Most medical transcriptionists are struggling. You are typing 550 words per hour if you are making that much. Most MTs are not able to do that due to confusing platforms, excessive editting needing to be done, ESL doctors, etc. I've worked for a bunch, and haven't gotten as lucky as you obviously are with these companies. Why not share the names of the places you work so we can all have a chance. I would surely share those names with my fellow MTs without hesitation if I was able to get in with a good company that paid $45/hr.
