On one particularly difficult ESL, I put in a lot of time pulling prior reports and researching to try to decipher what he was trying to say. Still, report sent to QA loaded with blanks. I have 30 years experience in the specialty of this doctor, so these blanks were definitely not my fault. The QA person didn't want to deal with it so she sent it back to the pool and I wasn't paid for the report. You would never see this with an in-house job.
