I had to pend a report for multiple blanks (bad dictator with bad audio). One of the blanks was for a drug, and I had a hunch what it was, but wasn't about to take a chance. Checked FIESA. QC dinged me .25 point for leaving a blank for the drug name they figured I should have got. THEY filled in the blank with the drug name and misspelled it! If I, on the other hand, had misspelled it, it would have been a critical 3-point error for me. Go figure.

It sux! I just got an e-mail from my TL- stating I was below 99% accurate twice the past month and my job is on the line! It's time to scurry to get out of this industry NOW!

I pend a report because I am asking for help, and then it gets QC and I get an error. System is set up for MTs to fail.

Help with the math: How does 100 - .25 = 97.5. Shouldn't it be 99.75?

100 - .25 -.25 = 98.84? Shouldn't it be 99.50?

Little by little they chip away until no one questions anymore.

It is absurd that US medical records are going offshore, yet no one seems to care.

The way I was explained it is:
The shorter the report the more an error is worth. So there is no way a MLS really can figure out her score on any report. It is not just a 0.25 they take off for an error - it is based on how many lines the report is long.... another way to ruin our scores.

drop the score down to like 90% easily. It's freaking unbelievable some of the stuff they will "find" to mark off for.

I just submitted something for correction reversal (after refusing to try for reversal EVER again after the last one I sent in), and it was just plain ridiculous. There were like 8 "found" errors throughout the entire report. It was a medium-length report by the absolute worst of the worst dictators on my account. I was marked off at least twice for making a word plural instead of singular (and I know darn good and well it was dictated plural), and it had absolutely no bearing as to whether it changed the meaning of the sentence or not! It basically stated the EXACT same thing except in plural form instead of singular and made perfect sense...0.25 points off for that x2.

Then, I was marked off for vital sign values that I heard perfectly, but the offshore QA person "couldn't make it out", so changed the vital sign values to blanks....0.50 points off for that!!

I was so furious by the time I got to the 3rd "found error" that I figure my blood pressure was probably at stroke level. Something like 8 "corrections" or "errors" or whatever the heck they are calling them now and my report accuracy was somewhere around 88% FOR ONE REPORT!

I just couldn't help myself... I sent it in for correction reversal simply because it pissed me off... Otherwise, I have just decided that it's a no-win situation, and FIESA is the gateway to hell online. I normally just go in, inital everything that needs to be "reviewed" and log out. Can't win, so why even try.

And if they decide to tell me my "job is on the line" because of my accuracy score, I'm going to be certain to ask if that is a threat or a promise. I'm surely not going to take it as a threat because I really don't like this job enough to feel threatened by someone telling me I might not be able to work for Nuance anymore.

Because my income makes up more of the household finances than my significant other's social security disability, I should probably be worried that I might be fired or let go or whatever because I really haven't found anything else...YET... BUT, I have an escape plan and I'm certain that by the end of February 2014, I will NOT be working for Nuance or even doing MT anymore..NEVER EVER AGAIN. Nuance has ruined what was once something I LOVED and actually enjoyed. I actually LIKED going to work and even working overtime and on my days off. Now, I loathe having to even think about sitting down to do this junk and making a paycheck that is just pathetic.

My teenage daughter said it best not too long ago. She was invited to attend a conference/group type thingy for careers in medicine. It consisted of traveling out of state and actually shadowing real medical residents and doctors in various well-known hospitals. She misunderstood and thought it was something like medical transcription, and when I told her she should be very proud to have been selected, she promptly stated, "No way Mom...There's NO WAY I ever want to do what you do and be so unhappy like you are!" Yep, that speaks volumes about what Nuance has done to me in the last couple of years...volumes...

There was a time when my daughter asked me to go and speak for career day at her school because what I did is something no one else ever went to speak about...Now, she doesn't want to be within 10 feet of ANYTHING that has to do with medical transcription and/or editing because Mom is "always working" or "working again on her day off" (and I quote her exactly when she tells her friends why Mom is STILL sitting at the computer or is ALWAYS sitting at the computer.

My job can be "on the line" for whatever reason they can make up... I personally don't care. I clock in, do my time/lines, clock out. I'm done with the days of enjoying ANY of it.

you'll go crazy!!! It is a game rigged against you. You will be better off to ignore the whole quality gradng system completely and do your job as quickly as you can. If you get a bonus, hooray! But don't count on it.

Blanks - awful

I try to do everything possible to NOT submit a report over the allowed number of blanks. Sometimes I will if I absolutely have to, but those are not often. I will do my best to fill them in and will usually go back and listen to the trouble spots if I am over. I will leave a medical blank (medication, diagnosis, etc) if I am not sure, but will fill in the most reasonable answers for nonmedical blanks. They have taught me that I am better off rolling the dice and possibly getting the report reviewed and marked off rather than submitting the report for blanks which guarantees a complete review. I DO NOT CARE if you are able to figure out the blank. GREAT, but DO NOT mark me off because I could not understand what they are saying. Theoretically we should all be working together to produce the best report, but I abslutely do not feel that we are all on the same team. Do they think constant threats of termination is supposed to motivate us? Do they think making us feel stupid and worthless is supposed to boost our morale and make us want to try harder? Do they think having a 99.4% accuracy rate while meeting line rate and QC percentage is NOT good enough for a bonus is supposed to make us work harder for the next quarter? FOR WHAT???? The pay is sooooo not worth the aggravation and they high expectations. I hate so much how this place has made me feel. Management has created this awful environment, and they do not care enough to fix it. I cannot wait for the day I am able to leave this place...and it is coming soon.

This is a perfect explanation of how things are done at Nuance. I got terminated for the same exact reason. Yes, leave before its too late. Keep your explanation in case you need it for unemployment. I went on unemployment after I got fired cause I pretty much went through the same thing you did.

One thing you don't understand is that the QC1 - who reviews your report is then

responsible for EVERYTHING in that report on post audit. When that report is pulled for post audit, it's the QC1 who is responsible and gets any dings, not you. On the other hand, the corrections made by the QC1 do not count against your pay, bonus or anything else. Your scoring that counts are based on reports that have not been touched by QC1.

Umm, I have had 3 reports over the past year where I was penalized by QA for errors that QC made to MY reports. All these were pended to QC for blanks, QC filled in some blanks (poorly) and these reports were randomly reviewed by Post Audit, who gave ME the error deductions. I could only prove 2 of these and got them reversed.

You couldn't be more wrong . . . a misspelled medication is not a 3-point error and if you've been marked for that in the past, it was marked incorrectly.

slowly sinking here because my quality has always been 98-100 quality, years and years. Now, however, I am expected to maintain 99% quality, on a hideously difficult account, but I must also maintain my LPH. In other words, I have to listen to reports on the fastest speed I can to get my LPH yet maintain some form of quality. The reports go to QA who in turn listen at the slowest possible speed to give errors for not including 'the' and mind you this cannot even be heard unless you ...

Just got an email about getting a new quality coach. This, of course, answers my question of why no emails for the past week from her. I hope she didn't lose her job. She was a really good coach, the best one I've had in the last 7 years, and wasn't one who corrected little things like punctuation in audits or things that made no difference in the report integrity. She is one I will miss. I only hope the new one is also with a strong QA ...

I'm trying to troubleshoot something. Does anyone else have sound-quality issues on DSG dictations? Just curious if I should seriously have this looked at on my end. It's not all reports, but enough so that I need to look into it one way or the other. If I'm the only one, I'll hire someone to look into this. Just wondering if anyone else has had this happen lately. I received a quality audit on 4/25 on my audit, which I passed at 98.3% (had one icky one in there). Then on 4/29 got another email of an audit of 5 reports, did not give a total score, but they were all pretty darn good (3 at 100%, 1 at 98, 1 at 98.75). Then on 4/30, another email, they checked 3 more reports (all those above 98% too). But what is going on, are they searching for something bad? Are they doin ...

Just read an article in the AARP Bulletin that 1 in 7 Medicare patients died or were harmed by their hospital care and the number of patients who die each year from preventable hospital errors is equal to 4 full jumbo jets crashing EACH WEEK. They can't figure out what to do about it. Maybe auditing the employees like crazy and holding them to a 99% accuracy standard and cutting their pay 50% if they don't meet it like MMMMMModal/Q? Sure seems to work for me :) ...

I just got a warning for my LPH being too low, and possible termination if I don't increase them right away. I was taking time to double check my reports and making sure all blanks were filled in, rarely had blanks. With the threat of termination, I have been trying to focus more on quantity. Now I have been going through each report once and hit send. I have seen example reports from MTs who have great LPH, but I have seen the minor mistakes that come with them too. ...

i am on 100% QA and am constantly confused. I am corrected for things, ask my coach. She tells me QA is correct. Then when I change it how she says to I get it changed back to the way I was doing it before~! I love Fluency, i am amazingly fast at it but i think QA will ultimately do me in...for doing what i'm told to do...GRRRRR ...
