Flunking audits --- and nothing to do with patient care!!!!! - cannot wait to take this job and shove it!

I spend more time trying to figure out what and whether or not I will be dinged for missing or "adding" 'the', 'a', etc. Getting dinged now for leaving output just as it was dictated. Got dinged for missing 'of' even though there was a note that it was dictated really fast. Got dinged for putting the beginning 2 words of one sentence at the end of the preceding sentence, and I remember that report. I could not figure out what the doctor was trying to say! Got dinged because every tense in the PE did not match exactly! I was passing all my audits before I was moved to this group! I am so sick of failing audits. >:-o. Now I am _supposed_ to be changing this stuff?? I used to get marked down for that! And now I should editing the reports like I did years ago, i.e., making the doctor look good? Editing has ruined my typing speed. If I can get my speed back up, I should just retype all the reports! Having to stop so much is causing me to lose track of the doctor's train of thought.

For missing or inserted words like "a" and "the" or "of," there should be no point value unless it changes the meaning of the sentence or is a grammatical error (like a/an). If there are point values assisgned to those types of mistakes, I would talk to your QC Team Lead and/or TSM. If this is a newer account for you, I would also ask them if the client would prefer you to "make them look good" or just leave it as is. The PE should be changed so that it is all in the same tense. Taking this a little further, if it is too difficult to change or too messy, then I would leave it. However, my best advise to you is to talk to your team leads and TSM.

If we fail audits by such a little amount, such as 99.0 to 99.5, will they really fire us? and if so will we be able to get unemployment? Never had to draw UE so not sure of rules. Surely they will look at the real medical words we know and not fail us for "prettying" up a report, and it is their word against ours as for the life of me I cannot hear what they seem to be hearing. TIA.

I wonder that too, how many reports did they pull before they found a critical error. I only had 1 error and the other 4 reports were 100%, but that 1 critical error failed me.

Failed 1 audit by one critical error, medication dosage that the doctor did actually dictate, but it was incorrect and I missed it. I was told I would be audited again. I was and received 100%. However, I remember mistakes that I DID make and just knew I was going to fail the second audit.

Now either they did not look at the reports where I know there were mistakes, but not enough to fail me, or they are using this 99.7% when and if they want to. I can get 100% for 3-4 audits in a row. If they know it is time for me to go, I am sure I will fail an audit and no longer be good enough. MTs who have been around 30 years or more are failing audits. Doesn't add up. I seriously believe they are now using 99.7 to get rid of people, trying to show just cause so they are not eligible for UE.

If you get canned from a job with a 99.3 score, this is probably a blessing in disguise. At the rates being offered to do transcription these days, time to move on to something better.

I was dinged for the same, failed 3 audits with 99.5 scores, but none of the errors related to patient care. A 'to' here, an 'a' there, all really nonpatient stuff and did not change the patient care. The "miracle" auditor made a comment if I do not understand to leave a marker! Ha not understand, I relistened x2 and still have not heard whatever he/she says is supposed to be correct and possibly will end my 30+ MT career. I miss typing and learning, here we just hurry read and try to proof over noise, blowing noises, sniffing, eating, chewing gum, barking dogs, and some other rather unpleasant noises! If I can make it 4 more months, car will be paid for an I will be out of here!!! thank goodness. Good luck everyone. We need it against the "miracle" ear auditors.

How can I be?  I'm too focused on what I'll get dinged for. Every other day there are updates to account specifics.  How on earth can one remember every little thing for every single account (I do 5)? I don't know about the rest of you but I have very few male doctors who are careful enough to adequately edit normal templates to meet the patient's particular criteria. It's gets pretty time consuming to do it myself, especially when we are not qualified to do this in every instance. Female doctors almost always attend to these details. Can somebody tell me what this audit/QA procedure is now?  I'm so confused with when they are done, how many reports are done, etc., etc.  I had been told earlier that 5 reports would be randomly pulled monthly and checked.  You would be notified you had been audited and the results.  If you passed, another 5 would be done the next month and so on.  If you didn't pass the audit, then a redo audit within 7-14 days would be done on another 5 random reports.  ...

I don't understand how auditing works.  Two of my reports were reviewed, and in each case, a word I had puit in had a line through it, but then it was replaced with the same exact word!  For that, I got marked off, getting only 99.50% or 99.75%.  I wish there was a way we could give direct feedback to the auditors at the time we're viewing our reports. ...

:(   I have tried to give the QA program the benefit of the doubt, thinking it was being done fairly, rewarding us for near perfect QA scores.  I have managed to keep my QA score above 99% every week until last week.  Before last week, I normally have 8 to 11 reports pulled for audit, some short, some long.  Last week I had 1 critical, 1 major, and 2 minor mistakes which dropped me down to no bonus pay.  Not faulting anyone for that, totally my mistakes, legitimate errors.  But....once th ...

I have only had 3 audits in 2-1/2 years. Even though I received notice of scores of 99.6 or 99.7, I did not receive any account of what my mistakes were to produce those scores. A comma? A hyphen? Okay, I may be the last one to know this, but I thought the only QC audits I was getting were the ones showing up on the DQM QA Feedback screen where it says (1 unread) or (how many ever unread). When you click on them, if the report has been audited by your QC, it will have a percentage score.  But that's just for the reports you sent in with a blank. This is silly Not only do we need to follow CP guidelines but BOS and then DocQscribe standards. So even if you have the first 2, the third will get you audit.    Why can't they spend the money and create one set of rules to follow and be done with it.  We work on production now.  checking on previous dictated reports and then double checking guidelines from all three areas ??  and when you send in a blank now I am also being questioned about the audio etc.&nb ...

I got CRITICAL errors on audit for things like:  I typed "I used a translator telephone..." (that is what I heard) and the person auditing changed "a translator telephone" to "the translator telephone" (I suppose she heard "the" instead of "a"). In the physical exam portion of one report, the dictator used "was" throughout except for one time he said "is" and so I changed it to "was" to make it consistent and got a critical error for that. Please tell me how these are critica ...

Does it seem like no matter what you do it is not good enough? Here lately it seems like they only audit the worst dictator. The only reports they have audited for me lately have been ESLs lately.
