If I could put one factor into this, it would be going from analog tapes to digital. If transcription were done by tapes still, the transcriptionist would have to live close to the clinic/hospital to pick up/drop off those tapes. But with digital, it is world-wide.
Here's another dying industry in this - the courier I used to hire to pick up/drop off those tapes/printed work for me. That guy made a lot of money off me and every other transcriptionist in the area! And he saved me a lot of time!
