Did anybody hear about a recent breach of confidentiality by an "offshore partner?"  I got a whiff of this and was unable to verify on Google... just curious if it has happened (again).

Cogent Healthcare, which manages group practices of hospitalists and intensivists working in hospitals across the nation, is notifying about 32,000 patients following a breach of protected health information by a contractor.

The breach affects 25 Cogent physician groups in at least 17 states. Cogent Healthcare learned that M2ComSys, a medical transcription company in India, was not storing protected health information on secure Internet site, and care notes were accessed. âThe access to these notes through the site began May 5, 2013, and ended following Cogent Healthcareâs discovery of the lapse on June 24, 2013,â according to a public notice from Cogent. âIt involved care notes of approximately 32,000 patients from across the county. We are generally unable to identify who accessed the care notes. In some cases, the care notes were indexed by Google.â

Compromised information includes patient and physician name, date of birth, diagnosis, treatment summary and medical history, and medical record number. Copies of patient records and Social Security numbers were not included in the accessed information.

Cogent is offering one year of paid identity protection services from Experian. The company also terminated the contract with M2ComSys and took possession of the hardware, confirmed with Google the removal of all PHI from its files, and initiated a security review of other Cogent vendors.

Genesis Health System, one of the affected provider organizations, believes the breach involves about 1,160 of its patients and worked to issue its own notifications before patients were contacted by Cogent, a company they may not recognize, according to a spokesperson. âWe are disappointed with this lapse in securing protected health information. Weâre reviewing our relationship with Cogent.â

Seeing as the hospital will probably pay out their A@@ to remedy all those lawsuits. I hope this brings back MT to America. American MTs for American patients seen by American doctors paid by American health insurance carriers/taxpayers. Each country should do their own work for their own people in their own country. Sadly by the time MTSOs realize this, American MTs won't be around unless they are paid $$$$$ and then they will be "oh we should've listened to you in the first place, we're sorry, please come back." HA!

Cogent Healthcare offices across the country, Cogent Medical Care, Endion Medical Healthcare (Endion SeniorCare), Inpatient Specialists of Southwest Florida, and Comprehensive Hospital Physicians of Florida were affected.

M2ComSys (M2), a medical transcription company, stored physicians' notes for Cogent Healthcare. It was discovered that the online system that stored the notes could be accessed. Patient care notes with names, physician names, dates of birth, diagnosis descriptions. summary of treatment, medical history, medical record numbers, and other medical information were exposed. The notes could have been accessed on May 5, 2013 and improper access to the site ended on June 24, 2013. M2 no longer provides services for Cogent Healthcare.

I recently took a job as an IC, thinking it would be great to make my own schedule. HOWEVER, I have to work when the work is there. There is no making my own schedule. The work flow dictates when I work. There is no work on Monday or Tuesday. Maybe a few reports trickle in late Tues, but then more work comes late Wednesday afternoon, then work Thurs, Fri, Sat, and by Sunday no work again. Account is for clinic docs, no acute care. Coumadin 30 mg per day. Now, I want to make sure I'm not dreaming here, but that is completely incorrect, right? Maybe I'm having a brain lapse (been an MT for 20 years) but I want to make sure my thinking is correct that this dosage would probably be lethal? For the first time in a long time, I was in the cesspool.  I spent 3 hours with an ESL serial dictator, that was speaking so softly I couldn't hear most of what he was saying, and what I could hear I couldn't understand.  In one of the reports I only got 5 random words after spending a half-hour on it - and it was only 20 seconds long.  What should we do in cases like this - when you literally just Can't Get It, and not for lack of trying.   ...
