Put yourself in the patient's place. (I want everything on that page to be right so my medical record is only seen by MY doctors. Also, I want my insurance company to pay the bill, which they won't if dates are off or an H&P is coded as a consult if the consult wasn't requested for example.

I do understand where you are coming from though. I hate that MM doesn't believe in humans making human mistakes and penalizing us for 6-plus months for them.
