Fixing doctors mistakes - ae

Posted: Dec 27, 2011

I am so TIRED of fixing doctors mistakes constantly. I dont get paid for that.  I get paid to pound out reports per hour.  Instead so far today, my first 3 hours have been fixing reports, leaving notes to the hospital....line 10 blank..dr stated..., line 24 blank..dr stated, etc.  These guys have to take some responsibility for their dictation.  They are so messy and haphazard in their dictations.  It would be different if we were getting paid by the hour, I would not have a problem with this. One doctor said ferrous sulfate 3.5 mg.  for pete sake..IT IS 325 mg.  I could see once in a while we all make mistakes, but this is a constant thing.

Fixing/catching their mistakes IS what we are - Excuse me?

[ In Reply To ..]
trained and paid to do. If they were perfect, then anyone off the street could do this job, and ASR would have eliminated your job entirely. Sorry, I am old-school and that is what this job used to be about, catching mistakes that could harm a patient and making the doctor look good on paper, and I was proud to be able to do that.

Saying that, I do think our pay has gone down to the point like we are "off the street" without the knowledge it takes to assure patient safety and to make the doctors look good.

hello, you have a collect call from India, do you - acccept the charges

[ In Reply To ..]
yeah, anyone "off the street" can do this job and what do you mean our pay has not gone down to the point like we are "off the street?" Anytime you see an add offering 7 cpl for IC work, it is a job for anyone "off the street!" That is minimum wage baby.

Anyone on the street could do the job?? - Not by a long shot

[ In Reply To ..]
The training an MT has in medical terminology is what sets us apart from the average person on the street. I agree with the OP on this one. Certainly, when an experienced MT hears something that is obviously incorrect she would correct the error, but our job is not to think for the doctor. Otherwise an MT would sign the report, and not the physician. The responsiblity ultimately lies with the doctor. They make far too much money to be careless.

they are pulling them off the street overseas - little training whatsoever

[ In Reply To ..]
low-skilled, low-paid
that is an assumption - where do you get your information?
[ In Reply To ..]
Sorry, just because someone offshore is transcribing does not mean they were pulled off the streets. That's ridiculous.
and you think that they actually had schooling? - whatever
[ In Reply To ..]
You think that they have to pass a test to gain employment? You think they have to hold the standards we do? Do they have HIPAA training?

OMG, whatever. I can have more fun talking to a brick wall.
Yes. They are working for a US MTSO... - Again - where do you get your information?
[ In Reply To ..]
You can't go around spewing specious information just because your annoyed. You think an MTSO tests Americans, but not our offshore counterparts?

Learn something and pat attention: HIPAA rules extend to every individual employed by the MTSO.

Here, this is from an Indian MT site. Pay close attention to the phrase "Medical Transcription Jobs in India are in great demand. With a few months of Medical Transcription Training, one can start a career as a Medical Transcriptionist."

FEW MONTHS!!!!!

What is Medical Transcription ?

"Medical Transcription is the job of helping a doctor maintain the details of his interaction
with a patient by converting it into a medical record by transcribing the doctor's dictations."

Medical transcription job is a type of BPO job and it comes under ITES in India. India is one of the favorite countries for Outsourcing US Medical Transcription Jobs as well as British Medical Transcription Jobs and even Australian Medical Transcription Jobs. There are several cities in India where Medical Transcription in India is done. Some of those cities include Delhi, Kolkata (West Bengal), Chennai (Tamil Nadu), Mumbai, Pune, Nagpur (Maharastra), Bangalore, Mangalore (Karnataka), Hyderabad (Andhra Pradesh), Cochin and Thiruvananthapuram (Kerala).
Visit Medical Transcription Discussion Forum to see job openings in different places in India.

Bangalore is called the Silicon Valley of Asia. There are several Medical Transcription Companies in Bangalore, India. So it is easy to get a Medical Transcription Job in Bangalore. Medical Transcription Jobs in India are in great demand. With a few months of Medical Transcription Training, one can start a career as a Medical Transcriptionist. A Medical Transcriptionist is also called a MT in short.
The hierarchy in Medical Transcription Industry varies from company to company. Usually it starts from OJT (on the job trainee) who is paid stipend, then Junior MT, Senior MT, Executive MT, etc.

Sometimes people assume transcriptionist's job is just mechanical typing work, but in reality a Medical Transcriptionist requires plenty of Medical knowledge. He will need to refer a lot of medical books or electronic dictionaries like the Stedman's, Quick look, English Dictionaries, and all possible medical reference books. He will need to use his listening skills. He will need to use his intelligence in determining what doctor means as the doctors will not be available to clarify the medical terminology he used. Hence medical transcriptionists are sometimes called as Medical Language Specialists just to point out that it is not a simple job that any Tom, Dick, and Harry can do.

After a junior MT finishes typing it goes through editing by another experienced person called as the Editor. The Editor's job is to make correction on what his junior has done. It should not be forgotten that a medical transcription work involves simultaneous use of eyes for reading, ears for listening, fingers for typing, legs for foot pedal to control the voice player software of the computer.

Internet search engines like Google, Yahoo, and MSN have really made transcription much easier by helping the MTs find the right word by searching through various medical sites on the internet. Also, search on a wrong spelling on these search engines leads to suggestion of the actual spelling based on the importance of the actual word.

After Editing work is finished, it goes to the Quality Analyst(QA)/Quality Controller (QC), who will fill in the blanks or words which could not be filled by the Editor. The Quality Analyst(QA)/Quality Controller (QC) will be usually be a doctor who will use his medical knowledge and skills to fill the blanks. Finally, a Proofer or Proof Reader goes through the whole document checking for its grammatical errors or sentence restructuring before finally it reaching the dictating doctor's table as a perfect record. All the while it is the Production Manager who keep the tab of the job work flow be it a 24-hour TAT (Turn around time) or be it a 12-hour TAT or be it a STAT job. Various companies cut short the number of levels to suit their needs and as per client requirements.

A medical transcriptionist in US is sometimes paid on hourly basis even up to $16. In India, Medical Transcription Job is done at a price ranging from as low as 1 cent to 12 cents per line (65 characters). The low price resulted as some new companies tried to undercut the older ones in the pricing.

Now get Home Transcription work from multiple companies from rates as high as Rs.2/- per line. The boom in Home Transcription in India has helped many Indian Medical Transcription Companies to lower the cost of transcription since they save on overheads. However, there is some worry as to the level of HIPAA compliance and security that can be got on a home transcription platform. Regardless of that more and more MTs are opting to work from home, especially housewives and retired people doing part time jobs.

Indian Transcriptionists earn as low as Rs. 1000/- as monthly salary to as high as Rs. 50000/-. Indian Medical Transcription Professionals can earns in Lakhs if they become successful in getting a direct Home transcription Job from US or a direct Medical Transcription Contract as each Dollar earned becomes upto 50 times when it gets converted to Indian Rupees.

that you need to take your own advice and ''learn something'' yourself (such as how to post without spelling and grammatical errors in every sentence) and the fact that HIPAA laws are NOT enforced in third-world countries, despite your ridiculous claims to the contrary.

[ In Reply To ..]
My claim - which remains correct - is that HIPAA laws extend to all contractors and employees of American MTSOs. I made no such claim for companies that do not originate here in the United States.

As for your criticism of my spelling - you win. I shot off a response and hit enter. Your remark on grammatical errors, however, begs documentation.

[ In Reply To ..]
The employees in the US have to be HIPAA compliant and the OS MTs do not. That was stated in the original post, to which you came spewing out that its not true, which by the way, is true.
No, I did not know that - why would you work for them then??

As I said before, people off the street with absolutely no medical terminology cannot do this job. Period.

I kind of agree with above poster. I have been - demeaned for years now

[ In Reply To ..]
that I am "just" a transcriptionist, not a doctor or a nurse and I should type verbatim and not ASSUME and correct a blatant error. I have been treated and now paid like a data entry clerk - apparently that is what they want.

Fixing/catching mistakes is what we WERE. - The MTSO dumb-down

[ In Reply To ..]
of this profession has changed things, unfortunately.

I'm old school too, and before the MTSO takeover, it was my responsibility to make sure that the dictator sounded professional as well as make sure that the record was accurate. I actually worked for the chief of staff of a hospital who would dictate "you know what I mean" or "make me sound good" because he knew I was more than capable of changing the flow of a sentence or replacing redundant wording without changing the meaning of the report, and he also knew that I and no one else would be transcribing for him. And, just in case I may have done this incorrectly, the doctor would READ THE REPORTS before signing them. Imagine that! When I first started with an MTSO years ago, one of the biggest problems I had was getting used to verbatim and fighting back the urge to correct obvious mistakes, eliminating run-ons, replacing "Jane" dictated a hundred times over in the report with an occasional "the patient" or "her/she," etc., etc.

The MTSOs have made dumbed-down verbatim rules to increase production the standard. They can either get what they ask for and try to pay us peanuts (while we churn out as many lines as possible to compensate) OR they can have us take the time to flag obvious mistakes (and this is usually time consuming depending on company rules) or fix them and start paying us for it... they can't have both. They don't care about patient safety or making anything look good except their profits. Sad.

You go girl, times have changed, this is now - Adios

[ In Reply To ..]
a sweat-shop job from hell, we are not just producing shoes or clothes, this is people's medical records that are important but we are paid based on production at probably the same rate. We don't have time to do the doctor's job. Every time I flag a mistake a doctor makes I cringe because I know I am screwing myself pay-wise.

They are getting it both ways, paying us slave wages and expecting us to still be top-notch "transcribers," what that used to mean.

My pride won't let me leave a doctor's sloppy work sloppy. That's why I have to get out of this profession. At least it used to be a profession.

What we fix - OABO

[ In Reply To ..]
Sure, were are supposed to fix the dictation but these docs now can't even put in the correct report type or the correct patient number. No one has ever showed them where the pause button is so we have to listen to all their extraneous conversations, nose blowing, coughing, etc. Are we also supposed to teach them the rules for outlines - that is, no numbering if there is only one item, the difference between is and are, the difference between diagnosis and diagnoses, that the term for a single nostril is naris, not nare, that the singular for phalanges is phalanx, not phalange, that you don't say paragraph after every sentence? If we try to change all of this plus the incorrect ASR we wouldn't even make half of minimum wage.

FIXING MISTAKES - ee

[ In Reply To ..]
We do not get paid to fix mistakes. We get paid to produce. The doctors have a responsibility to get their dictation correct. I have done this job for over 40 years and, of course, I correct the mistakes. In the past, I got paid by the hour and I could take the time to fix mistakes, now we have to produce 150/hour plus fix mistakes

MD mistakes - LeeLee

[ In Reply To ..]
You go girl! I get that ALL the time due to massive ESLs. I have a hospital that wants to "jump" ahead of the Obamacare and they are the worst. MD stated he gave the patient Coumadin 500 mg q.i.d. (he meant clindamycin; so he said later). Unfortunately, even if an MD kills someone, under the Obamacare law, you are not permitted to sue them for malpractice. Makes it sweet for the MDs that really do not give a damn. It is more like babysitting now then it ever was....been in this field over 40 years. Sad to see what it has become, especially because of Obamacare. READ that law....absolutely frightening!!

Healthcare - Supporter of change

[ In Reply To ..]
The health care law is voluminous; did you actually read it or just listen to Fox News?

Healthcare - LeeLee

[ In Reply To ..]
I actually READ it. I certainly would put Fox News far above CNN, HSNBC in how they informed people of what is in there. I also watched Obama's poor excuse for bipartisanship when he televised his meeting. Obama was very, disrespectful to ALL that disagreed with him in any way, shape or form. There were several plans presented to him that did not completely destroy our healthcare system...he out of hand rejected every single one without any review or discussion whatsoever.

There is a little known clause that if you sell your home (do not have the actual date in 2011 it went into affect at my fingertiips), 3.5% of your selling price is charged to the seller for Obamacare. What does selling your residence have to do with healthcare? This is not a healthcare plan under any definition. I also supported common-sense changes and this was not it. As soon as this was passed, I lost ALL of my hospitalization because it was deemed unlawful under Obamacare. Did not matter that it was reasonably priced, served my purposes for years, and gave me ample coverage, as well as many others in the company. Obamacare replaced it with a plan that was 4 times the price, had tripled what I had to pay out of pocket and covered far less. Needless to say, I could not afford Obama's solution, which was higher than my mortgage. Unfortunately, so many do not/have not read this law and rely only on the Lame Street Media to tell them how wonderful this law was....No wonder close to 60% want it repealed...they see what is coming. Hope this answers your question.
Briefly - oabo

A recent nonpartisan survey by Fairleigh Dickenson University found that people who rely on Fox News are the most ill-informed people in the country. They are not a news station. They are a propaganda network.

I'm sure the Affordable Health Care Act has problems but the system we have now is neither fair, affordable or sustainable as 50 million Americans are totally without insurance and the number is growing every day because of the joblessness.

Also, I believe the 3.5% of the selling price of a house is helping to pay for the payroll tax deduction, not health care but I will check it out.

Anyway, this will probably be moved to the political board.
50 million americans are without insurance - because
[ In Reply To ..]
WE CANNOT AFFORD THE INSURANCE. I pay 500 a month premium and have a $12,000 deductible per year! That is not insurance, that is a catastrophy plan!

And don't forget, people are without jobs and without insurance because THE WORK IS BEING OFFSHORED! There are no jobs here anymore!
And I should add... - because
[ In Reply To ..]
I make 60,000 per year. You take the 6,000 per year in premium costs, the 12,000 per year deductible, and the taxes off the top of my income, what is left? Less than half!!!
Believe it or not - I know that
[ In Reply To ..]
The reasons premiums are so high are several. The insurance companies are cleaning up. Ten years ago 95% of our premiums went to health care with 5% profit. Now it is 80-20. Those of us who have insurance are paying for those who don't so that doctors and hospitals don't bear the burden. Also technology is expensive. The whole point of trying to change the system is that if everyone has to have insurance, the costs will average out and go down. We also need single-payer insurance which is like Medicare and NOT socialized medicine. Believe what you will, but the current system is broken.
You are wrong - sociliazed medicine
[ In Reply To ..]
Take out the illegals...take out the abusers of the system, costs go down. Also your percentages are wrong. Obamacare IS socialized medicine. Go to Canada, see what they pay in taxes(not cheaper and they have single payer). Family member living in Canada waited 18 months for a hip replacement; was given the wrong hip and then had to wait another 2 years for it to be corrected. She ended up in a wheelchair before the second surgery in severe pain. Very, very common occurrence in "single-payer" aka socialized medicine. If that is what you want, move there, live with it 5 years or through a major health situation. I am sure your views would change. I have been to Europe numerous times for extended periods; they have single payer many places-NOT CHEAPER. Pick up a paper and read what is going on with their health care now. The govt determines what care they get, when and how much. Live there, use it and then you can write intelligently about the subject.
