I work for 9 specialty docs so I type a lot of letters.   Two of the docs have me stamp their outgoing letters with their name.  One doc has a manual signature line.  Our physician insurance company representative was recently here doing an audit and said that is not legal and that the docs must sign each and every letter or at least initial them.  Can you tell me if she is correct?  Is there somewhere where I can actually get some documentation as to the proper procedure?  I want to do right by my docs and keep them happy. Thanks gals.

electronic signatures, the doctor has to fill out some kind of form stating that he is allowing electronic signature and I am sure there is some "legaleze" in it concerning that sort of thing. It is kept on file at each and every hospital, clinic, etc. where he uses electronic signatures.

Ask the doctor's lawyer - or call the local chapter of the AMA. They will know. I worked for an interventional cardiologist and I used his signature stamp all the time - never a problem.

Joint Commission standards for hospitals require that the author of a document authenticates the document -- which does not allow an electronic signature or a stamped signature to be inserted by anyone without the physician/clinician having reviewed the content first. Such signatures are fine, as long as the person inserting them is the one who is legally responsible for the content: the doctor or other clinician, not the transcriptionist.

Stamps will no longer be accepted as signatures for Medicare and I believe many insurance companies are following suit.

A few weeks ago, for no reason that I can find, I cannot get the first letter of a sentence to be in all caps automatically, (unless I do it manually.)  The same for the letter " I " by itself when typed.   It was always automatic and now all of a sudden it is doing this and I cannot figure out how to change it back. Has anybody else had this problem, if so, how did you fix it? Thanks, appreciate the help ...

Co-workers Signing Off Shift Early

Do any of you have this problem? There are several us on one particularly account. They will either sign off 5-6 hours into their 8 hr shift (saying they've met their minimum quota)or sometimes on weekends just leave work not done til the next day. I have the last shift and have to stay until all docs are finished no matter how late. Therefore, I get "mop up" duty every day. Any helpful suggestions? Just typed a letter to a peer review committee regarding a discrepancy in an operative report and a discharge summary.  The doctor gave the explanation that it was simply a typographical error either from his mumbling or unclear dictation, and that the dx listed in the discharge summary did not make sense and he would not dictate that. Yes, he actually admitted it could have been from his mumbling.  But, why didn't he catch that incorrect diagnosis when he signed the discharge sum ...
