s/l high cele lesion of cervix (for LEEP) - dawn

This pleasant 29-year-old gravida 1, para 0, A1, last menstrual period two weeks ago, has a high cele lesion of the cervix. This is very small at 6 o'clock.
Maybe HGSIL lesion ?? - GT

High grade squamous intraepithelial lesion
I have the BOS2 which says the hyphenation should be "a 3-cm lesion," but "the lesion measures 3 cm." I was recently told that the BOS3 changes this and now they don't want hyphens in there at all.
I don't have the money to buy the BOS3 yet, but if there is anybody who does have it, would you be able to tell me what it says about hyphenating or not hyphenating this? Have they changed this now? ...
Doctor says, "At the 12 o’clock position where high grade changes were previously diagnosed, there is an area of arborization that was a little bit atypical in presentation." He is doing a colposcopy but is referring to a previous LEEP...arborization??? Can't seem to find this anywhere and he's got cotton in his mouth, so I can't hear him clealry. Any thoughts? ...
Question is s/l Lugall. He used it twice and not quite sure what he is saying, but s/l Lou-gulll
GROSS PATHOLOGY: s/l Lugall finds take up at the 12 o’clock position. Biopsy slightly toward the 1 o’clock position.

I'm having trouble w/ this operative report...I've never transcribed a LEEP procedure before. At one point he says what sounds like "(s/l blue) guaze was then placed in the cervix. I identified the transformation zone. The green-colored (s/l loop or loof) was used. A blended cutting current of 40 Watts was used to perform the conization." I wasn't sure if the wattage part was right, either. Any help is greatly appreciated!! ...
An ESL OB/GYN dictator on laparoscopy, ovarian cystectomy & lysis of adhesions:

S/l "reticulated" or "articulated".

This is a colonoscopy report and this is the first time this has come up for me. I'm not exactly sure but it sounds like he is saying a type of shelf.
s/l ballooners shelf
He says it fast so I can't be sure but shelf sounds right.
"A rectal examination revealed a mass in the area of the cervix and uterus, questionable ___ shelf."

It is dictated "There is a lesion on top of her head that is leaking some type of material. It looks like a S/L cary, kery, carian or kerian or an abscess.

The physician says "I recommend we do need to complete staging with a PET CT scan and endoscopic ultrasound to determine T-status of the lesion."
T-status is the correct term or can it be anything else?
