days.

This is my theory - VR is actually foreigners typing the reports for 1-2 cpl.  When then get it to correct all the mistakes for 3-4 cpl.  This brings the total average of 8 cpl down to around 6 cpl.  They say the programs "learn the physician," which I think means the Indian girl is getting better.  I think it is all a BIG conspiracy!  Sometimes I make me laugh and other times I'm scared I could be right! ...

Doesn't it drive you nuts when a doctor consistently cuts himself off when he dictates?  I sometimes get half words, the last couple letters of words, places where I haves to leave blanks because they say something like:  EKG showed (silence) rhythm.  What kind of regular?  Regular rate and....?  Arrrrggggghhhh! encounters, are they required to have a short course on proper dictation procedure? I would think, being that this medical record is sooo freaking important that we are drawn and quartered if not perfect, that these professionals who generate these reports would also be schooled about proper pronunciation, enunciation, etc. before they throw it out there for us to perfect. Getting kind of sick of these lazy, mush-mouthed, slurred, rapid and mispronounced dictations. ...

Why is it the doctors cannot give you the information you need in the order you need it.  They all know the standards and they will sure raise hell if we deviate.  Would it be too much to ask for just a little consideration in this regard so we are not constantly jumping all over the place?  I mean, heck, their God, God can do anything. ...

To the poster who posted below looking for information on TTD.  They don't have their own accounts.  They work for Superior Global and maybe others.  Superior pays them to find ICs.  They probably pay them 4 cpl and then TTD pays you a cruddy 3 cpl to edit VR reports.  They don't offer direct deposit because Superior is paying them and then they pay you, which is why you have to wait a month for your check.   I would never work for a company that is just bascially an agency hir ...

I recently graduated form CS and I have been looking for a job for the past 2 weeks. TTD offered me a job without doing any testing and I thought that was odd. Why, oh why, oh why can't the students hit the PAUSE button when they are dictating to gather their thoughts?  Why must I sit through listening to page turning, and "thinking" noises?  Meanwhile, time is money in this job ... and I really am getting tired of this!  My main account is a large teaching hospital, and so I get alot of students, residents, fellows ... etc.  <grrrrrrrrr> OK .. that they are dictating for ASR and how to do it, punctuation and all. They need to take some responsibility for this. If they would dictate for ASR, ASR output would likely be better. I base this on the doctors who do dictate everything, including punctuation and the fact the output is good for this doctors. Although, these doctors do tend to speak in a more understandable manner. I spend too much time stopping to punctuate, and I shouldn't need to. ...

Ok, I'm curious, but can doctos do their dictations anywhere? A lot of times, I hear a bunch of doctors in like some sort of dictation room. You can hear other doctors doing dications.
