Reduce Services Provided? - FairCompensation

Posted: May 21, 2012

Those of us working from .07 to .08 cents per line with a service are making .03 cents per line less than we made 25 years ago. How many of you have reduced the services you used to do for clients to compensate for this gross reduction in wages? Is that an option? Should we stop doing ANYTHING but pounding the keyboard, afterall, that is all was get paid for. The rest of our services are unpaid: research, verifying patient data, keeping spreadsheets for the client, phone calls, installing software, tweaking software, working with their IT and working the bugs out of their software, etc. How much time do we really spend transcribing with fingers on the keyboard? What if we decided to ONLY provide that service, transcribe verbatim with blanks and be finished with it all? I know it goes against an MT's grain to work that way but perhaps that would bring about change.

It is the same as having your own clients, all inclusive - typing for less and less...

Your pay is all inclusive in the cpl, just as having your own clients, and is included in the cpl. We do not charge clients for document setup, hiring/training transcriptionists, editing behind, software installation, dictation system setup, equipment installation, advertising, etc., and definitely not for researching terms, interacting with platforms, etc. I get paid considerably less than even 11 years ago when I first started transcribing --on the same physicians and more. At least you are doing better than us who have our own accounts with respect to this. We have larger overhead costs while the average transcriptionist just purchases a foot pedal and headset.

There is a difference between IC and employee status. sm - IC versus employee

As remote employees, we do more than just purchase a foot pedal and headset. I used to be self-employed and deducted expenses...cost of computer, electric, heat, internet, phone, insurance upkeep on my home office, not to mention electronic word books and shorthand program. As an employee, I find it difficult to deduct those items. It was a much better deal being self-employed. JMO

It costs about the same these days for ICs as it does to work for a service. The only thing service ICs save on these days is printing and pickup & delivery, although that was my best tax deduction as a contractor.

Obviously you are not paying for a call-in dictation system with 96 dictators. These dictation systems are expensive and have to be replaced every few years as well as the computer it resides on. In fact, during the last 7 years, I have had 5 dictation system. Then add the several phone lines, yearly service contract, fax lines, cell (as the clients always need to get a hold of you), etc. Trust me, it adds up. When I was dealing with tapes, it cost $200 per month in courier and now I pay at least over $1000 per month (not including the replacement costs). Also, when you have your own accounts, you have to have backup equipment and computers. advertising, websites, your own FTP folders, two of each. The costs of hiring, training, and editing behind any subcontractors needed, etc. It is more expensive than transcriptionists realize. Account setup: Some accounts can be very simple, and others are extremely extensive to include thousands of templates. The one thing I can say is I have control over sound and dictation habits. It can cost up to or more than $60,000 to train 1 transcriptionist, considerably more on specialized accounts.

LOL. You mean our 14-foot sciff with a broken motor.... full of rust, etc., since my wages went down. Thank you EMR and offshore.
I wouldn't chance getting that big - Angie

I've only had call in systems for 4 dictators with other clients picked up by courier. I've only used one sub at a time and know it takes a lot of training before they start saving you any time. It would scare the crap out of me to expand more than that in this business, but I have to hand it to people who have the guts to do it. My post refers to the majority of ICs whose expenses are about the same as working for a service now. The problem working for services is that after investing a LOT of time to computer equipment that is compatible with their systems, the work is not even guaranteed and the pay is literally half the line rate I charged in the 90's and with consideration for inflation - we might as well be on welfare.
expenses, IC and other - just me

After having my own accounts for several years and several subcontractors working for me, my largest account went EMR directly after I trained 9 MTs to cover me for surgery. I worked for 29 different services (only 2 at a time most) before building my client base back up, and my heart goes out to the transcriptionists working for nationals. I did not even get paid from 26 services! I had no time to chase the pay either, or I would lose everything. I know what they mean when they say no work in the queue. By the time you log on, they are gone. Cheating MTSOs, accounts that are severely cherry picked, and the worst dictators (most of them English-speaking) that you could imagine. I cannot believe the MTSOs do not address that, as I definitely do! No cell dictation or VOIP is allowed, and if by chance they do, then they are charged accordingly. And the expense of starting with each service.... only to have the clients disappear and start all over. Yes, I would not change places. I do miss having time off every once in a while but other than that. No way. I definitely could not make as much money. At least I have control over audio, which is very important to me.
I can certainly sympathize... - Angie

I'm so sorry they did that to you (going EMR after training 9 subs)! It has been my experience, working as an IC for 20+ years, that we are not given the same consideration as employees because we just aren't seen and are forgotten. No notice before vacations, switch to EMR, you name it. I had one client that was my full time job I paid a courier to pick up who approved going digital (my expense). Took me a year to get all the equipment and after installation in their office they tell me that a new group is taking over the clinic and they don't approve of going digital.

Shorthand No Longer Being Provided
Dec 05, 2012

The Web/Nuance part has been told next year we have to buy our own Shorthand license.  I know Transcend downloaded for me (and am still using at my PT job) shorthand licensed to MDI way back when.  Will we get to keep that?  If part of Nuance (Trans/Nuance) gets to have it provided free I would think they could figure a way to let the rest of us have it also.  ...

Which Pays Better? IC Or Employee With Equipment Provided?

I've got several options out there.  I was with MQ/TL for 12 years.   Transcription experience 25 years.  I'm getting a little freaked out about learning a new platform.  I don't even know Microsoft Word!  I did manage to muddle through it to type a new resume though.  Not too hard.  How much should an IC charge per line to cover taxes, expenses?  Any IC advice would be helpful.  Thanks. ...
