I am finding that I type what they say and then they correct it. Now I know we are supposed to correct some grammar errors, but this is getting ridiculous. We are documenting, not writing a book.
I put medications listed and they put it in paragraph form because that is how it started out in a paragraph. I put numbers, they take them out, I indent, they make it flush and so on and so on.
I am at the point to just delete the reports and not even check them, because it ticks me off. I have yet to see an error that would affect patient care.
