First year I have gotten one. Where is the supposed $10.00 gift cards? At least they could have forked over one in each person's generic card. Wonder what the suits got.

Six years ago they gave me a tube of chapstick on a key ring. That was the last Christmas present I got from them. I never did identify the flavor. It said made in China so probably cadmium flavor.

I can't remember what the program is called, but we got an email about it. Each abbreviation is given a supply of gift cards ever quarter (I think) to reward those who have gone above and beyond or something like that.

It seems to me that most posting on here about not getting anything just do not know the reason for this season. People, we are grown and should actt like that. What difference does it make if we don’t get a gift from another person? We are not kids here. I work, get my paycheck and am happy. I'm trying to activate the gift card I got from IMEDX for Christmas. I threw away the documentation that came with it that apparently had the "activation code". I called customer service and they stated the only hint they could give was it was the last four digits of my employee ID number. Can anyone tell me if this was the last digits of our last name?, last 4 of SS number? I can't remember my employee ID. Thanks! ...

I worked as an IC transcriptionist in 2013 for approximately 4 months. The company sent a W-9 form and is requesting it be signed and sent back. I did not make a total of $600.00 when I was working for them, which I've understood from others that if you make less than $600.00 you don't need to send in a W-9 form. Can someone clarify on this?
