Hello!  I am a fairly new MT (2 years experience) and I am currently doing IC work.  I have been approached by a physician (that I used to work for as a medical assistant) to do her transcription for her as she has gone out on her own and has a private practice now.  It is a family practice, so pretty basic office visit notes.  My question is what should I charge her per line?  OR should I charge per report, per page, or what?  I'm not sure yet that her EMR program has anything in it that would calculate the lines for me and I would hate to have to try and count them out each and every report :-(  Her nurse/manager is currently doing the transcription and said that her visits are typically 2 or 3 pages in length, but she uses a lot of templates that I would have access to.  As I said, I am new to this and have never worked for someone on my own before.  Any ideas, comments, hints, suggestions, etc. would be GREATLY appreciated!

I have both a page rate for my clients and a per line rate. The line rate for office transcription is 10 cpl for a 60-character line (with bold counted as 2, underline as 2, etc. counted with Sylvain Gold IV, which counts headers, footers, spaces, etc. For page rate, as follows, which if in Times New Roman 12 point font equals a little over 10 cpl at $4 per page, which the physicians really like:

Page count is as follows: Headers/footers are included but stops at end of text before signature line. On the side of the document is a ruler. For 1-.3.4 on first page is ½ page (2nd page on is no charge for these measurements), 3.5 5.0 is considered ½ page on all reports, and 5.1 on is considered a full page.

I am in Southern California, Newport Beach area (extremely rich area) and if I go above that, then I lose them.

I pick up tapes from him with the narrative portions that go into the EMR. The office staff or the doctor does the point and click stuff on site. I have access to his EMR from my home. Since the EMR has no way to count lines or even a spellchecker, I transcribe them in Word, then copy and paste into the EMR. I am thinking of going digital with him, so he'll dictate, upload to my website (I'm thinking of doing Docsonline which cost $30 a month for a 3 physician account, so room for growth). Since he is an interventional cardiologist with a thick Jordanian accent and I actually can understand him, I charge 0.14/65 character line with spaces. He is happy as a clam at high tide with the price and my service. If I were you, I'd ask for a look at her EMR and you and she can decide which parts she is going to be dictating and where it will actually be entered into the EMR. If you'll only be doing the narratives parts, 0.12/65 cpl might work. If she also is going to be dictating the point and click stuff and having you do that too (which is more time consuming), I'd charge 0.14/65 cpl. Start high, as there is always room for negotiation. Don't forget to spell everything out in a contract, like when you will invoice and how long you expect to wait for the invoice to be paid, etc. Best of luck.

I think you should give options. I would give options for a salary for: A) Transcription with schedule of desired days and hours.
B) Transcription with schedule that includes on-call status - with only exception of prior agreed holiday, meaning you and her have agreed she will know "X" amount of advance time of you are unavailable, as in vacation.

None of my private clients pay nearly the amount that has been suggested; however, I have no idea if I am comparing apples to apples for the same services provided. The first thing I realized when I got started was that I asked for A cpl, and after the laughter stopped, I was told the client had been paying B cpl to the previous MT. I agreed to it on a limited basis to see if it was a liveable wage. Some number of years later, with no raise in the interim, I am quite happy at my 7 cpl for that client because I routinely make between $30 and $45 an hour for that work with a loosely structured term-around-time and minimal interaction with the client. When word of mouth spread and I was asked to bid on work for his peers, I negotiated an identical cpl for a 3-day trial period. They were happy, and for that client I average $40 to $60 an hour. Now I have another client based on word of mouth with a peer of the first two doctors.

The message here is this: Look at what it costs you to do your business. Figure in your overhead, calculate the TAT, decide on a liveable wage, ask what they paid their last MT, and give it a 3-day trial. Ask yourself where this client could lead you in terms of referral business. People on this board can offer suggestions, and that is helpful, but in the end, it is what you decide that you end up living with.
