I've been an MT for over 5 years and have always been paid by the line. I would think that most MTs preferred wage is to be paid a set amount per hour. If this were not an option, what has given you more of an income, paid by the line or by the AUDIO hour? It's beyond me why anyone would do VR for 0.03 per line or straight transcription for 0.07 per line, even 0.08 stinks. Does getting paid for spaces add about 2 cents to line rate? I was getting 10 cpl for an account (not paid for spaces) and then lost that to offshoring. I have decided I will not accept any MT transcription rates lower than 9 cpl (in an employee position) or 11 cpl (in an IC position). For medical editing, I'm not going to work for lower than 50% of the transcription rate. For me, it will just not be worth it to do it for less.
