unsure where to pause - mt3

I am usure as to where one thought ends and the next begins as in the part of plantar flexion in the toe at teh extremes....something just does not seem like I have the sentences correct. Any idea what I am doing wrong? tia.

 

Evaluation of the foot does show dorsomedial bony prominence that is tender to palpation.  There is also pain with passive dorsiflexion and plantar flexion in the toe at the extremes.  She is stiff as compared to the contralateral side.

Started a new job this week and I can't see how I can make minimum production due to the long pauses in every report I have done so far.  They must not even have a Pause command in the software, as why would 100% of the dictators suddenly stop using it?  I'm sure others know the stress of the threat of termination with each long pause dimishing their counts.  ...

How would the doctors like it if every patient they saw paused for over a minute between words? Then they complain when we are out of TAT. Can they comprehend that for all the hours MTs are left sitting at their desks not producing because they are waiting and waiting for the next word to finally ooze from the dictator's mouths, they are delaying TAT for that report? I wonder if they are aware how much time we spend working while producing nothing. ...

Does anyone know if EMRs still even have a pause command? Do dictators have any concept that they are forcing us to sit idly without pay while they flip pages looking for lab results, or reviewing a chart? Maybe the MTSOs should advise their clients that if they want their reports back within the specified TAT that they cannot waste hours of our time sitting and doing nothing. ...

Last night I was stuck editing an ESL doctor who pauses so long between words my hourly rate dropped down to less than $2. an hour! I filed & painted all my nails between words, cleaned and organized my desk and sat for an agonizing minutes with nothing to do but wait. The neighbors could probably hear my yelling "COME ON!!!!! SAY SOMETHING YOU ____ _____ _____ _____"   Why do these ... people not even think to use the pause command?  The clients and MTSOs can bitch about TAT ...

I am very interested in taking the classes for coding and billing.  Has anyone taken the inpatient and outpatient classes that Career Step offers?  If so, can you share any pros or cons about the program.  I am also considering another online coding and billing course through Gatlin University, but I am not sure if they offer both inpatient and outpatient coding.  Has anyone taken that program and can you share your pros and cons here too. I want to work outside my home, get an hourly clinic job, but I have worked at MQ/MM for so long now in my house...I am nervous about going back into an office setting.  I'm so comfortable here with my heating pad, getting up when I want to, not dressing up.  Do you have any encouragement for me?  I want to earn more money, because this is pitiful.  It just kind of scares me.  ...

unsure of punctuation and also the last sentence. I typed all as dictated, but think there should be a comma after femur, and no fractures noted as a separate sentence. There is some weakness and tenderness in the supraspinatus portion of the rotator cuff.  She also has s/l with some___ inferior traction a catching sensation over the inferior aspect of her shoulder the glenohumeral joint.  She also gets this too from a neutral to an abducted position of the shoulder.   the s/l sounds like with some and the rest of the sentence doesn't seem to fit correctly. What happened?? With my own accounts I've worked for 100 different doctors and every single one used the pause as a standard courtesy. Today - not ONE single dictator uses it, yet some still keep saying "sorry" when they have more than 10 seconds of dead air time. Do the dictators now consist of a new generation of rudeness or did they "forget" to include a "PAUSE" command in the programs??? ...
