Everybody keeps talking about getting bought out by Medquist. Medquist seems to be taking out the field left and right. How dominating is Medquist? And if a lot, would the government step in to discourage a monopoly?

One would think that the government would get involved ... but not in this case. Medquist is totally under the radar. Another element that *might* be coming into play is that Medquist isn't an American-owned company. They are owned by CBay, an Indian-owned company so to make it worse, they are buying up American companies and taking the business and the accounts to India, where their employees are very well-paid with lots of fringe benefits. Medquist is a very, very controversial company.

work at home MT thing was just taking off with C-phones and computers and WordPerfect. Through a fellow MT I heard about a company called Transcriptions, LTD. They had regional offices in every state,and specialized in the big university hospitals in their regions.

The lady who ran the office in Richmond informed me that TL's business model was to acquire every small transcription company they could get their greedy paws on.

Fast forward to today. TL is now MedQuist, and their plan for corporate domination has not changed. Likely we will only have 2 players left, at least in the home-based industry.

That's exactly how it will end up: 2 players (Nuance and MedQuist) who hate each other and want to win the race to send all transcription offshore. Their strategy: Use the Americans temporarily to train the speech engine, then let the Indians edit the work and fire the Americans.

You're right about everything you've said except for one thing. I'm an MME with Medquist and spend at least 1/3 of my day QA'ing work from India. That's right ... you heard me. I'm literally teaching and training our Indian counterparts to become better transcriptionists.

And did you know that you dont get paid for the qa markers - you fill in? No pay for those additions.SM

I edited for about a year, the work coming from overseas was pretty bad. The interesting thing was that the MTs NEVER got any better. One of the big problems was simply that American English is American English, it is not the same as Indian English, a lot of idioms, slangs, etc. -- expressions that we don't even realize that we use, don't translate well, they don't understand what is actually said and attempt to make sense of it, and in the process utterly change the meaning of the report. Also, the Indians really are not smarter and better educated than we are; plus the good MT is a rather odd duck and not everyone who wants to do this it, can do it, and being a ESL foreigner does not give you a special knack for this. Most of the work just isn't good, and frankly I don't think it is ever going to be any good.

will want to go with one of those two and the companies that are big enough to have a lot of transcriptionists and have technology will still have a lot of business. I am not sure who they all are, and I don't think there will be many, but if I had to guess, I think it will be Transcend, Keystrokes, Future Net and MD-IT, just because they also have their own technology. Other than those 4, I don't know of any, but I could be wrong.

God help us all if it is just MQ and Nuance.

That the hospitals will take transcription back. The work product quality of these mega-companies is really not very good, and as these monopolies grow, they will raise their prices -- so much so that it may be very advantageous for the hospitals to drop them. Anyway, that is what I am praying and hoping for. As for me, I just wish I could retire and get out of this mess.

I think the MT profession is a very underground thing. Hardly anyone knows about it. When I tell people what I do, it's either somebody who works in the medical field or knows someone who is an MT and already knows about it or people who have never even heard about it before. Never any in between.

But, I know a lot of people would be PISSED if they knew their medical record was being seen by foreigners and being EDITED by a foreigner.

Perhaps we should focus our energy on telling the people where their medical information is going, and public outcry may be enough to do away with the outsourcing.

Outsourcing is common, but I think a majority of people would much prefer their medical records be kept in the U.S. That is sensitive material that they'd rather not send to another country where prosecution of handling this material inappropriately is wishy washy.

Over the years I have been in contact with "active" members of the "professional" organizations like AAMT/AHDI, AAMRA, etc. I have written letters to the journal editors, etc. imploring them to begin a public relations campaign to educate the general public. They kept saying that "something was in the works," but it NEVER materialized.

One thing I have learned that I did not realize back then: The medical industry is completely controlled by the government. The feds WANT this to be taking place or it WOULD NOT BE HAPPENING.

That is a stark realization, but it's the truth.

Certainly nothing new to be confused over QA, but this one is really confusing me. Perhaps I missed the memo. I had mL changed to cc by QA. Yes, dictator stated cc, but I surely do remember being told to use mL instead so why was it changed to cc....not sure. The other change was 12 mm changed to 12-mm. OK I get NJA, change Schedule Source to work availability, then I get a job 10 minutes into it, now what? I know I don't get paid for 10 minutes of NJA/work availability. Clock out and back in under production? Will I get in trouble for the 10 minutes of work availability? Then if I get NJA again, clock back out of production and back into work availability and SEE if it goes the full 15 minutes??????????? I need a tutorial on this I guess. Any suggestion ...

You have to punch out of production to change to "work availability" and I was under the impression you can't type while in "work availability" so you have to clock out and back in under "production" if you get a job during that 15 minute wait period. I just took an online test and I found it rather easy and they said I didn't pass and then suggested more training. WTH??!! I have over 20 years experience and maintain a 99% QA. Would you take an insurance transcription job at WorldWide Transcription that pays .06 per line, has lots of work, and offers a M-F schedule. The only con I can find is that they mail paper checks, instead of direct deposit. Or would you take a transcription job with Integrity HD. I am just venting!!! Somewhere along the line I guess I misunderstood the whole concept behind VR. I have a real hard time understanding why a bunch of words, many that don't make any sense or even sound like what is dictated, in no type of formatting constitutes a report; VR dictation that needs to be restructured and pretty much retyped but yet I get paid less and required to do it faster than a real "typed" report. Does BayScribe only count the total time you're actually 'in' reports as the time that's weighed against your server time? OK....just to be sure I understand this.....if I am an IC, do I deduct approximately 15% (like what my previous employer deducted for SS and Medicare) OR do I deduct 30% for my part and employer part?
