You don't have to tell them why you are getting out of the field at all, they probably don't care. Most on line applications now want you to attach you resume, so just copy and paste it from Word, there is no need to fax or mail anything, they provide plenty of space for references and necessary info. It isn't even necessary to list references until you are asked, "furnished on request" is sufficient on a resume. It is okay to discuss salary, at some point in the interview it is going to come up.
