Inconsistent Referral To Patient - Ready To Scream

Couldn't find a definitive answer to this question but it comes up a lot:   One of the PA's I transcribe for is not consistent when they refer to the patient.  They use the patient's first name and then last name throughout the report.  I've been following how they dictate, but think it makes it look disrespectful to the patient to use their first name.  What would you suggest?  Also when the dictator says "patient" instead of "the patient" do you generally put in the the word "the" or do you generally just say "patient"?  I feel that's disrespectful too.

Here's What I do - If names are allowed in report

...then I transcribe first name as dictated. If they ONLY use the last name, then I put Mr. if it's a male and Ms. if it's a female unless it all fits that she's a Mrs. I've hated that stupid Ms thing from when it first came out in the 1970s, but Miss or Mrs isn't appropriate if you're not sure.

...account specs/employer say otherwise, or if it's not a complete sentence and just "patient" would make more sense:

Patient with hypertension.
versus
The patient with hypertension.

I don't care if a report is what I deem - respectful

If the dictator calls patient/the patient John, then Mr. Smith, then John again*, that's what I type. That's my job.

It is not my place to decide what is or is not respectful or to change dictation based on my personal opinion of terms used.

To answer your question - I would suggest you type what the dictator dictates. First name, last name, patient, the patient. Don't drive yourself nuts with this kind of trivial junk.

*Mr. Smith may very well have told the dictator to call him John.

This is the first time I've worked for a company with multiple people doing QA.  You would think it would be as simple as following the client specs, using proper spelling, grammar, and accurately transforming a VR mess into an accurate medical record.  No matter what you do, you will be corrected for their differing impressions of what the client specs mean.  When you then change the way you do something to suit a particular QA person, another QA person corrects that!   ...

I am on an account with the MTSO I work for that I keep getting very inconsistent QA feedback from.  Some say follow the account specs, some don't seem to know the specs, some say verbatim, some add words and expand abbreviations while other don't.  Often words will be rearranged probably for clarity, but the specs say in numerous places  to type what the dictators say their way.  I feel like I am on a QA merry-go-round that will never stop.   ...

So did anyone else receive an e-mail that some unnamed company was looking for 50 MTs, and if you responded, that persone-mailed you back asking to attach your resume so she can forward it to the company in hopes of getting a referral bonus?   This person did not name the companye, so no idea what the real deal is!  Sounds very suspect to me! ...

every single time.  Either 100% or something like 98. something, nothing in between.  No 99.8, no 99.1..  She said I need to take the Care Boost as this under 99% is unacceptable.  I said well isn't the average of 98.5% and 100% around 99.4 or so percent, which is acceptable?  LOL.  But she didn't think it was funny....Oh well.  So, what happens if you don't consistently get over 99% ?   Do they let you go?  Or do they only let yo ...

If the client profile makes no mention of patient name (yes or no) being left in the report, account does not specify verbatim, is the ADHI/JCAHO directive to type the patient's name or replace with "the patient?"  Just checkin'.  Am paranoid about everything with the audits these days.  ...

I swear to all that is holy, this is what this less-than-stellar PA just dictated:   "The patient IS a high-risk gestational trophoblastic disease..."  I went back and listened thinking I must have heard wrong and she said patient HAS ... Nope.  She said IS.  This is the same PA who dictated a creatinine of 92 for someone.   That sound you hear is me hitting my head on the desk. ...

Just FYI....I applied to PF about 3-4 months ago. Took the transcription test, thought I did great, never heard a word. Today out of the blue, got a call that I had done great on testing and they would like to offer me a position.  I said thanks but no thanks since I'm no longer looking for a job. She told me that they are currently looking for lots of MTs due to opening several new facilities. Just thought I would pass this along to anyone who is interested in jumping on and taking ...
