shortened form for subcutaneous - Jill

Good morning.  I am a court reporter who does many medical depositions and a discussion ensued amongst us whether subcutaneous is shortened to subq, sub-Q, subcu. 

Is there a preference amongst medical transcriptionists on how to write it?  Thank you.

But I am also from the school that it's disc and not disk in all medical instances. This is a big big issue with me and I think some high-up person with influence in the field always spelled it disk and made a big stink about it. Every anatomy book I've ever seen that a doctor would use in their training spells it disc and every doctor I've asked says it is disc. Until I'm shown legitimate proof otherwise and not just the popular opinion, I will always spell it disc.

So, yeah, I'm big against slang and improper terms that become trendy in this industry and my opinion is that it should be subcu since that is the shortened form of subcutaneous.

and I also hate using disk instead of disc. Unfortunately, I have to use disk, unless referring to the eyes in my company (MModal) or I get marked off points for improper word usage. Ridiculous.

Follow the rule in the above post: subcu or subcut. Both are acceptable ways to write the shortened spoken form of subcutaneous.

Sub with the letter Q is incorrect because the word subcutaneous does not contain a letter Q.

The idea that a letter Q could be used here comes from the practice of doctors writing subq by hand. As you can see from the above post, that is an unsafe abbreviation. It should not be used.

They are rather good at that medical technology stuff, after all! (Well, and technology in general.)

Though the inventor of the EKG was actually Dutch.

I would guess maybe the German mediks championed the new technology at the turn of last century better than anybody else, and it just stuck.
It's also abbreviated ECG, which makes more sense and is how it should be. - sm

I worked as an IC transcriptionist in 2013 for approximately 4 months.   The company sent a W-9 form and is requesting it be signed and sent back.  I did not make a total of $600.00 when I was working for them, which I've understood from others that if you make less than $600.00 you don't need to send in a W-9 form.  Can someone clarify on this?
