Confused by the term EHR - I thought we all typed EHR

I see a lot of contempt on here for the phrase EHR. But I type in futurenet and on the start up screen it says EHR with the word electronic health records underneath. and yes i type not edit.

I thought as trancriptionists in 2013 we were all processing Electronic Health Records.

What gives?

Similar to the way "outsourcing" and "offshoring" are often confused.

Point-and-click (including EPIC) really is a "part" of EHR that does deserve contempt from MTs (and patients, IMO). But not EMR/EHR itself overall, like you were saying.

EHR - combination of all records including hospitals, different consultants, PMH (personal health record that a patient maintains, which companies like MIE have). These are being maintained 'in the cloud' and there are companies like Praxis who offer this...

EMR - record maintained in your doctor's office, or hospital at that site.

If you have some free time, google Praxis and watch some videos explaining EMR/EHR and what they offer. since healthcare is changing incentives to achieve quality (meaningful use) in care, there is a race/push for companies like Praxis, MModal etc. to develop EMRs/EHRs to use for standards.

Just google Praxis.com or Nextgen, MIE, etc. These programs are even so sophisticated, they know what normals are and will alert a doctor say if a lab value is abnormal, and it even knows 'normals' for all the different diseases, like diabetes for example. Or if the doctor enters an incorrect medication, it 'knows' and will alert the doctor, based not only on the current report, but based on the EHR (the entire record) of the patient's history. It 'knows' the difference between
loratadine and ranitidine, you know, medications that can get mixed up in speech recognition.

MModal has, not only speech recognition, but speech 'understanding' which not only 'thinks,' it will also put information where it belongs in a report, and also send information out, like labs, medications - it is fascinating.

With all this progress, however, and the government behind it, it would be nice if they would not only acknowledge the work MTs have done, but offer each and every one of us retraining for free.

For MTs, the problem seems to be that healthcare is not nationalized yet, and I mean one way or the other, it should be finalized. However, when you start learning about how much money and time these companies are putting into this, it seems virtually impossible healthcare would not become nationalized.

Perhaps rather than trying to hold it back, the people against it should get on-board to engage and add their ideas. I think they are fooling people into thinking this will go away, and go back to how it used to be. There is actually a website you can go to, to become 'involved' in this process...can't remember off-hand.

Anyway, that is the difference between EHR and EMR - they are basically the same thing only one is kept in your doctor's office, (on his/her server) or if you are in a hospital, and the other is a combination of all your records from everywhere, maintained in the cloud for access from your doctor or hospital, and even yourself. Your doctor will be able to access (securely) immediately the specialist he sent you to while you are in his/her office, and see exactly what was diagnosed or prescribed, rather than waiting for the report (at least once are all using EHRs). Think about the better quality of care, and also how many more patients a doctor can see and spend time with. Ideally, it would be better.

There will be options for a patient to also access this record, albeit only what a doctor wants you to see, or only what you want other people to see.

Companies like Praxis, NextGen, MModal etc etc develop EHR programs, and each of these are a little bit different from each other. Go to their sites (or google EHR companies) and you will see how fiercely competitive this is right now... see how the charts work and you get an idea of any clerical future in healthcare.

Basically, the gov would like standards. It makes sense for quality purposes...I get that. But I still wonder how 'man' (mankind) will survive its own insignificance.

Too bad we never had advocacy...and my belief, it is because we didn't stand together from the beginning. We all sold each other out (the old AAMT - and whatever they are called now - I don't care because I lost faith in that organization long ago).

Medical Transcription has no voice. Years ago, my thoughts were that we should have joined right away with our overseas MTs and worked together - we could have been so strong. We could have educated them to demand more money, etc.

profession that has been run over by the advancement of technology - you must hang on for the ride or get left in the dust. I believe only the youngest and most ambitious of us will be able to handle the bumpy ride. I'm an old-timer and am still shaking my head and crying in my teacup over what I saw coming but could do virtually nothing to stop. We were once highly respected, well-paid, and integral members of the health community. A little achknowledgement would have been apreciated.

I was never a member of AAMT or knew much about it - would you care to explain further why you now feel the way you do? Just curious...

because I remember the push from them for us to be called 'medical language specialists' and posters we had hanging up in our office. we went from typing pool' to MLS, and as you mention and were becoming respected. AAMT back then had a lot to do with that.

With the onslaught of overseas MTs, they seem to have shifted their loyalty. That is my opinion.

Once they were advertising a discount for getting a CMT (I never could afford it), and when I inquired, I was told it was only for overseas MTs. I would have to pay full price because basically their hardships were worse than mine.

Really, I think waiting around for someone to "recognize" you for work well done and to offer you retraining is fruitless. It isn't going to happen. It doesn't need to happen.

You can get retrained on your own. There are many sources of assistance.

Many of us have already done it. We know that it is possible.

The longer you wait the worse it will be. You should start looking at other opportunities now.

I'm always asking myself, well, what do you want to go into? and I just can't think of anything. I love doing genealogy but I need something that will pay the bills and provide insurance too. I don't want to be a coder. I just don't know what to do. I think I'm hoping to inherit!

when I went to college I majored in MRS. All I ever wanted to be was a wife and mother. I'm a product of my generation. Then when I became an MT I hoped I'd meet a nice doctor. I never dreamed I'd end up supporting myself till I was 37, when I finally married. And now (almost 20 years later) I'm just to tired to go back to school and don't really want to work anymore anyway.

OK I get NJA, change Schedule Source to work availability, then I get a job 10 minutes into it, now what? I know I don't get paid for 10 minutes of NJA/work availability. Clock out and back in under production? Will I get in trouble for the 10 minutes of work availability? Then if I get NJA again, clock back out of production and back into work availability and SEE if it goes the full 15 minutes??????????? I need a tutorial on this I guess. Any suggestion ...

You have to punch out of production to change to "work availability" and I was under the impression you can't type while in "work availability" so you have to clock out and back in under "production" if you get a job during that 15 minute wait period. Would you take an insurance transcription job at WorldWide Transcription that pays .06 per line, has lots of work, and offers a M-F schedule. The only con I can find is that they mail paper checks, instead of direct deposit. Or would you take a transcription job with Integrity HD. I am just venting!!! Somewhere along the line I guess I misunderstood the whole concept behind VR. I have a real hard time understanding why a bunch of words, many that don't make any sense or even sound like what is dictated, in no type of formatting constitutes a report; VR dictation that needs to be restructured and pretty much retyped but yet I get paid less and required to do it faster than a real "typed" report. OK....just to be sure I understand this.....if I am an IC, do I deduct approximately 15% (like what my previous employer deducted for SS and Medicare) OR do I deduct 30% for my part and employer part?
