MLS are guessing rather than leaving blanks.. - oh my goodness

Just was notified by my TSM that an account isn't happy.  They feel the MLS are guessing and they have found errors to prove it.  Someone needs to let these clients know how Nuance is enticing some MLS to guess.  They would surely reconsider outsourcing their work and maybe this is the start of hospitals hiring their own MLS again.  Wouldn't that be wonderful?!

I have often thought about sending a letter to the client anonymously to let them know just how badly the people who are entrusted to transcribe their reports are compensated and the very high standards we are expected to keep and the fear of not enough line count or too many blanks. Hmmmm, maybe I will do just that.

Do It!! I would love to see some of these companies taken down - a few notches on the totem pole. sm

It could start a whole new ball rolling, just think of it, some of these companies might actually be held responsible for their own mess. Wouldn't that be great!!!

I'd do it myself, if I worked for one of the 2 worst who were pushing out the crap work.

If you guess, which you should never do IMHO, or even just make a subjective change you will get killed in post audit = no bonus.

If you leave blanks and send to QC you end up sending more than 10% to QC = no bonus.

And now they are going to offer bonus to QC for catching (or creating) more errors = no bonus for MT.

It is a lose/lose situation at Nuance.

Personally, I know I am not getting the bonus ever so I average 15-20% to QC weekly. I am on a horrible account that has a bunch of mandatory QC review dictators and I never guess even if it is the difference between the words "of" and "an" or similar. It just isn't worth a 1.0 error deduction. I can only assume that type of "error" is about to become much more common with QC's new bonus structure.

I would rather they just get rid of the bonus entirely and treat the MT with a modicum of respect.

You just took the words right out of my mouth. I got slammed by QA for correcting an obvious grammar error, and I was dinged for that. However, I also got dinged on the VERY same account for NOT correcting a grammar error because the account is "modified verbatim" and we are supposed to correct incorrect grammar or any other glaring errors as long as it doesn't change the meaning of the sentence.

The latter IS actually exactly what we are supposed to do, which I normally do all of the time. BUT, when I got dinged for doing it, I QUIT doing it and got dinged AGAIN for it.

You're damned if you do and damned if you don't. I also got dinged for changing the patient's age to the CORRECT age after it was dictated wrong (only off by 1 year), and again we were always instructed on this account that we should double check the patient demographics for correct information. I tried to get the thing reversed since I did exactly as I was supposed to have done by correcting the patient's age, but it was refused. I just don't understand how they expect us to do something 99% correct when there is no consistency in our "instructions" per each account we work.

I'm to that point where I just want give up because it's just not worth the anxiety and frustration that it causes to have to do this job, and I have always loved doing medical transcription whether it be the editing part or the straight transcription part... I just hate what the sharks in this industry have done to destroy our pay and try to chip away at our self-esteem at the same time.

Unfortunately, the only thing that'll happen is MLS will get fired and companies like Nuance just hire new people to replace them. Even if the hospitals were aware of how the MLS were treated, they wouldn't care. They would hire chipmunks to transcribe if they could do it without errors.

Seems like you want to discourage anyone from even trying - to do anything that might help? sm

Would you rather see things stay the way they are, as horrible conditions for all MTs?

If someone's got the guts to try something, I say go for it. Each person would need to weigh their own risks and consequences for their own actions.

But if no one ever tries to do anything, how would things ever get changed?

If the MT sends this letter, she/he runs the risks of getting fired. It may get the attention of the hospital but then the hospital will likely switch to a different large national. Even the other nationals that do not offshore ran out of work consistently.

Hospitals go with MTSOs for one reason and that is to save money. If they truly cared about the people working for them, they would have kept their in-house departments. I think if most of us honestly evaluated the situation, the bad-guys would not only be companies like Nuance and M-Modal but the hospitals too who willingly took the bait in order to save some $$$. They are just as complicit in bringing transcription to the state it is in now as the MTSO's are.

What they would really like is for us to do the job for free, but they have not yet figured out how to get us to show up for work every day. Instead, they just whittle away at our stash of peanuts a little bit at a time by saying, do this for free, do that for free and jocking with how they count the lines.

It's the same concept as boiling a frog. If you drop it in boiling water it dies right away, but if you put it in cold water you can turn up the heat slowly and the frog will adapt. So have we as MTs adapted to those pay cuts. Right now we are trying to swim in a pot of 200-degree water and fussing about it. What these companies will learn soon enough is that they can hire, fire, replace us, but eventually they will find they have exhausted their frog supply so that there is no one left to hire.

I relish the day, which appears at hand as I see the desperation for companies like Nuance and M-Modal to hire new people. When these companies ask you to refer someone and you would not refer your worst enemy for employment with the company it is a sign they are on the brink of dire straits as far as new hires.

I recently tested for a company.  The test consisted of two typing reports, one 5+ minutes, the  other 7+ minutes. I agree with the poster below who says they do better without QA hanging over them, leaving the same blanks and just nit picking the rest of the report.... Very infuriating.  However, here I am, stuck on a report with a resident who is speaking so low I have the volume all the way up, then he gets loud for a second or 2, just long enough to blast out my ears, and then I can't hear him again.  I'm only 3 paragraphs into it (its 668 seconds long) and already have 3 blanks where ...

All this talk about guessing is not to the point.  Even if you do not guess, you are going to make mistakes - we all do.  Some of the worst and fastest dictation I get is for the lab data and drugs and dosages which should be the most careful part of the dictation.  The responsibility to see that these values are correct in the medical record belongs to the doctor, not to us.  ...

It really grinds me that we're told repeatedly not to GUESS in order to make the sentences "make sense."  Yet, that's exactly what VR on M*Modal does!  Adds words that were never said, takes out important verbs and (God forbid! an article!).  I spend more time taking out commas that are substituted for "and."  This VR program not only edits, it MAKES WORDS UP!  So now, we have to edit the lousy dictators and the In-*******-Credible VR too.  Only 2 more yea ...

Would rather not say which company, but I am in my initial training period and the lack of support is like nothing I have ever encountered before! I am not a new MT and I am not a job hopper. QC Leaving?? Leaving More Work For MTs?

Sorry QC was put on the grid and suffering losses (like we have), but from my point of view the more of them leave, the more work left for us.  Isn't that horrible we are reduced to feeling that way about our co-workers??? Some QC say they are not allowed to do MT unless the account is behind, some say they are allowed when they run out of work.   From the length of time we have been suffering from OOW I have to believe they can and do jump in on our work when out of QC ...

Utterly confused. I took a big, BIG hit with the QA deduction in paycheck this week. So, I decided to investigate. Like everyone else, I have about 10 "regular" accounts. No blank rule is mentioned anywhere on any of the Account Specifics. I wrote the QA person for my CMC questioning how many blanks were acceptable without sending to QA, and got the "first time right" explanation. This is BOGUS...if I have one blank on a lengthy report and hafta send to QA and they take the total line ...

I got a complaint from corporate that my lph are not where they should be via my TSM. I have been at MModal 6 weeks and just got off training. I was hired in at tier 3. My lph are gradually going up every day but not to suit corporate. I do straight transcription, no VR, on Futurenet. A lot of ESL bad dictators. I am still getting new dictators every day I work. I'm just sayin....as a radiology MT in this company for 14 years...we have always sent radiology reports straight through to the docs to review and sign (because of turnaround time)....never knew they did QA until I got new accounts 2 years ago....and have had some pretty bad foreigners.  I believe it is docs job to go over his reports anyway...he is legally responsible.. ...

It just occurred to me that MQ could have another card up its sleeve in order to knock down our pay, yet another scourge to get us to quit.  All it takes is more than 1 report sent per day to put my percentage of reports to over 5% to QA, which would result in 50% salary decrease.  I just had a report where ALL of the blanks were from horrible sound quality.  Words were missing, the surgeon was speaking from his speaker phone which picked up shuffling papers, you know those s ...

They are more and more following M*Modal.  Now if a report is QA'd with a blank, you will be penalized in your score for that blank, and this has been verified by a QA person, who received the new directions recently.  As I understand it, you are penalized if QA was able to find the blank either in old reports or by research, or just knowing the term.   So you have a choice, spend a lot of time on research and see your LPH plummet to minimum wage, or blank it and then be pen ...

I'm on accts that do not allow any blanks at all through to the cliient.  ALL blanks must go to QA.  I haven't ever worked on an acct that would not even allow one blank to the client.  Anyhow, might not would be so bad but the dictators are TERRIBLE!  Many ESLs also.  Combine no blanks allowed with terrible dictators = sending more to QA.  That means docked pay if 5% or more.  Oh, and don't let me forget, no samples yet.  So you have to ho ...

I just looked at my review for March and I see all these 0.5 deductions for "blank filled in by reviewer."  There are also Valid Blanks that take off no points.   Sorry if this has been asked but did they say they were going to do this?  And if so, when?  Also, I listened to these blanks.  There is no way they can tell me 100% what was said in any of them, which is why I blanked them.  So they are guessing.  Two reports I have gone through so far are for real ...

I have been taught better safe than sorry and I have been a transcriptionist for 15 years.  I am docked .05 for each blank, but the standard 3.0 (CPSE) if I got it wrong it happens to be pullled for QA.  I know this is a rhetorical question as we are all professionals, but would you leave a blank and take the 0.5 right off the bat or risk the patient on a lab value, drug name, dosage or diagnosis and take the 3.0 IF that report was pulled for QA? ...

Yep, way back when they started this 99.6% to 99.7% junk that the clients would be complaining about blanks. ROLF As predicted, their little policy bit them in the butt. Since they implemented the 99.6% to 99.7% requirement, I have left a lot more blanks out of self defense that I normally would not have left, but if it is the least bit iffy that QA might not agree, there goes a blank.
