How to charge for EMR work - DianaB

I currently transcribe from home for a local ortho clinic. When they switch to EMR and I can convince them to keep me to transcribe, what is the best way to charge for my work? Per patient entry (how much?) or still do the per line thing using cut and paste from Word. I need to keep it simple and relatively cheap in order to try to keep their work. I certainly do not want them thinking they should be able to do their own starting at a keyboard while in the exam room with patients or hiring someone to trail them around and put the stuff in the chart for them.
Also, I am assuming one can cut and paste from Word, as that is the most efficient and has all my shortcuts. Any advice there? I do not know what system they will be using but am trying to convince them ahead of time they cannot do without me.

[ In Reply To ..]I am in a similar situation. Working with local ENT docs who are switching soon to EHR. The head of the clinic suggested he may be per patient visit, which I think is fair. We decided to count up a group of visits from the invoice (for example, an entire month ..yes, this took me a long time)and then see how much I was making per patient (total lines divided by total lines) and that came out to about 7 bucks per patient, so that's what we will be doing. Good luck!! I too hope to cut and paste from Word into EHR but there are places where it can't be cut/pasted and so counting lines wasn't going to work. Probably the same for you. [ In Reply To ..]With the service I work for, I type the report in Word, and then cut and paste into the client's EMR so it is doable for at least some systems. My MTSO bills her client per line as normal.

[ In Reply To ..]Sorry if I am asking the wrong question. I worked for a practice that went to EMR and did not think transcription could be done at home anymore. I know that some of the ARNPs are not happy with it because they are not as able to see more patients and it has slowed them down by having to enter the info in the computer while in the room with the patient. How does this work with doing the work at home and EMR. Are there a lot of physicians doing that? Any information would be helpful. Thanks so much.

[ In Reply To ..]I've been doing this for about a year. Hospital made the docs use MM, then Dragon. They realized they were spending too much time on dictation so I've slowly been getting them back. It is slower because you have to pull up each patient chart and cut and paste from Word. I use a computer the hospital provided, which is encrypted. I am not allowed to use my personal computer. It's just great to see some docs going back to the old way. It probably won't last as I'm sure the hospital will come up with another way to cut out transcription.

[ In Reply To ..]With my facility, the doctors can dictate directly into the EMR, and I then log on to the EMR to access the dictation and type the report and copy and paste to the record. I'm sure every system has different features though, so it may be that your practice's system is set up differently.

[ In Reply To ..]I have access to the hospital's system and do it from home using their encrypted computer. You would have to check on security issues. The doctors like it because it saves them time and frustration. They were using Dragon. Hope this helps.

[ In Reply To ..]I work from home and type into the system from my computer. Our office uses Medent. We type into it just as if we were typing into Word. I get paid by the hour. The office I used to work for uses a different EMR system and they don't use transcriptionists any more so I'm sure it depends on what is being used.
