When the doctor dictates "sclera" singular and says "sclera anicteric", do you pluralize (is that even a word or am I starting to get as bad as the doctors and am now making works up?) it when you're sure he's referring to both eyes because he referred to "eyes" not "eye". What about conjunctiva? Do you use the plural form for that too in the same case? I'm always unsure on this. Let's assume this is for non-verbatim accounts.
Last question - doctor dictates "PERRLA". No specific letters are given, doc just says word. I was always taught to use two R's. What about you guys?

If it's verbatim, I type what Dr. is saying.
If it is not verbatim, I correct it to "sclerae," if it is obvious that Dr. is referring to both eyes.
Same with conjunctiva/ae.
If Dr. dictates PERRLA I type PERRLA (abbreviation for pupils equal, round, react to light, accommodation), if Dr wants only PERLA, he has to spell it.

a heme/onc doc who abbreviated it PEARLA (pupils equal and reactive to light and accommodation). They all march to different tunes. ;)
For sclera/sclerae and conjunctiva/conjunctivae, what do you type for the eye exam? Do you only type then in the pleural form if the doctor says it that way? Do you automatically make then pleural? This one is confusing because generally, they're examining both eyes unless the patient just happens to be missing one and I would think they'd mention that, so shouldn't it always be pleural? Just curious. ...

No obvious esophageal lesions were found. She does have a somewhat dilated (natonic) appearing esophagus, but I suspect she simply has a low-grade motility disorder associated with her age.

PE on neurologist consult. Pt with spinal sarcoma. Doc is a pretty clear dictator. Not sure if "tesseril" is actually "tests for all" or what. How about "effort". Is this customary in a neuro consult?

When a dictator asked that you insert a normal ROS or PE, then dictates "for the respiratory put ..." I often get dictation that contradicts what remains in the normal template but the dictators do not instruct to delete the rest of that heading. At my company (a smaller one) there are a lot of mistakes in the normal dictations that the doctors request. The way they are created in the first place is the transcriptionist types up the report and sends it to QA saying the doctor would like to use this in the future for his normal, but I don't think they get reviewed by QA. I am thinking for right now I don't really care if I "fudge" on my shift a little here and there. First they offer OT, then no work.
