I have come on this site for awhile, mostly because this is a place where everyone gets what you do and can understand what you're going crazy over. Try telling your neighbor the ESLs are getting to you today and you get what I mean. However, since I have become an MME (in October), this has become a not so friendly place. I love doing QA work. I have been in this field for 20 years and feel I can and do help MTs out. When I make a correction, it's to give the MT some feedback, not to make her/him feel bad, and I do take the time to leave feedback to let the MT know why I changed something; whether they read it or not is up to them. I understand the frustration when something gets corrected that shouldn't be because I've been there. At the same time since becoming an MME, I have also seen some unbelievable mistakes made on the MT's part. There is a reason why there is a 98% accuracy rating, no one is perfect, no one, not one MME and not one MT and goodness knows not the voice recognition (oxymoron that it is). Anyway, please realize the majority of MMEs are just doing the job we're supposed to be doing. If something is 100% read, then yes I have to do that and correct anything wrong. If I get a job with a QA marker and I see something else in the report that is not correct, then yes I'm going to correct it. It's my job. That's all it is. I'm not out to get you. I'm doing what I'm paid (not much ) to do. In closing, thanks for reading this and hope this gives a little perspective and please read the client profile!!!

Oh, I was in the QA Dept (back when it was called that) for several years. You are talking to deaf ears here--been there, done that, tried that. We have gotten harrassed by MTs since the dawn of time, and we will until the end of time. Save your energy. I stopped explaining my feedback, because it was not useful. Just make the corrections and carry on. It will be much better for your stomach and your blood pressure (or whatever ends up ailing you). I tried explaining my self for at least two years and it was good for nothing. You have nothing to defend.

Sorry to any MT who doesn't harrass the QA types. I know you're out there and we types appreciate you.

TOTALLy agree with that. Started QA with MQ in 1998 and from that day until I quit MOST of the MTs appreciated QA but there were the few that made our lives miserable, or at least tried. Called the office, harrassed the manager on down. We had to coddle the heck out of the MTs; it was very frustrating. You have to LOVE doing it to hang in there.

Wow! How refreshing! You're so right! I've been leaving feedback for an MT for over 2 weeks now for major errors. Nothing at all has changed. When I receive the work to QA, the same mistakes are still happening. It's frightening because the patients are the ones who end up paying for some of those errors.

Thanks for the reminder Jaybee. I'm sure both sides make mistakes and both sides dislike realizing the mistakes we make. I'm sure we all have a love/hate relationship with the whole ball of was but I thank you for the reminder that it isn't personal on the MME end. We as MTs might take it personal, I know I do. I will work to be more tolerant of the feedback I get.

Sometimes when I listen to the report after feedback/corrections have been made, I can't believe I didn't understand it the first time around! of course, it's clear as day now!. Also, I get reports back and can't believe the stupid things I miss. I have also caught myself making corrections that don't take (hit keys to close together or too fast and the computer doesn't realize what I want it to do so the edit doesn't get made). Those are all my bad, I'm caught between rereading every darn thing I send trying to catch those errors and trying to make line count while stumbling through the horrible dictation/bad ASR.

I would like to put out there though that it is kind of hard not to take it personal. I've found critical and noncritical feedback that is incorrect, I am told that it is incorrect, yet it will always be on my "record" that I made that mistake. Alas, they will never be fixed on the QASAR side nor can any notes ever be entered to show that the error was in error. Those I take very personally as they are part of a permanent record no matter how many platitudes we get to the contrary.

Enough ranting. Thanks to QA (all levels) for catching the stupid and not so stupid stuff. NOTE TO SELF: IT'S NOT PERSONAL LOL

of report is yours, what part is 1st QA, what part is 2nd QA. If you received critical/noncritical, that is not the feedback from an MME. That is from your QC. MMEs only provide feedback. No grading involved. I think if you knew the number of critical errors that we do find you would be surprised. We just treat them all the same -- correct them, provide a feedback note if we think the MT might actually check it out -- and move on.

It would be nice if we could all view this endeavor as teamwork. I felt kinda good the other day when I got an ILP report that had been gone over and about 20 blanks filled in by the ILP QA person and then I was able to fill in the remaining blanks they couldn't get. It was then a complete report with no blanks. But then I didn't feel so good when I happened to see some obvious errors in the ILP QA fill-ins in blanks. I then had to do 100% QA on the report. But still, it took 3 people to turn out a complete report and I thought -- ok, that is what it's about.
But do not take it personally and it is not on your record. What is on your record is your original report as it came to MME, then the version after the MME worked on it. If client complains about anything, those 2 versions are brought up and can be compared.

Here's where MTs are still confused. The AUDITS are what count, not the QASAR feedback.

Someone said this about a week ago here---the supervisors are not interested in looking at every MTs QASAR feedback. They can barely get through their e-mails and phone calls, so they're not looking at the QASAR feedback.

The QASAR feedback is NOT your "record." Your audit is. And even then, as has been said here numerous times, the MME gets audited too. Your audited reports iare pre (repeat pre)-MME. A totally clean slate.

Julie, as others have said, the feedback you get from MMEs has nothing to do with your QA score. It's just feedback and hopefully helpful. In fact, if I see something I think an MT might get dinged on when their QTL does their "official" quality score, I usually leave a feedback note to that effect. It really is just to learn from and nothing more, so official score, not personal, and certainly not something to stress over.

Personally, I appreciate the feedback. Sometimes I see unbelievable mistakes I make, and it embarrasses me. But I also realize that we all get tired and sometimes miss the most obvious things. The only time I've ever complained is when an MME left a note stating that "The MT could have found this easily and is just sending reports to QA because of laziness." I felt that was the opinion of the MME and uncalled for. I always knew that someone could listen to something all day and not "get it," but another could listen to it the first time and understand it easily.

But, again, I do appreciate the feedback. So, you are appreciated by me, anyway. Thanks!

I think a comment like that is unfair. One can think they hear a V sound, but all the while it's a B sound. Meanwhile, the MT indeed spent a lot of time looking for V sounds and finally gave up.

I don't think an MME should decide a thing like that. A more appropriate response would be I found this at blah-blah site then link the MT.

Yes, very unfair, and I was glad that person wasn't in close proximity to me on that day.. My supervisor agreed with me on that one. That's the only time I've ever said a word about the feedback I've been given, but I felt justified in complaining about that one!

Condescending remarks like this cause much of the resentment against QA. MTs have every right to object loudly to the supervisors when receiving comments like this. The QA responsible should be held accountable, and if she continues, be let go.

See now, this is what makes MMEs seem like jerks, frankly, but for every 1 like that there are a lot of others that really want to be helpful. I love the QA markers that are easy to find, they are very few and far between because MTs do a good job at research, no matter what she/he said. Thanks for appreciating the feedback. :)

MMEs had to do this if the reports came in for blanks only and CCM said yes - that they have the right to request a full QA. Seems like a completely different attitude than what some US MTs/MEs have. They invite and appreciate feedback. Don't take it negatively or personally. They rightfully see it as a learning tool.

If you mean by checked that you're getting scored, that is only done by the quality team lead. It usually is only once a month, but it seems to vary among service areas. MMEs give feedback and have nothing to do with the official QA score. If I see something as an MME that you might get dinged for from the QTL, I will correct it and leave feedback, but it still has no affect on the QA score.

Yes, I for one realize you are doing your job and I for one like the feedback. But, I use 4-5 different reference books, usually 1-2 that I really trust not to have to look elsewhere. What I do not like is for the MME to put in their preference for a word. Both of my top word books (Dorland's Medical Speller and Radiology Imaging I believe are topnotch reference books and both have end plate as 2 words, but they always correct it to one word. And air-space disease is always changed to airspace. I panic when I see so many corrections in my report until I start to read it and realize just why did they waste their time. I really don't care if it is in the AAMT book as preferred spelling, I am not going to look up everthing I have spelled one way for over 20 years for 4000 doctors that I know exists in my word books. These are just 2 examples.

Much less have them available. Multiply yourself by 150+ MTs all using their own preferred references. Can you imagine? All of us know there are several ways to spell/transcribe things depending on which reference you use. The only leg you have to stand on is if you are using the MedQuist approved references from MQ Central and the MME changes it to something that isn't in one of those references.

The main problem is that everyone uses difference reference books. The only way to be "official" is to use the ones that MQ recommends, unfortunately. My Stedman's has endplate (end-plate) both ways and also has air-space (airspace) both ways, so it may be a difference in when you learned what you learned and whom you learned it from. Either way, we can't all be expected to have every book or word source available nor the time to look it up. I can only speak for myself, but I go by Stedman's because it seems to be the most widely accepted. One of the biggest problems I see is the programming of the ASR, now there is where any word book would come in handy, dorsolithotomy position, really?, puleez.

Anyone hear any rumblings yet about Open Enrollment?  Last year at this time we were already sent emails to "get ready" for it...so far this year, nothing.  I heard that other companies were making the spousal/partner "penalty" for not taking insurance was going to go up to $500!  We are now paying $100/month...let's hope that won't be the case, as that would essentially be one of my paychecks.  Wonder what surprises are in store for us.  Maybe we won' ...

You all have it reversed for VR.   Those of us that do the VR editing should be paid the hourly wage, not the QA people!  We take this abysmal voice that comes in, turn it into something that can be understood, and is perfectlly fine for the patient's chart, but then you want QA to get a hold of it, pay them $12-15 an hour to make is a pretty report, hey who's doing all the work here!  I want to be paid $12 to 15 an hour to fill in the blanks, put in commas, periods ...

I was on family UHC Choice and now being forced to MS HRA.  My deductable went from 1500 to 6000!!  Max out of pocket from 5000 to 20,000!!  Instead of paying a $25/50/100 copay for my doctor/specialist/ER visits, its 30% after the outrageous deductable.  But they cover routine physical 100% and they contribute $500...bah!  Fuming. ...

Cannot Open Express Scribe Files

I have downloaded free Express Scribe but cannot open the loaded file. The foot pedal will work on the sample file but not the true downloaded transcription file. The file shows in Windows media because of an Ipod. Is this because Ipod is the default and how do I change it. The ES files I cannot open are DVF.
