I really need some help. I am having trouble trying to prove my case to my manager. I have not been receiving my incentives for a few weeks and today I started doing some research to figure out what is going on. Turns out when I enter the dates in fiesa, I show a totally different percentage than what they show when I go to approve my timecard. I talked to my manager and she cannot give me a straight answer. All I need to know is where is it documented about how to qualify for the weekly incentive and quarterly bonus and the date frames involved. I know it is documented somewhere, I just can't remember where. Thanks for the help!

When the lines are uploaded into Webclock on Monday, October 14, 2013 your Q4 QA score will be visible. This score will be used starting on timecard week-ending 10/11/2013 and will be visible and in effect until timecard week-ending 1/4/2014. This is the score that will be used to determine if you qualified for the Q4 quarterly QA bonus. This is also the score that will be used for the weekly production incentive up until week-ending 1/4/2014.
The Q4 QA bonus will be paid out by October 31, 2013. Below you will find details regarding the QA bonus payout.
1. Bonus will be taxed at the rate of 25%
2. The payment will be a live check if you are not set up with direct deposit
3. The payment will be direct deposit if you are set up with direct deposit
4. If you are on Incentive II you do not qualify for the QA bonus. Incentive II includes the following: Radiology, Pathology, Transcription Leads, QC’s, QA Specialists, On Site Coordinators, and US TOPS & Bayscribe platforms.

Here's the qualifying info. Your TSM should certainly let you know what factor/s are currently disqualifying you.
The Production Bonus (99% QA minimum, based on the previous quarter's score) is calculated weekly (7001 lines minimum/week), paid biweekly with regular paycheck. So you might get it one week and not the next, which I think is actually an improvement over having to get 14,001 lines per pay period, the way it used to be.
The Quality Bonus (99.5% QA minimum, based on the previous quarter's score) is paid quarterly (used to be paid about 20-30 days after the end of each quarter). It's based on total earnings for the quarter, not lines. It's 10% of earnings for 99.5% to 99.9% QA score or 15% for 100% QA.
For both, QC submissions have to be not over 10% and LPH 186 or greater.
